    Gastroschisis is a congenital defect characterized by the protrusion of abdominal viscera through a paraumbilical opening. OBJECTIVE: To determine the frequency of gastroschisis and describe variables associated with neonatal mortality. PATIENTS AND METHOD: A cross-sectional study based on data from the Perinatal Information System of the Hospital Gineco Obstétrico Isidro Ayora (2008-2022). Sociodemographic, obstetric, and neonatal variables were analyzed using Bayesian tests for contingency tables, logistic regression, and the Bayesian t-test analog. RESULTS: A total of 175 neonates were included (58.9% female). The frequency of gastroschisis was 0.13% (95% CI: 0.11-0.15), with a mortality rate of 22.8%. There was a negative association with gestational age (OR: 0.65; 95% CI: 0.49-0.85) and calendar year (OR: 0.79; 95% CI: 0.68-0.91), indicating lower mortality with each additional week of gestation and over time. The presence of ≥ 2 complications (OR: 3.46; 95% CI: 1.30-7.12) and Apgar score < 7 at five minutes (OR: 3.12; 95% CI: 1.01-60.91) were positively associated with mortality. Deceased neonates had lower birth weight (2,003.6g versus 2,316.7g; BF10=35.2) and shorter hospital stay (20.9 days versus 34.5 days; BF10=204). CONCLUSIONS: Gastroschisis showed a high frequency, with mortality associated with multiple complications, low Apgar scores, and low birth weight, while higher gestational age and advances in clinical management over the years were associated with a lower fatal risk. The need for a multidisciplinary approach to improve neonatal outcomes in this population is emphasized.
  • Maternal and neonatal factors associated with exclusive breastfeeding upon hospital discharge

    Objective: to identify maternal and neonatal factors associated with exclusive breastfeeding at hospital discharge from a tertiary care hospital. Methods: a cross-sectional study using secondary data from 22,136 records of newborns hospitalized in a neonatal unit (2009-2022). Maternal and neonatal variables and feeding practices at discharge were analyzed. Bayesian logistic regression models were applied. Results: exclusive breastfeeding at hospital discharge was 91.55%. Negatively associated factors were cesarean delivery (OR: 0.90), illicit substance use (OR: 0.30), low birth weight (OR: 0.84), and cleft lip and palate (OR: 0.15). Breastfeeding counseling and labor and delivery support promoted exclusive breastfeeding at hospital discharge. Conclusion: despite the high prevalence of exclusive breastfeeding at hospital discharge, barriers persist that must be addressed through promotion strategies, staff training, and support for mothers at risk.
  • Neurological disorders in infants may represent a potential risk factor for the development of atopic dermatitis

    Background: Central nervous system injuries can trigger some cellular systemic inflammatory reactions, which can also express receptors for neurotransmitters, neuropeptides, and neurotrophins. Thus, epilepsy, neuropsychiatric problems, stress, attention deficit disorders, hyperactivity, and autism spectrum disorders may be associated with some immune-mediated diseases, like atopic dermatitis. We aimed to investigate a possible link between neurological disorders caused by neonatal brain injuries with atopic dermatitis and high IgE levels in infants. Methods: Infants at risk of neurological disability, including structural brain injury detected by transfontanellar ultrasound (SCITU), intraventricular hemorrhage and secondary hydrocephalus, periventricular leukoencephalomalacia, and neonatal asphyxia, were included. Bayesian analysis was performed. Infants who presented with neurological disorders, atopic dermatitis, or exposure to environmental allergens were identified, and IgE levels were measured. Results: Seventy-four patients completed the study. Thirty infants (40%) presented atopic dermatitis. Patients with neurological disorders had a higher risk of presenting with atopic dermatitis compared to patients without neurological disorders (RR 3.23 95% CI 1.68-6.19). Patients with SCITU also had a higher risk of developing atopic dermatitis compared to patients without SCITU (RR 1.78 95% CI 1.23-2.56). Relative risk of atopic dermatitis was also higher in patients exposed to environmental allergens (RR 3.72 95% CI 1.76-7.87). No differences were found in IgE levels between the compared groups. Conclusion: These findings suggest a potential link between neurological disorders with atopic dermatitis and high IgE levels, highlighting a possible bidirectional communication between central nervous system and the immune system.
  • Dietary Patterns and Factors Associated with Food Affinity in Pregnant Women from Quito, Ecuador

    Nutrition during pregnancy influences perinatal outcomes and predispositions to chronic diseases. A prospective cohort study was carried out with the objectives of describing the dietary patterns in the pregnant population in the city of Quito, Ecuador and analysing the sociodemographic and lifestyle factors that influence the adherence to each dietary pattern. The body mass index was calculated for each patient, and the patients were classified according to the Atalah criteria. The Global Physical Activity Questionnaire was also applied. The dietary patterns were assessed using a dietary survey with a 24 h recall on two occasions. A total of 535 pregnant women were included. A positive association was found between the pattern “dairy, salads and sweet snacks/dressings” and foreign nationality (β = 0.82 (0.43;1.21)). The “refined carbohydrates” pattern was negatively associated with education equal to or less than 7 years and an income of up to one basic salary (β = −0.59 (−1.05; −0.14)). The “traditional Ecuadorian” pattern showed a positive association with being born in the coastal region of Ecuador (β = 0.62 (0.22; 1.01)). This study identified three dietary patterns in pregnant women and their possible associations with certain sociodemographic factors. More studies are needed to better understand these patterns as well as to analyse their nutritional and caloric properties.
  • Frecuencia de la práctica de la episiotomía en una institución en Quito, Ecuador, 2009-2022

    Objetivo: determinar la prevalencia de episiotomía en un periodo de tiempo de 14 años, y explorar su asociación con variables maternas, del parto y los resultados neonatales mediante la utilización de datos administrativos. Materiales y métodos: estudio transversal. Se incluyeron mujeres que tuvieron parto vaginal con recién nacido vivo, en una institución hospitalaria de referencia en atención obstétrica en Ecuador. Muestreo consecutivo. Se utilizaron los registros CIE-10 y del sistema informático perinatal (SIP), se evaluaron las variables sociodemográficas, de la gestación, el parto y neonatales. Análisis descriptivo. Se determinó la prevalencia global y por año. Se realizó análisis exploratorio bivariado y multivariado. Estudio aprobado por el Comité de Ética en Investigación en Seres Humanos de la Universidad Central del Ecuador. Resultados: se analizaron 11.862 datos. La prevalencia global de episiotomía fue del 36,69%, con un máximo del 42 % y un mínimo del 28 %. Se insinúa una tendencia a la reducción de la frecuencia desde 2019 a 2022. Se encontró una asociación positiva entre su realización y la edad materna menor a 20 años, cesárea previa, ausencia de acompañante durante el parto, recién nacido a término, y grande para la edad gestacional. Se halló una asociación negativa con partos vaginales anteriores. Conclusiones: la prevalencia de episiotomía supera la recomendación de la Organización Mundial de la Salud (OMS). Las guías de práctica clínica, en sus recomendaciones, deben considerar los factores maternos y neonatales asociados, potencialmente modificables. Se deberán realizar estudios prospectivos para evaluar la seguridad del procedimiento, confirmar estas observaciones y utilizar metodologías robustas para evaluar si hay una disminución real en su práctica.
  • Prevalence of congenital anomalies and factors associated with their development at a national referral hospital in Quito, Ecuador

    Introduction: Congenital anomalies (CA) have a significant impact on health and quality of life. Therefore, knowing their prevalence and the factors associated with their development is essential for designing and implementing educational and preventive programs. Objectives: To determine the prevalence of CAs in a national referral hospital in Ecuador between 2009 and 2022 and to explore the factors associated with their development. Materials and methods: Cross-sectional study conducted using 2 data sets for the period 2009 and 2022: 105 385 live births delivered at the HGOIA (estimation of prevalence) and 26 236 neonates hospitalized in the hospital's neonatology service (exploration of factors associated with the presence of CA). The Chi-square test was used to evaluate differences between groups (neonates with and without congenital defects), and binary logistic regression models, both simple (crude Odds ratio [OR]) and multiple (adjusted OR), were utilized to assess associations between the perinatal conditions considered and the presentation of CAs (overall and per anomaly category). Results: The prevalence of CA was 2.92% (n=3 075). Anomalies of the nervous system (25.6%), cardiovascular and respiratory systems (21.1%), and musculoskeletal (16.1%) were the most frequent CAs. Maternal age &gt;35 years (OR: 1.20, 95%CI: 1.07-1.33) was positively associated with the presentation of CAs, whereas planned pregnancy (OR: 0.88, 95%CI: 0.81-0.96) and multiple pregnancy (OR: 0.50, 95%CI: 0.42-0.60) were negatively associated. Folic acid supplementation, being male, and several maternal-related factors, among other variables, showed an association with the presence of specific types of CA. Conclusions: The prevalence of CAs at the HGOIA between 2009 and 2022 is slightly higher than what has been reported in the region, with neurological, cardiovascular and respiratory system anomalies being the most frequent. A positive association was found between maternal age &gt;35 years and the occurrence of CAs, whereas planned pregnancy and multiple pregnancy showed negative associations.
  • INTEGRATIVE REVIEW: BENEFITS, CHALLENGES, AND PERSPECTIVES OF KANGAROO CARE IN NEONATAL CARE

    Objective: Review the current scientific evidence on the physiological effects of kangaroo care, explore the barriers and facilitators to its implementation, and identify areas of knowledge that have not yet been explored. Theoretical Framework: The kangaroo care method, despite its benefits for premature and low birth weight babies, faces implementation challenges. Its acceptance and application vary due to various factors, including cultural and institutional ones, as well as the lack of support for mothers and families. Method: Integrative review of quantitative and qualitative studies. The critical appraisal of the studies was conducted using the Joanna Briggs Institute tool. Principio del formulario Results and Discussion: Evidence supports the effectiveness of kangaroo care in improving physiological parameters of the neonate such as heart rate, body temperature, and oxygen saturation. The main barriers to its implementation include: visiting hour restrictions, healthcare staff workload, negative cultural beliefs, lack of information and empowerment of mothers, and limited involvement of fathers. Research Implications: Provides information on how the implementation of kangaroo care benefits the care of premature newborns, as well as aspects that influence its implementation. Originality/Value: This study contributes to the literature by providing a comprehensive assessment of the effectiveness, safety, and applicability of kangaroo care in neonatal care. It allows for the visualization of barriers and facilitators to its implementation, providing identification of areas for improvement and gaps that may require future studies.
  • COVID-19 in pregnant women and neonates: Clinical characteristics and laboratory and imaging findings. An overview of systematic reviews

    Introduction: SARS-CoV-2 infection in the perinatal period may be associated with an increased risk of morbidity and mortality in both the mother and the neonate. Objective: To describe the clinical characteristics and, laboratory and imaging findings in pregnant women with COVID-19 and their newborns. Materials and methods: We searched PubMed, Scopus, Web of Science, and Cochrane databases for systematic reviews published between February 1, 2020, and May 30, 2021, describing clinical characteristics and laboratory and imaging (chest) findings in pregnant women with COVID-19 and their newborns; there were no language restrictions. Data were reanalyzed by means of Bayesian meta-analysis using Markov Chain Monte Carlo methods. The study protocol is registered in PROSPERO under code CRD42020178329. Results: Six systematic reviews were retrieved (for a total of 617 primary studies). A narrative synthesis of the proportions of signs, symptoms, and imaging and laboratory findings of both mothers and neonates was performed. The Odds ratios (OR) between pregnant women with and without COVID-19 were as follows: fetal well-being involvement: 1.9 (95%CI:1.09-3.63); stillbirth: 1.73 (95%CI:1.01-2.94); preterm birth: 1.77 (95%CI:1.25-2.61); maternal admission to the intensive care unit (ICU): 6.75 (95%CI:1-31.19). Regarding symptomatology, the following OR was obtained for myalgia between pregnant women and non-pregnant women with COVID-19: 0.67 (95% CI:0.51-0.93). Conclusions: Cough, fever, dyspnea, and myalgia are the most common symptoms in pregnant women with COVID-19; in addition, there is a higher risk of admission to the ICU. Regarding complementary testing, the most frequent alterations are lymphopenia and the evidence of lesions in chest imaging studies. The presence of COVID-19 in pregnant women is associated with premature birth. It seems that SARS-CoV-2 infection in neonates is not serious and the risk of vertical transmission is low, since no data about congenital malformations attributable to the virus were found.

    durante el confinamiento provocado por la COVID-19, las instituciones de educación implementaron el ciberaprendizaje, esta transición reveló que algunos docentes y estudiantes universitarios no estaban preparados para este cambio. Además, los alumnos fueron retirados de las unidades docentes, donde realizaban preceptoría. El objetivo fue analizar la evolución del nivel de acuerdo-desacuerdo en los estudiantes de Obstetricia con el aprendizaje virtual e identificar las variables que pueden predecir los cambios de opinión. estudio longitudinal predictivo, en fuente secundaria, mediante una encuesta Likert adaptada y validada, que se aplicó a los estudiantes al inicio y al final del confinamiento. Se realizó un análisis estadístico bayesiano utilizando análogos del modelo de regresión ordinal debido a sus ventajas sobre los métodos frecuentistas. las opiniones evolucionaron desde en desacuerdo al inicio hacia la opinión neutral, en la mitad de los ítems de la encuesta final. Se puede predecir que los alumnos estarán en desacuerdo con el aprendizaje en línea, cuando se encuentran cursando los semestres finales de la carrera. el nivel de acuerdo con la teleeducación mejoró, porque se asocia a múltiples ventajas y por las habilidades informáticas adquiridas en este periodo especialmente entre los alumnos. Los estudiantes de los semestres finales de la carrera expresaron su desacuerdo con el ciberaprendizaje, lo que se puede atribuir a la falta de tutorías clínicas con pacientes en los servicios de salud. Se deben mejorar las competencias digitales en los alumnos y los docentes, para aprovechar las ventajas que ofrece el aprendizaje en línea. During the COVID-19 lockdown, educational institutions implemented online learning, revealing that some university teachers and students were not prepared for this transition. Additionally, students were unable to attend clinical preceptorship in healthcare settings. The objective of this study was to analyze the evolution of agreement-disagreement levels among obstetrics students with virtual learning and identify variables that can predict changes in opinion. A predictive longitudinal study was conducted using a Likert-adapted and validated survey administered to students at the beginning and end of the lockdown. Bayesian statistical analysis was performed using ordinal regression model analogs due to their advantages over frequentist methods. Opinions evolved from disagreement at the beginning to neutral opinions in half of the survey items at the end. It can be predicted that students will disagree with online learning when they are in their final semesters. Agreement with teleeducation improved due to its association with multiple advantages and the computer skills acquired during this period, especially among students. However, students in their final semesters expressed disagreement with online learning, which can be attributed to the lack of clinical preceptorship with patients in healthcare settings. Digital competencies must be improved among students and teachers to take advantage of the benefits offered by online learning.
