In recent years, Cytopathology has undergone significant growth driven by technological and molecular advances, which have expanded its scope and complexity. In this context, SMS play a crucial role by offering training activities such as congresses, courses, workshops, and publications, acting as a bridge between scientific evidence and clinical practice. The article also addresses current challenges related to equitable access, pedagogical innovation, and the integration of new technologies, as well as potential strategies for strengthening continuing education and international cooperation.
Erdheim-Chester manifesting purely as a neurological disease Lídia Laura Salvador Ramos, Iago Resende Carvalho, Diogo Fernandes dos Santos, Andrea de Martino Luppi, Bruno de Carvalho Dornelas Arquivos De Neuro Psiquiatria, 2025 A 60-year-old man presented with vertigo, paraparesis, and dysarthriathat evolved over 3 months. A magnetic resonance imaging (MRI) scan revealed nodular masses in the pons, peduncles, and cerebellar hemispheres, indicating a metastatic neoplasm ( ► Figure 1 ). A craniectomy was performed, showing dense histiocyte proliferation, Touton giant cells, and lymphocytic in fi ltrate consistent with Erdheim-Chester disease ( ► Figure 2 ). The cytogenetic analysis con fi rmed a BRAF V600 mutation. Vemurafenib therapy was initiated. Six months later, the patient still exhibited dyslalia and left hemiparesis, and a follow-up MRI scan was performed, revealing an area of sequelae ( ► Figure 3 ). Erdheim-Chester disease is a rare non-Langerhans histiocytosis, with neurological symptoms occurring in 41% of the cases. 1,2
Familial Amyloid Polyneuropathy (FAP): an important differential diagnosis of leprosy neuropathy Diogo Fernandes dos Santos, Leonardo Peixoto Garcia, Iago Resende Carvalho, Bruno de Carvalho Dornelas, Isabela Maria Bernardes Goulart Leprosy Review, 2025 A 64-year-old male with a family history of leprosy and cardiomyopathy presented with paresthesia and neuropathic pain in the lower limbs for one year, progressing to the upper limbs for the last 6 months, besides erectile dysfunction for at least 3 years and episodes of postural hypotension. Due to the family history, primary neural leprosy (PNL) was suspected. The nerve conduction study showed a severe, predominantly sensory, axonal, symmetrical, chronic axonal polyneuropathy with signs of acute denervation, and laboratory tests for PNL were negative. A peripheral nerve biopsy (sural) showed amyloid deposits and molecular evaluation confirmed the diagnosis of Familial Amyloid Polyneuropathy (FAP). Specific treatment for FAP was started and the family members were evaluated, confirming that many had FAP and were misdiagnosed as leprosy. This case shows the importance of differentiating PNL and FAP in clinical practice. We propose a table to help distinguish these two conditions in clinical practice.
Impact of histopathological and serological assessments on early diagnosis of leprosy relapse Bruno de Carvalho Dornelas, Willian Vargas Tenório da Costa, João Pablo Ferraz de Abreu, Juliana Salomão Daud, Felipe dos Anjos Rodrigues Campos, Deiriene Rodrigues de Oliveira Campos, Douglas Eulálio Antunes, Lúcio Borges de Araújo, Diogo Fernandes dos Santos, Cleverson Teixeira Soares, Isabela Maria Bernardes Goulart APMIS, 2025 This study aimed to identify laboratory factors predicting leprosy relapse (LR) after multi‐drug therapy (MDT). A case–control study included 80 patients treated with MDT at a national reference center over 12 years. The Relapse Group had 40 patients who relapsed after an average of 89.2 months post‐MDT, while the Control Group had 40 patients who remained asymptomatic for an average of 113.1 months. Significant predictors of LR included neural/perineural lymphocytic infiltrate (OR = 4.67; p = 0.0076) and foamy granulomas (OR = 15.55; p = 0.0005), increasing odds by 4.7 and 15.6 times, respectively. The Relapse Group had a mean histological bacillary index (hBI) of 3.23+ compared to 1.8 in the Control Group (p = 0.004). An hBI ≥3+ had 72% sensitivity and 65% specificity for detecting LR (AUC = 0.72; p = 0.0002). Elevated anti‐phenolic glycolipid I (anti‐PGL‐I) IgM antibody levels (ELISA index, EI ≥1) were also associated with LR (OR = 4.67; p = 0.0031). An EI ≥3.6 had 71% sensitivity and 62% specificity (AUC = 0.70; p = 0.0012). Multivariate analysis indicated that neural/perineural infiltrate, foamy granulomas, hBI ≥ 1+, and EI ≥ 1 significantly predicted LR, with up to 94.32% probability. Conclusively, these factors can identify individuals at high probability of LR after MDT.

The current World Health Organization (WHO) fixed-duration MDT regimen, based on lesion count, might not be adequate. Leprosy lacks clear-cut objective cure criteria, and the predictive value of post-MDT histopathological findings remains uncertain. This study aims to identify predictive factors for TF among leprosy patients who have completed the WHO-recommended MDT. METHODS: An analysis was conducted on 80 individuals from a national leprosy reference center, comprising 40 TF cases (with a mean relapse at 13.0 months) and 40 controls (with a mean of 113.1 months without disease signs). Various epidemiological and clinical-laboratory parameters were assessed post-MDT. RESULTS: In skin samples, the presence of foamy granuloma (OR = 7.36; 95%CI2.20-24.60; p = 0.0012) and histological bacillary index (hBI) ≥ 1+ (OR = 1.55; 95%CI1. 22-1.99; p = 0.0004) were significantly associated with TF, with odds ratios of 7.36 and 1.55, respectively. Individuals who experienced TF had a mean hBI of 3.02+ (SD ± 2.02), while the control group exhibited a mean hBI of 1.8+ (SD ± 1.88). An hBI ≥ 3 + showed a sensitivity of 73% and a specificity of 78% for TF detection (AUC: 0.75; p = 0.0001). Other histopathological features like epithelioid granulomas, and skin changes did not show significant associations (p > 0.05). Additionally, higher anti-phenolic glycolipid-I (anti-PGL-I) ELISA index (EI) levels were linked to a 1.4-fold increased likelihood for TF (OR = 1.4; 95%CI1.13-1.74; p = 0.0019). A mean EI of 4.48 (SD ± 2.80) was observed, with an EI ≥ 3.95 showing a sensitivity of 79% and a specificity of 59% for TF detection (AUC: 0.74; p = 0.0001). Moreover, the presence of Mycobacterium leprae (M. leprae) DNA in real-time polymerase chain reaction (qPCR) was associated with a 3.43-fold higher likelihood of TF. Multivariate regression analysis indicated that concurrent presentation of neural/perineural lymphocytic infiltrate, foamy granuloma, hBI ≥ 1+, and EI ≥ 1 markedly increased the likelihood of TF by up to 95.41%. CONCLUSION: Persistence of nerve-selective lymphocytic infiltrate, foamy granulomas, and bacilli in skin biopsies, and elevated EI post-MDT, may serve as predictive factors for identifying individuals at higher probability of TF.
Intramedullary Neurocysticercosis: A Case Report Stefano Machado, Egon Ewaldo Lindorfer Neto, Bruno de Carvalho Dornelas, Andrea de Martino Luppi, Elder Henrique de Oliveira, Paulo Cesar Marinho Dias, Maykell Queiroz dos Reis, Diogo Fernandes dos Santos Neurology, 2023 An 80-year-old man from the Brazilian Midwest presented with tetraparesis with brachial predominance and hyperreflexia with 1 year of evolution. In investigation, cervical MRI showed intramedullary cystic formation (4.6 × 1.7 × 1.3 cm), and brain MRI was suggestive of neurocysticercosis (Figure 1). CT of the chest, abdomen, and pelvis and endoscopy and colonoscopy were normal. CSF showed hyperproteinorachia (113 mg/dL). The excision of the lesion of the spinal cord was uneventful, and the anatomopathological examination was compatible with neurocysticercosis (Figure 2). Slight improvement in paresis was noted on reassessment after 2 weeks, when albendazole (15 mg/kg/d) and corticosteroids were started for 4 weeks. This rare presentation of neurocysticercosis (Taenia solium) corresponds to 1%–3% of cases.1 Infectious etiologies should be considered in the differential diagnosis of spinal cord syndromes, mainly in emerging countries, where Dr. there is precarious basic sanitation and contact with contaminated water and food.2
Fever of Unknown Origin, Wasting Syndrome and Bone Marrow Involvement: A Leprosy Case Report Marcela Araujo de Oliveira Santana, Willian Vargas Tenório da Costa, Isadora Costa Celestino, Diogo Fernandes dos Santos, Bruno de Carvalho Dornelas, Matthew Martin Pavelka, Andrea De Martino Luppi, Isabela Maria Bernardes Goulart Frontiers in Immunology, 2022 Mycobacterium leprae, the etiologic agent of leprosy, is an acid-fast-staining and slow-growing bacilli that infect macrophages and Schwann cells individually or through forming globi. The clinical presentation of leprosy is broad and depends on the host immune response. We report a case of a 42-year-old Brazilian man presenting with fever of unknown origin (FUO), anemia, wasting syndrome, and neuropathy. The diagnosis of lepromatous leprosy was made after an extensive investigation revealed the presence of M. leprae in the bone marrow. Bone marrow involvement in leprosy is rare and some authors believe the presence of M. leprae in the bone marrow can act as a reservoir of the disease facilitating future relapses. It is important to investigate bone marrow involvement in leprosy, especially when the patient presents with cytopenias and positive epidemiologic history.
Leprosy Relapse: A Retrospective Study on Epidemiologic, Clinical, and Therapeutic Aspects at a Brazilian Referral Center Ana Cláudia Mendes do Nascimento, Diogo Fernandes dos Santos, Douglas Eulálio Antunes, Maria Aparecida Gonçalves, Marcela Araujo de Oliveira Santana, Bruno de Carvalho Dornelas, Luiz Ricardo Goulart, Isabela Maria Bernardes Goulart International Journal of Infectious Diseases, 2022 OBJECTIVES: We aimed to characterize the profile of patients diagnosed with leprosy relapse and understand the influence of different multidrug therapy (MDT) treatments and initial disease presentation. METHODS: This retrospective study included patients diagnosed with leprosy relapse at a referral center in Brazil from 2013 to 2018. We analyzed their clinico-epidemiologic characteristics, laboratory data, and bacilloscopic tests. Survival analysis was used to determine the time elapsed until relapse according to the previous treatment and clinical forms of the disease. RESULTS: A total of 126 cases of relapse were analyzed, which comprised 11.89% (126/1059) of the cases. The median time elapsed until a relapse was 10 years, and most patients had previously undergone 12 doses of MDT (40.48%; 51/126). Undergoing 24 doses of MDT was associated with a better prognosis regarding relapse over time compared with 6 or 12 doses of MDT therapy. Most cases of relapse were classified as multibacillary (96.03%; 121/126). CONCLUSION: The incidence of relapse was greater than observed in other studies. The high percentage of multibacillary patients who had negative bacillary indices demonstrated that the bacillary index cannot be considered to be an essential criterion for relapse, especially concerning making an early diagnosis.
Type 2 leprosy reaction presenting as a monoarthritis post multidrug therapy Isabela Maria Bernardes Goulart, Marcela Araujo de Oliveira Santana, Willian Vargas Tenório da Costa, Matthew Martin Pavelka, Bruno de Carvalho Dornelas Idcases, 2022 Type 2 leprosy reaction, or erythema nodosum leprosum (ENL), involves a complex interaction between the host's immune system and Mycobacterium leprae. It may occur before, during, or after treatment and have a variable clinical presentation involving different body systems, such as skin, osteoarticular, kidneys, and others. Thus, the differential diagnosis, depending on its clinical presentation, can be broad and challenging. The authors report a case of a severe monoarthritis during a type 2 reaction after the multidrug therapy (MDT) was discharged and the investigation of the differential diagnoses.

Peru was the headquarters for this event, where 17 national and international professors of high academic standing participated. They interacted in a multidisciplinary context through talks with national panellists and the general public. The recent promulgation of the ‘National Cancer Law’ fosters the development of discussion forums to analyse the national realities and uphold continuous learning about experiences in other Latin American countries with successful cancer programmes, in which pathology holds a principal role. The topics addressed during this JoPaO included the exchange of Latin American cancer management experiences, an emphasis on investments in and the development of strategic plans to improve care, the use of new technologies, laboratory quality control, and the need to advance scientific research.
