    PURPOSE: The Global Andrology Forum (GAF) has developed an innovative training method to equip novice researchers with the fundamental skills to conduct high-quality systematic reviews and meta-analyses (SRMAs) under the supervision of senior experts. This article aims to describe and evaluate the effectiveness of the GAF training program in enhancing these skills. MATERIALS AND METHODS: A six-module training program was designed to cover the key steps in SRMA studies, enabling GAF members to conduct high-quality SRMAs. Initially launched as a synchronous (remote) training program, it was later transitioned into an asynchronous format and implemented on the Moodle platform. Trainees participating in the synchronous training program were asked to evaluate the course using a specially designed questionnaire. Participants' assessments of the asynchronous course were collected at various stages. Data from both training models were statistically analyzed. RESULTS: Among the trainees who participated in the questionnaire on the original synchronous course (n=101), the majority rated the material and assessments as clear, helpful, and effective in enhancing their knowledge. Additionally, the trainees indicated that the features offered in the training made them feel comfortable and engaged. Of the 29 participants enrolled in the asynchronous program, 22 took part in the assessment. Among them, 12 (54.5%) completed all course requirements, achieving an average score of 83.7%, qualifying them for conducting SRMAs. CONCLUSIONS: The GAF SRMA online training program has proven to be an effective and well-received initiative for equipping researchers with essential skills to conduct high-quality SRMAs. By addressing the diverse needs of trainees through its innovative and flexible design, the program has demonstrated its capacity to enhance knowledge and engagement. The successful implementation of this training program highlights its potential to significantly improve the quality of SRMAs in medical research, thereby contributing to the advancement of evidence-based medical practice on a broader scale.

    Leukocytospermia, defined as a leukocyte concentration in semen exceeding 1×10⁶ leukocytes/mL, significantly impacts male reproductive health by affecting sperm functionality and fertility outcomes. This condition arises from various etiological factors, including infections, autoimmune responses, lifestyle factors, and other physiological conditions. Adverse effects on sperm associated with leukocytospermia include acrosome damage and abnormalities in the sperm midpiece and tail. The review explores the complex interplay between leukocytospermia and oxidative stress, emphasizing the harmful effects on sperm DNA integrity and overall sperm quality. Due to the multifactorial nature of leukocytospermia, diagnosing this condition presents several challenges. Effective management strategies discussed include the use of antibiotics, anti-inflammatory agents, and assisted reproductive technologies. Diagnostic methods range from traditional peroxidase staining to more advanced techniques such as immunocytochemistry and flow cytometry, which offer higher sensitivity and specificity. Infections of the male genital tract, particularly male accessory gland infection and male genital tract infection, play a significant role in the etiology of leukocytospermia. These infections lead to an inflammatory response, resulting in leukocyte infiltration into the semen. Systemic conditions like diabetes mellitus and autoimmune disorders also contribute to leukocytospermia by provoking inflammatory responses that facilitate leukocyte presence in semen. This review underscores the importance of a comprehensive diagnostic approach that includes patient history, physical examination, and advanced laboratory tests. Treatment is tailored to the identified underlying cause, whether infectious or non-infectious. Lifestyle modifications, such as reducing stress, improving diet, and avoiding environmental toxins, are also recommended to enhance semen quality. For clinicians, this review provides a concise yet thorough overview of leukocytospermia, integrating the latest research findings and clinical insights to aid in the effective management of this condition, ultimately aiming to improve patient care in male reproductive health.
  • A review of testicular histopathology in nonobstructive azoospermia

    One major challenge in male factor infertility is nonobstructive azoospermia (NOA), which is characterized by spermatozoa-deficient semen without physical duct blockage. This review offers a thorough overview of the histopathology of the testes in NOA cases, clarifying its complex etiology and emphasizing the possible value of histopathology inspection for both diagnosis and treatment. Variable histopathological findings have been linked to NOA, such as tubular hyalinization, Sertoli cell-only syndrome, hypospermatogenesis, and germ cell arrest. Understanding the pathophysiology and forecasting the effectiveness of treatment are further enhanced by both morphometric and ultrastructural analyses. The potential significance of testicular biopsy in forecasting reproductive outcomes is assessed, especially concerning assisted reproductive technologies like intracytoplasmic sperm injection (ICSI). Besides, testicular microlithiasis, serum hormone profiles, and testicular size are investigated concerning NOA histopathology. It is concluded that understanding the histopathological patterns in NOA is crucial for its accurate diagnosis and appropriate management. Further research is still warranted to improve understanding of the complex pathophysiology underlying NOA.
  • The Diagnostic Performance of Magnetic Resonance Texture Analysis in Histological Subtyping and Grading of the Renal Cell Carcinoma

    Backgrounds-Aims: To evaluate the efficiency of MRTA in distinguishing RCC types and in the distinction of tumor grade.Methods: 62 patients were analyzed retrospectively and grouped as 40 clear cell-RCC, 11 papillary-RCC and 11 chromophobe-RCC. In the MRI (1.5-T) protocol, the axial T2 weighted (W), axial T1W in-phase (IP) and apparent diffusion coefficient (ADC) images were used. Additionally, postcontrast images obtained in the corticomedullary (CM) phase and nephrogram (NG) phase of the axial fat-suppressed T1W (VIBE) sequence were used. In MRTA were used parameters as mean, median, entropy, skewness, kurtosis, variance, uniformity. Statistical analysis was performed to compare CC-RCC &NC-RCC, and high-grade& low-grade tumors and between the subtypes.Results: Tissue parameters that perform best in separating CC-RCC from NC-RCC (AUC in brackets): Values obtained included entropy (0.67) in CM phase, mean (0.75) and median (0.76) in ADC, entropy (0,66) and variance (0.66) in NG phase were obtained. The p values in IP and T2W images in the distinction between the high and low degrees were significant.Conclusion: Several MR texture parameters performed well (AUC> 0.75) in separating CC-RCC from NC-RCC. MRTA can be a useful noninvasive tool for this purpose. The first order parameters that we used in TA can be used to evaluate the prognosis in RCC patients.

    Purpose This study aimed to examine current global practices in regenerative therapy (RT) for erectile dysfunction (ED) and to establish expert recommendations for its use, addressing the current lack of solid evidence and standardized guidelines. Materials and Methods A 39-question survey was developed by senior Global Andrology Forum (GAF) experts to comprehensively cover clinical aspects of RT. This was distributed globally via a secure online Google Form to ED specialists through the GAF website, international professional societies, and social media, the responses were analyzed and presented for frequencies as percentages. Consensus on expert recommendations for RT use was achieved using the Delphi method. Results Out of 479 respondents from 62 countries, a third reported using RT for ED. The most popular treatment was low-intensity shock wave therapy (54.6%), followed by platelet-rich plasma (24.5%) and their combination (14.7%), with stem cell therapy being the least used (3.7%). The primary indication for RT was the refractory or adverse effects of PDE5 inhibitors, with the best effectiveness reported in middle-aged and mild-to-moderate ED patients. Respondents were confident about its overall safety, with a significant number expressing interest in RT’s future use, despite pending guidelines support. Conclusions This inaugural global survey reveals a growing use of RT in ED treatment, showcasing its diverse clinical applications and potential for future widespread adoption. However, the lack of comprehensive evidence and clear guidelines requires further research to standardize RT practices in ED treatment.
  • Can lifestyle changes significantly improve male fertility: A narrative review?

    Male infertility is increasingly recognized as a significant global health concern, with lifestyle factors being critical contributors to declining male fertility rates. This narrative review comprehensively analyzes the impact of various lifestyle choices, including diet, physical activity, substance use, stress, sleep, weight management, sexual habits, and environmental and occupational exposures on male reproductive health. The review examines the biological and ecological mechanisms through which these lifestyle factors affect spermatogenesis and sperm quality. Furthermore, it discusses potential interventions, such as dietary modifications, weight management strategies, substance cessation programs, stress reduction techniques, and workplace policy changes to improve male fertility outcomes. Emphasis is placed on the role of oxidative stress, hormonal regulation, and DNA integrity in mediating the effects of these lifestyle factors. While lifestyle modifications can significantly enhance male reproductive health, the available evidence highlights the need for more rigorous research to establish solid guidelines and interventions for mitigating male infertility.

  • Anti-sperm Antibody Positivity in Men with Varicocele: A Systematic Review and Meta-Analysis

    PURPOSE Despite the significant role of varicocele in the pathogenesis of male infertility, its association with anti-sperm antibodies (ASA) remains controversial. This systematic review and meta-analysis (SRMA) aims to investigate the frequency of ASA positivity in men with varicocele. MATERIALS AND METHODS This SRMA is conducted in accordance with the Meta-analysis of Observational Studies in Epidemiology guidelines. We investigated the frequency of ASA positivity in ejaculates or serum of men with varicocele as compared to men without varicocele (controls). A literature search was performed using the Scopus and PubMed databases following the Population Exposure Comparison Outcome, Study Design model. Data extracted from eligible studies were meta-analyzed and expressed as odds ratios (ORs) and confidence intervals (CIs). RESULTS Out of 151 abstracts identified during the initial screening, 6 articles met the inclusion criteria and were included in the meta-analysis. Using mixed antiglobulin reaction (MAR) assay, 61 out of the 153 (39.8%) patients with varicocele tested positive for ASA in their ejaculates as compared to 22 out of the 129 control subjects (17%, OR=4.34 [95% CI: 1.09-17.28]; p=0.04). Using direct or indirect immunobead test, 30 out of 60 cases diagnosed with varicocele (50%) had shown ASA positivity in their ejaculates as compared to 16 out of 104 controls (15.4%, OR=3.57 [95% CI: 0.81-15.68]; p=0.09). Using enzyme-linked immunosorbent assay (ELISA), out of 89 varicocele patients, 33 (37.1%) tested positive for serum ASA as compared to 9 out of 57 participants in the control group (15.8%, OR=7.87 [95% CI: 2.39-25.89]; p<0.01). CONCLUSIONS This SRMA indicates that ASA positivity is significantly higher among men with varicocele when tested by direct method (MAR) or indirect method (ELISA). This data suggests an immunological pathology in infertile men with varicocele and may have implications for the management of these patients.
  • Impact of Varicocele Repair on Assisted Reproductive Technique Outcomes in Infertile Men: A Systematic Review and Meta-Analysis

    PURPOSE In this systematic review and meta-analysis, we investigated assisted reproductive technology (ART) success in infertile men with clinical varicocele and abnormal semen parameters who underwent varicocele repair (VR) before the ART procedure as compared to those who did not. MATERIALS AND METHODS A comprehensive search of the Scopus, PubMed, Embase, and Cochrane Library databases was conducted using a specific query string to identify studies examining the impact of VR on ART outcomes, including fertilization rate, clinical pregnancy, pregnancy loss, and live-birth rate, until October 2023. Outcomes were analyzed based on the type of ART. Studies on VR in infertile men with non-obstructive azoospermia and those who underwent ART only due to female factor infertility were excluded from the study. RESULTS Out of 1,554 articles reviewed, only 9 met the inclusion criteria for the study. All the included articles were observational studies. The variability in study quality in the included literature resulted in a moderate overall risk of bias. Data analysis showed that for intrauterine insemination, there was no difference in the clinical pregnancy rate (odds ratio [OR] 1.01, 95% confidence interval [CI]: 0.42, 2.45; p=0.97). However, for intracytoplasmic sperm injection (ICSI), men with VR showed a significant improvement in fertilization rate (mean difference 10.9, 95% CI: 5.94, 15.89; p<0.01), clinical pregnancy rate (OR 1.38, 95% CI: 1.07, 1.78; p=0.01) and live-birth rate (OR 2.07, 95% CI: 1.45, 2.97; p<0.01), compared to men who did not undergo VR. CONCLUSIONS The findings of this systematic review and meta-analysis suggest that VR has a positive impact on pregnancy and live birth rates after ICSI. However, biases like small sample sizes and heterogeneous populations highlight the need for larger, well-designed prospective studies to validate these findings.
