  • PERINATAL ASPECTS OF CESAREAN SECTION IN FULL-TERM PREGNANCY: A COMPREHENSIVE REVIEW

    Cesarean section (CS) remains one of the most frequently performed surgical interventions in obstetric practice, particularly in term gestations. The influence of CS on perinatal outcomes in term pregnancies continues to be a subject of rigorous investigation and academic discourse.Aim. This review evaluates perinatal outcomes associated with cesarean section (CS) in term pregnancies (37 to 42 weeks gestation), with particular focus on comparative maternal and neonatal health parameters. The analysis examines both immediate and longitudinal risks and benefits of CS relative to vaginal delivery.Materials and methods. A systematic examination of scientific literature published within the preceding twenty years was conducted, incorporating clinical studies, cohort analyses, and systematic reviews. Selected investigations met the following criteria: (1) evaluation of term pregnancies (37-42 weeks gestation), (2) comparative assessment of maternal and neonatal outcomes following CS versus vaginal delivery. Data sources included PubMed, the Cochrane Library, and Google Scholar.Results. Cesarean delivery in term pregnancies demonstrates association with multiple maternal risks including postoperative infectious morbidity, hemorrhagic complications, and prolonged convalescence periods. Longitudinal data indicate elevated risks for subsequent pregnancy complications such as uterine rupture and placental implantation disorders. Neonates delivered via prelabor CS exhibit increased incidence of respiratory distress syndrome and delayed physiological transition compared to vaginally delivered infants. Importantly, CS retains critical importance in obstetric emergencies including fetal compromise, malpresentation, and placenta previa, where timely intervention prevents severe maternal and perinatal morbidity.Conclusion. While cesarean section constitutes an indispensable component of contemporary obstetric practice, its escalating utilization warrants judicious clinical consideration. Although potentially life-saving in indicated circumstances, the procedure carries substantive risks for both parturient and neonate. Rigorous indication assessment remains paramount to avoid nonessential interventions and optimize perinatal outcomes. Additional investigation is warranted to elucidate long-term consequences across the reproductive lifespan and pediatric development.
  • Legal and medical aspects of the problem of postmortem donation in reproductive medicine

    The reproductive system plays a crucial role in preserving the human race. The health of the reproductive system reflects not only the genetic viability of future generations but also overall health. It is also related to numerous human rights, including the right to life, liberty, personal integrity, health, education, and the principle of equality and non-discrimination. The state, on the other hand, has positive obligations to ensure the observance, protection, and implementation of reproductive rights. Everyone has the right to the highest attainable level of physical and mental health. Any individual or couple has the right to decide how many children to have, when to have them, and at what intervals, even if it involves postmortem reproduction. That is why it is necessary to pay attention to the need to ensure the rights of servicemen to biological paternity (motherhood), which is extremely important, especially in the context of martial law in Ukraine.Despite some controversial aspects arising in the context of postmortem donation in Ukraine, it is the duty of the legislator to adopt laws and create legal provisions that can properly regulate this area. In doing so, the principle of proportionality should be taken into account, which can help to achieve the necessary balance between personal integrity and respect for the body after death, on the one hand, and the right to family, on the other. It should be understood that the experience of states at war has highlighted the urgency of postmortem donation and the position of society on its legal integration along with other areas of social relations that are subject to regulation. Military service, given its daily risks, can certainly compromise fertility, particularly through genitourinary trauma that can be associated with combat, gonadotoxic exposure, and physical separation from a partner.In addition, the issue of disposing of oocytes in case of death or declaration of death of a servicewoman remains unaddressed. Implementation of postmortem donation in the event of the death of a female military service member is possible only through surrogate motherhood. However, the current legislation in this area does not regulate the possibility of in vitro fertilization after the death of one of the spouses and completely “bypasses” any derivative issues related to surrogate motherhood after death.
  • Lyme disease as an interdisciplinary problem

    Lyme disease (LD) is one of the most common transmissible diseases, but its course and outcomes for the health are poorly understood. Our knowledge of clinical symptoms, diagnosis and treatment is limited due to the lack of reliable epidemiological studies and long-term follow-up.Patients with suspected LD have a wide variety of symptoms, including fever, rash, fatigue, muscle and joint pain, facial paralysis and arthritis. Therefore, the clinical picture of LD sometimes resembles other pathologies (rheumatological, autoimmune diseases, neurological disorders, etc.).Rare coinfections with this disease transmitted through a tick bite have been described also. Therefore, doctors need to diagnose this disease in time and refer the patient to an infectious disease specialist, because an incorrectly established diagnosis complicates the choice of a treatment method in the future and further worsens the course of the disease.The objective: to determine the relevance of LD as an interdisciplinary problem.Materials and methods: The methods of bibliographic and content analysis of the literature, comparative approach and synthesis, statistical data processing, as well as semantic grouping regarding various aspects of symptoms, diagnosis and treatment of LD were used. 125 patient histories were also analyzed.Results. It was found that among 125 patients LD is most often manifested by symptoms characteristic of arthritis of various localizations, Sjogren’s syndrome, Raynaud’s syndrome, gout, spondylitis. Among the patients the age group 51-60 years (34,4 % - 43 patients) prevailed, followed by 31-40 years (18,4 % – 23 people), 41-50 years (17.6 % – 22 people), 61-70 (12 % – 15 persons) and.Women are more affected by LD – 60 % (75) than men – 40% (50). The largest number of symptoms is observed in the age group of 51-60 years – 34.4 % (43), and the least complaints are expressed by patients aged 10-20 years – 2.4 % (3).Conclusions. Modern diagnostic methods are effective if their indication and interpretation are well followed, otherwise they can lead to incorrect diagnosis.Treatment of Lyme disease is based on antibiotic therapy according to its stage and clinical manifestations. However, there are no clear guidelines for the treatment of functional, persistent symptoms that patients experience over a long period of time.
  • Current perspectives on the metabolic aspects of polycystic ovary syndrome

    Polycystic ovary syndrome (PCOS) is a common endocrine disorder in women of reproductive age and is characterized by excessive androgen levels and ovulatory dysfunction. PCOS with a wide range of heterogeneous syndromes affects the health of a large part of the female population of the world.PCOS is a complex endocrinopathy that includes a variety of symptoms, such as menstrual disorders, infertility, acne, hirsutism, and several metabolic disorders. Given the different clinical manifestations, unknown etiology, and complex pathophysiology, the diagnosis of PCOS is still controversial.The prevalence of PCOS is often estimated between 2% and 26%. This significant difference in prevalence may be due to differences in diagnostic criteria, sample heterogeneity, socioeconomic status, access to health care, and general health awareness. Furthermore, given the multiple phenotypes of PCOS and the fact that it is predominantly diagnosed by gynecologists and endocrinologists, while it is poorly studied in other specialties and primary care physicians, these estimates may sometimes be lower.The article is a review of the scientific literature on various pathogenetic aspects of the development of PCOS, which arises as a result of polygenic susceptibility in combination with environmental influences, which may include epigenetic changes and intrauterine programming. Racial and ethnic differences may influence clinical manifestations through the genetic and environmental predisposition to endocrinopathies and metabolic diseases. In addition to the well-known clinical manifestations of hyperandrogenism and ovulatory dysfunction, women with PCOS are at increased risk for adverse mental health outcomes, pregnancy complications, and cardiometabolic disease.Treatment methods have limited effectiveness, mostly due to the fact that the development of drugs is difficult by an incomplete definition of the main pathophysiological processes. Advances in adipocyte genetics, metabolomics, and structure have improved understanding of key changes in neuroendocrine, enteroendocrine, and steroidogenic pathways, including increased gonadotropin-releasing hormone pulsatility, androgen excess, insulin resistance, and changes in the gut microbiome.Many patients have high levels of 11-oxygenated androgens with high androgenic activity, which may pose a metabolic risk. These advances have prompted the development of new therapies, including those targeting the neurokinin–kispeptin axis upstream of gonadotropin-releasing hormone, with the potential to reduce adverse clinical outcomes and improve patient outcomes.
