Frontal Lobe Meningioma Masquerading as Depressive Illness—A Confusing Coincidence: A Report of Two Cases Olakunle Michael Adegboye, Kabir Aliyu Suleiman, Nurudeen Abiola Adeleke, Oluwafemi Nehemiah Akeredolu, Stanley Onyeka Nnara, et al. Indian Journal of Neurosurgery, 2026 Symptoms manifested by patients harboring brain tumors vary. Although depression, in the form of frontal lobe syndrome, may be present in patients with frontal lobe neoplasms, these tumors may be missed when there are circumstances relating to grief, especially in the absence of a focal neurological deficit. We present two cases of frontal lobe meningioma initially misdiagnosed as depressive illness. Both patients presented with psychiatric symptoms, including low mood and social withdrawal, which coincided with the loss of husband and separation from husband, respectively. These symptoms improved significantly after surgical excision of the tumors. These cases highlighted the importance of considering organic causes in patients presenting with psychiatric symptoms, particularly when treatment response is inadequate.
The Profile and Short-Term Outcome of Children with Myelomeningocele in Ilorin, North-Central Nigeria N. A. Adeleke, O. M. Adegboye, O. Enaworu, S. O. Nnara, H. A. Yeqeen, et al. Journal of Pediatric Neurosciences, 2025 A bstract Background: Myelomeningocele (MM) is a type of open spinal dysraphism that occurs in the early intrauterine life, around the third and fourth week of embryonic development when the mother is not even aware she is pregnant. Pre-conceptional folic acid supplementation has been found to reduce the incidence globally. Materials and Methods: This was a prospective cross-sectional study of children with MM, who were managed at the University of Ilorin Teaching Hospital over a period of 22 months. Forty-six children were recruited into the study, however, the parents of six of them departed against medical advice leaving 40 patients for analysis. All forty patients had an echocardiogram and trans-fontanel ultrasound done and underwent surgical repair of their MMs. Prenatal and post-natal data were collected, which was analyzed by Statistical Package for Social Sciences version 23. Results: There was a slight female preponderance with female to male ratio of 1.4:1. Majority (75%) of the mothers were between ages 21 and 30 years, with only 7.5% of them being above 35 years of age. While the majority (85%) of the mothers took post-conceptional folic acid which was commenced at antenatal registration, only 5% of them took pre-conceptional folic acid. The lumbosacral region was the commonest location of MM in this series, accounting for 95% while the thoracic region accounted for the remaining 5%. The most common associated congenital anomaly was hydrocephalus (65%). Surgical site infection (SSI) was the commonest post-operative complication (17.5%), others were cerebrospinal fluid leakage (10%), wound dehiscence (5%), and pseudomeningocele (5%), 5% of the patients had combined SSI and wound dehiscence. Conclusion: The majority of the mothers of children with MMs in this setting did not take pre-conceptional folic acid which is a preventive measure against this malformation. The commonest associated congenital anomaly with MM was hydrocephalus.
Strategies to minimise and monitor biases and imbalances by arm in surgical cluster randomised trials: evidence from ChEETAh, a trial in seven low- and middle-income countries Didier Ahogni, Aristide Ahounou, K. Alassan Boukari, Oswald Gbehade, Thierry K. Hessou, et al. Trials, 2023 Background Cluster randomised controlled trials (cRCT) present challenges regarding risks of bias and chance imbalances by arm. This paper reports strategies to minimise and monitor biases and imbalances in the ChEETAh cRCT. Methods ChEETAh was an international cRCT (hospitals as clusters) evaluating whether changing sterile gloves and instruments prior to abdominal wound closure reduces surgical site infection at 30 days postoperative. ChEETAh planned to recruit 12,800 consecutive patients from 64 hospitals in seven low-middle income countries. Eight strategies to minimise and monitor bias were pre-specified: (1) minimum of 4 hospitals per country; (2) pre-randomisation identification of units of exposure (operating theatres, lists, teams or sessions) within clusters; (3) minimisation of randomisation by country and hospital type; (4) site training delivered after randomisation; (5) dedicated ‘warm-up week’ to train teams; (6) trial specific sticker and patient register to monitor consecutive patient identification; (7) monitoring characteristics of patients and units of exposure; and (8) low-burden outcome-assessment. Results This analysis includes 10,686 patients from 70 clusters. The results aligned to the eight strategies were (1) 6 out of 7 countries included ≥ 4 hospitals; (2) 87.1% (61/70) of hospitals maintained their planned operating theatres (82% [27/33] and 92% [34/37] in the intervention and control arms); (3) minimisation maintained balance of key factors in both arms; (4) post-randomisation training was conducted for all hospitals; (5) the ‘warm-up week’ was conducted at all sites, and feedback used to refine processes; (6) the sticker and trial register were maintained, with an overall inclusion of 98.1% (10,686/10,894) of eligible patients; (7) monitoring allowed swift identification of problems in patient inclusion and key patient characteristics were reported: malignancy (20.3% intervention vs 12.6% control), midline incisions (68.4% vs 58.9%) and elective surgery (52.4% vs 42.6%); and (8) 0.4% (41/9187) of patients refused consent for outcome assessment. Conclusion cRCTs in surgery have several potential sources of bias that include varying units of exposure and the need for consecutive inclusion of all eligible patients across complex settings. We report a system that monitored and minimised the risks of bias and imbalances by arm, with important lessons for future cRCTs within hospitals.
