Pinggera, Imad Ziouziou, Gökhan Çeker, Christopher Ho, Fereshteh Dardmeh, Fahmi Bahar, Nazim Gherabi, Parisa Dolati, Priyank Kothari, Aram Adamyan, Rupin Shah World Journal of Men S Health, 2026 PURPOSE: The Global Andrology Forum (GAF) has developed an innovative training method to equip novice researchers with the fundamental skills to conduct high-quality systematic reviews and meta-analyses (SRMAs) under the supervision of senior experts. This article aims to describe and evaluate the effectiveness of the GAF training program in enhancing these skills. MATERIALS AND METHODS: A six-module training program was designed to cover the key steps in SRMA studies, enabling GAF members to conduct high-quality SRMAs. Initially launched as a synchronous (remote) training program, it was later transitioned into an asynchronous format and implemented on the Moodle platform. Trainees participating in the synchronous training program were asked to evaluate the course using a specially designed questionnaire. Participants' assessments of the asynchronous course were collected at various stages. Data from both training models were statistically analyzed. RESULTS: Among the trainees who participated in the questionnaire on the original synchronous course (n=101), the majority rated the material and assessments as clear, helpful, and effective in enhancing their knowledge. Additionally, the trainees indicated that the features offered in the training made them feel comfortable and engaged. Of the 29 participants enrolled in the asynchronous program, 22 took part in the assessment. Among them, 12 (54.5%) completed all course requirements, achieving an average score of 83.7%, qualifying them for conducting SRMAs. CONCLUSIONS: The GAF SRMA online training program has proven to be an effective and well-received initiative for equipping researchers with essential skills to conduct high-quality SRMAs. By addressing the diverse needs of trainees through its innovative and flexible design, the program has demonstrated its capacity to enhance knowledge and engagement. The successful implementation of this training program highlights its potential to significantly improve the quality of SRMAs in medical research, thereby contributing to the advancement of evidence-based medical practice on a broader scale.
Association of CXCR7 and CXCR4 expression with tumor immune phenotypes in tubo-ovarian high-grade serous carcinoma Burak Tatar, Sultan Caliskan, Fatma Nur Uygun, Sercan Ergün, Neslihan Hekim, Mehmet Kefeli, Sezgin Gunes Irish Journal of Medical Science, 2026 Background Tumor-infiltrating lymphocytes (TILs) are associated with favorable outcomes in high-grade serous ovarian carcinoma (HGSC); however, the tumor microenvironment may modulate immune cell infiltration. This study aimed to evaluate the association between tumor and stromal expression of CXCL12, tumor expression of CXCR4 and CXCR7, immune phenotypes, and survival outcomes in tubo-ovarian HGSC. Methods Eighty-six patients with primary tubo-ovarian HGSC who underwent cytoreductive surgery were retrospectively analyzed. Immunohistochemical expression of CXCL12 (tumor and stroma), CXCR4, and CXCR7 was assessed. TILs were semi-quantitatively scored and dichotomized as low (score 1) or high (scores 2–3). Immune phenotypes were classified as immunogenic, immune-excluded, or immune-ignorant. Recurrence-free survival (RFS) and overall survival (OS) were analyzed using Kaplan–Meier methods, log-rank tests, and Cox regression. Multivariable logistic regression was used to identify factors associated with high TILs. Results Median RFS was 23.2 months (95% CI, 19.2–27.1) and median OS was 47.3 months (95% CI, 36.3–55.0). RFS differed significantly across immune phenotypes (log-rank p=0.047), with shorter RFS observed in the immune-excluded phenotype compared with the immunogenic phenotype (hazard ratio, 2.20; 95% CI, 1.14–4.25). High tumor CXCR7 expression was independently associated with a lower likelihood of high TILs (adjusted odds ratio, 0.22; 95% CI, 0.07–0.65). CXCL12 expression was not associated with survival outcomes. Conclusion Tumor CXCR7 expression is associated with reduced lymphocytic infiltration and an immune-excluded phenotype in HGSC, suggesting a potential role of the CXCR7 axis in shaping the tumor immune microenvironment.

Micrornas (mirnas) are key regulators of cell proliferation, apoptosis, and dna damage response, and have been implicated in therapy resistance. However, their roles in prostate cancer radioresponse remain incompletely understood. This study investigated the expression patterns of ten selected mirnas associated with radiation resistance in other cancers in prostate cancer models. Radiation‐resistant pc‐3, radiation‐sensitive lncap, and normal prostate epithelial (hprec) cells were exposed to 0, 2, 4, 6, and 8 gy of ionizing radiation. Mirna expression levels were analyzed by quantitative pcr using snord48 as an internal control and calculated with the 2 − δδct method. In pc‐3 cells, mir‐20a‐5p, mir‐128‐3p, and mir‐135b‐5p showed significant dose‐dependent upregulation, whereas mir‐23b‐3p and mir‐381‐3p were downregulated. Mir‐128‐3p correlated positively with radiation dose, while mir‐23b‐3p and mir‐381‐3p showed negative correlations. Lncap cells exhibited moderate, non–dose‐dependent mirna changes. Distinct mirna signatures differentiate radiation‐resistant and radiation‐sensitive prostate cancer cells. Mir‐20a‐5p, mir‐128‐3p, and mir‐135b‐5p may contribute to radioresistance, whereas mir‐23b‐3p and mir‐381‐3p may act as radiosensitizers.
Seminal 5′tRF-Glu-CTC as a Biomarker for Oligozoospermia and ART Outcome Prediction Neslihan Hekim, Sezgin Gunes, Bulent Ayas, Elzem Nisa Alkan Basic and Clinical Andrology, 2025 Background Epigenetic mechanisms influencing sperm production and function are closely linked to male infertility. Among these, tRNA-derived fragments have recently been identified as important modulators of cellular stress responses and gene expression. The purpose of this study was evaluate the potential role of 5'tRF-Glu-CTC in the context of assisted reproductive technologies (ART) outcomes by comparing its differential expression in the seminal plasma of oligozoospermic and normozoospermic infertile men. Results Total RNA was extracted from seminal plasma, and 5'tRF-Glu-CTC expression was measured by qRT-PCR. Sperm DNA fragmentation index (DFI) was assessed via the TUNEL assay. The results revealed significantly elevated 5'tRF-Glu-CTC expression in the oligozoospermic group compared to the normozoospermic group (15.58 ± 4.34 vs. 12.53 ± 4.99; fold change: 1.692; P = 0.024). Sperm DFI showed a significant negative correlation with both the progressive motile sperm (ρ = -0.537, P = 0.015) and the total progressive motile sperm count (ρ = -0.509, P = 0.026). However no significant differences in DFI was observed between the oligozoospermic and control groups (P > 0.05). Analysis of ART outcomes revealed that male partners of couples who achieved pregnancy had lower mean 5'tRF-Glu-CTC expression and DFI, although these differences did not reach statistical significance (P > 0.05). Conclusions These results suggest that 5'tRF-Glu-CTC might be serve as a potential biomarker and could play a role in the pathophysiology of oligozoospermia.
Evaluation of the semen microbiome for fertility in men with obesity using next-generation sequencing Elzem Nisa Alkan, Neslihan Hekim, Sezgin Gunes, Ramazan Asci, Ralf Henkel Basic and Clinical Andrology, 2025 Background The study aimed to evaluate the microbial content and diversity in semen samples of men with obesity, determine the differences between infertile and fertile groups, and investigate the effect of seminal microbiota on semen parameters, sperm DNA fragmentation, sperm chromatin condensation, and total antioxidant capacity. Results The study included thirteen infertile men with obesity as subjects and five fertile men with obesity as the control group (aged 18–55 years, body mass index > 30 kg/m²). The most abundant bacteria in both groups were seen to be belonging to the phylum of Bacillota , Pseudomonadota , Actinomycetota and Bacteroidota . The most common bacteria at the genus level were Pseudescherichia , Staphylococcus , Paenibacillus , Streptococcus , Klebsiella , and Moraxella , which had similar distributions in both groups. A negative correlation was observed between the percentage of aniline-positive sperm and motility ( p < 0.0001), sperm concentration ( p = 0.0001) and total sperm count ( p = 0.001). It was found that Brevibacterium , Paenibacillus , Alistipes , Lactiplantibacillus , Rhizobacter , Sphingomonas and Venlonella genera were correlated with sperm DNA fragmentation; Pantoea , Devosia , Bacteroides , Acidovorax were correlated with total antioxidant capacity, Fusobacterium was correlated with the histone-rich sperm, and Corynebacterium , Hydrogenophaga , and Paenalcagenes were associated with body mass index. Conclusion Bacterial species in semen may play a role in male infertility by affecting semen quality, sperm DNA fragmentation or total antioxidant capacity.

Russo, Selahittin Cayan, Rajender Singh, Eric Chung, Carlo Giulioni, Gian Maria Busetto, Ashok Agarwal World Journal of Men S Health, 2025 PURPOSE: The objective of this manuscript is to assess the effect of varicocele repair (VR) in patients with clinical varicoceles on serum total testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH), and inhibin B serum levels. MATERIALS AND METHODS: The study was performed in compliance with the Meta-Analysis and Systematic Reviews of Observational Studies (MOOSE) guidelines and the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols (PRISMA-P). All eligible studies were selected following the PICOS (Population, Intervention, Comparison/Comparator, Outcomes, Study design) model. The values of each outcome measured after VR were compared to the before parameters and, when available, to the values on patients with unrepaired varicocele, and to those of healthy controls with no varicocele. For total testosterone, the values were sub-analyzed based on the mean total testosterone levels before VR (<300 ng/dL or >300 ng/dL), the fertility status, the time of follow-up and the technique used for VR. RESULTS: From a total of 460 abstracts retrieved, 48 articles were included in our meta-analysis. Serum total testosterone levels were significantly higher after VR compared to both pre-treatment levels (mean difference [MD] 82.45 ng/dL, 95% confidence interval [CI]: 64.14-100.76; p<0.00001) and to the levels of patients with unrepaired varicocele (MD 91.64 ng/dL, 95% CI: 62.30-120.99; p<0.00001). They did not differ from the levels of healthy controls with no varicocele (MD -22.01 ng/dL, 95% CI: -68.59-24.58; p=0.35). The increase resulted to be independent from the mean total testosterone levels before VR, fertility status, time of follow-up and type of VR. After VR, a trend toward lower serum LH levels was found compared to before values (MD -0.37 IU/L, 95% CI: -0.74-0.01; p=0.06). When compared to the levels of patients with unrepaired VR, LH levels after VR were significantly lower (MD -0.96 IU/L, 95% CI: -1.56 to -0.35; p=0.002). LH levels were not significantly higher than healthy men without varicocele (MD 0.84 IU/L, 95% CI: -0.68-2.36; p=0.28). Patients with VR had significantly lower FSH levels compared to their pre-treatment values (MD -1.43 IU/L, 95% CI: -1.82 to -1.04; p<0.00001), and also to those of patients with non-repaired varicocele (MD -2.35 IU/L, 95% CI: -4.06 to -0.65; p=0.007). When compared to healthy controls with no varicocele, FSH levels were significantly higher (MD 2.71 IU/L, 95% CI: 1.12-4.31; p=0.0009). Lastly, after VR no significant change in inhibin B serum levels was seen compared to pre-treatment levels (MD 11.76 pg/mL, 95% CI: -3.83-27.35; p=0.14). CONCLUSIONS: The present meta-analysis is the largest to date to assess the impact of VR on Leydig cell and Sertoli cell function using a before-after analysis for uncontrolled studies, and using data from patients with unrepaired varicoceles or healthy patients without varicocele as controls. VR was found to increase and restore to normality serum levels of total testosterone and LH. This evidence could be of value in considering the treatment of varicocele in patients with low testosterone or those who show a progressive decline in testosterone levels.

Although various factors are well defined in the disruption of spermatogenesis, not all aspects due to the heterogeneity of the disorder have been determined yet. In this review, we focus on the recent findings and summarize the current data on epigenetic mechanisms such as DNA methylation and different metabolites produced during methylation and demethylation and various types of small noncoding RNAs involved in the pathogenesis of different groups of NOA.
