  • Sepsis Prediction: Biomarkers Combined in a Bayesian Approach

    Sepsis is a serious public health problem. sTREM-1 is a marker of inflammatory and infectious processes that has the potential to become a useful tool for predicting the evolution of sepsis. A prediction model for sepsis was constructed by combining sTREM-1, CRP, and a leukogram via a Bayesian network. A translational study carried out with 32 children with congenital heart disease who had undergone surgical correction at a public referral hospital in Northeast Brazil. In the postoperative period, the mean value of sTREM-1 was greater among patients diagnosed with sepsis than among those not diagnosed with sepsis (394.58 pg/mL versus 239.93 pg/mL, p < 0.001). Analysis of the ROC curve for sTREM-1 and sepsis revealed that the area under the curve was 0.761, with a 95% CI (0.587–0.935) and p = 0.013. With the Bayesian model, we found that a 100% probability of sepsis was related to postoperative blood concentrations of CRP above 71 mg/dL, a leukogram above 14,000 cells/μL, and sTREM-1 concentrations above the cutoff point (283.53 pg/mL). The proposed model using the Bayesian network approach with the combination of CRP, leukocyte count, and postoperative sTREM-1 showed promise for the diagnosis of sepsis.
  • Evaluation of nasogastric tube fixation methods: adhesion, displacement and skin integrity

    Objective: to evaluate three methods of nasogastric tube fixation in terms of adhesion, displacement and skin integrity. Method: ex vivo study, with a sample of 30 experimental noses (10 for each type of fixation), developed with porcine skin, based on the average measurements of the human nose, in which 14-gauge polyvinyl chloride probes were inserted and 2 methods of fixation with adhesive tape (Fixation A and B) and one with an industrial device (Fixation C) were used. Each group was exposed to traction of 50, 100 and 500g sequentially over 12 and 24 hours, testing: adhesion capacity, probe displacement and skin integrity. The Chi-square test of independence was calculated for nominal variables and Student’s t-tests and analysis of variance (p< 0.05) for rational variables. Results: fixation B showed lower adhesion capacity (p <0.001) when compared to the other two fixations. A mean displacement of 52.17 mm was observed in the probes fixed by methods A and B and a greater occurrence of lesions associated with fixations A and C (p = 0.001). Conclusion: the results show complications related to the fixations: lack of adhesion, displacement of the probe and skin lesions, drawing attention to the complexity of the procedure.

  • Urinary incontinence in women: Risk factors according to type and severity

    Objetivo: verificar fatores associados a incontinência urinária em mulheres por tipo e gravidade.Metodologia: estudo transversal, realizado de novembro de 2018 a abril de 2019, com 30 mulheres em um hospital escola de Pernambuco. Foi utilizado o instrumento Gaudenz-Fragebogen para identificar os tipos de incontinência e o Incontinence Severity Index para classificar a gravidade da perda urinária. Utilizou-se o teste Exato de Fisher para analisar variáveis qualitativas e t-Student para variáveis racionais.Resultados: incontinência urinária de esforço foi o tipo mais frequente (66,7%), na forma moderada (43,3%) e esteve associada a baixa escolaridade (p<0,001), índice de massa corporal de 28,6 (p=0,043), circunferência abdominal de 103,4 (p<0,001), diabetes (p<0,001), número de partos superior a 4 (p=0,046), cirurgia ginecológica (p=0,023) e falta de atividade física (p=0,001).Considerações Finais: a identificação do tipo, da gravidade e dos principais fatores de risco modificáveis poderá subsidiar intervenções preventivas e curativas mais eficientes e efetivas.
  • Urinary incontinence in resident women in nursing homes

    Research questionWhich person related and clinical aspects variables are associated with the types of urinary incontinence in older women?Research problemThe purpose of this study is to determine the occurrence of urinary incontinence among elderly women and verify the association between related to the person and clinical variables with types of urinary incontinence.Literature reviewThere is a wide variation in the prevalence of urinary incontinence throughout the world, as well as among the regions of Brazil (Milsom et al., 2014). The worldwide prevalence of any type of incontinence in women has been estimated at more than 275 million in 2013, with an increase of more than 300 million by 2018.MethodologyA cross‐sectional study with a quantitative aproach was conducted with 120 elderly women. The International Consultation on Incontinence Questionnaire—Short Form was used as data collection tool and all participants signed consent form.ResultsThe prevalence of UI found was 67.5%. Regarding the type of UI, the following frequencies were found: urgency (29.2%), stress (23.3%) and mixed (15.0%). Urge incontinence was significantly associated with age (mean: 81.1 years, P: .05) and abdominal circumference (mean: 97.9 cm, P: .01). Diabetes mellitus was associated with urgency incontinence (P < .001), while hypertension showed association with stress and mixed incontinence (both with P < .001).ConclusionsUrinary incontinence was present in 67.5% of elderly women living in long‐term care facilities, with a higher occurrence of urgency and stress, and parametric statistics showed an association between Urge incontinence and age and abdominal circumference.

  • Association between functional health literacy and self-care with diabetes mellitus

    Objetivo: associar o nível de letramento funcional em saúde e autocuidado com Diabetes. Método:estudo transversal, quantitativo, realizado com 97 pacientes de um centro de referência paraatenção ao diabético, de fevereiro a junho de 2018. Com dados pelo Brief Test of Functional HealthLiteracy in Adults e o Questionário de atividades de autocuidado com o diabetes. Foi feita análisedescritiva e o teste Qui-quadrado.Resultados: o letramento funcional inadequado foi identificado em 47% dos pacientes. Observouseassociação entre o letramento inadequado e baixos níveis de adesão a: seguimento de dietasaudável e orientação alimentar (p<0,001), ingesta de doces (p<0,001) e avaliação da glicemia nonúmero de vezes recomendado (p<0,001).Conclusão: o nível inadequado de letramento está associado com baixas médias de adesão acomportamentos relacionados à dieta e ao monitoramento da glicemia. Estes achados sugeremincluir na prática clínica a avaliação do letramento e de autocuidado com o diabetes.
