  • Case Report on Drug Reaction with Eosinophilia and Systemic Symptom (DRESS) Following Levetiracetam Treatment

    A BSTRACT Drug reaction with eosinophilia and systemic symptom (DRESS) is an idiosyncratic severe cutaneous adverse reaction and is characterized by a skin rash with systemic involvement. It is important to diagnose DRESS early with immediate cessation of the suspected offending drug. Patients with DRESS syndrome should be managed in an intensive care setup for appropriate supportive care and infection control. Topical corticosteroids can give symptomatic relief, but systemic therapy with steroids and other immunosuppressant is usually required. We present a case of DRESS syndrome in a 48-year-old woman with chronic kidney disease and hypertension due to levetiracetam given for seizure disorder.

    Folliculitis decalvans (FD) is a rare form of primary cicatricial alopecia, characterized by chronic inflammation and destruction of hair follicles, occurring predominantly in young to middle-aged men and uncommon in women under 30 years of age. Treatment for FD is mainly centered around the use of antimicrobial agents for the clearance of S. aureus. Isotretinoin, an analog of vitamin A, has been shown to have direct immunomodulatory effects in addition to its primary action on sebaceous glands. In this report, we present a unique instance of FD in two young sisters, treated with low-dose isotretinoin and erythromycin.

    Genital ulcers may be located on the genital region and may be due to infectious or noninfectious etiology. Although sexually transmitted infections are a common cause of genital lesions, it is essential to consider nonvenereal disorders in the differential diagnosis. Squamous cell carcinoma appearing in lesions of chronic vulval lichen sclerosus et atrophicus is a rare occurrence of clinical significance. We present a case report of a 62-year-old postmenopausal woman who presented with a long-standing history of vulval pruritus, and mild pain.
  • The prevalence of dry eye in patients with psoriasis attending a tertiary care hospital in western Maharashtra

    : Psoriasis affects 0.5–11.4% of adults. Psoriasis is caused by a complicated chain of immune cells and cytokines, containing tumour necrosis factors (TNF), interleukin (IL)-17, 22, and 23. Psoriasis causes systemic inflammation, which can cause arthritis, cardiovascular disease, and metabolic syndrome. Our study explores dry eye in Western Maharashtra's psoriasis patients at a tertiary care hospital. A tertiary care hospital outpatient dermatology and ophthalmology department psoriasis patients were included in a cross-sectional research. Patient ages spanned from 20 to 60 years. The anterior and posterior ocular segments were examined, and dry eye was assessed. OSDI, Schirmer's test, TBUT, and corneal and conjunctiva staining has been scored. The PASI score has been received. Degree of dry eye was assessed using Dews dry eye grading system. Dry eye was present in 63.1 percent of people. Cases included 24.4% with mild dryness in their eyes. Moderate dryness made up 46.3% of cases, and severe dryness made up 29.3% of cases. The PASI score and dry eye did not have a statistically significant connection (P=0.355). A PASI score of 10 to 25 indicated the most severe case of dry eye, followed by a score of 10 (29 eyes) and >25. (23 eyes). A total of 81.5 percent of eyes had abnormal OSDI scores, 56.2 percent had abnormal TBUT scores, and 49.2 percent had abnormal Schirmer Test results. Dry eye in psoriasis patients requires dermatologist-ophthalmologist teamwork for comprehensive therapy. Psoriasis, especially in long-lasting cases, can lead to meibomian gland dysfunction.
  • Testicular Regression Syndrome: Histopathological Evaluation of 20 cases from Rural India with Review of Literature

    Context: Testicular regression syndrome (TRS), clinically termed “vanishing testes,” is a condition with atrophy of testes in late fetal or early neonatal life. The spermatic cord ends blindly or into a small fibrovascular nubbin. Histopathology reveals a fibrovascular nodule with associated hemosiderin-laden macrophages and dystrophic calcification. Few cases show the presence of seminiferous tubules and germ cells. The presence of germ cells is associated with the risk of developing a testicular germ cell tumor. Aims: To study the histopathological features of testicular nubbins and to determine the presence of germ cells and associated neoplasm. Settings and Design: It is a retro-prospective observational study carried out on patients referred to a pediatric surgical pathology center in Miraj, who were clinically diagnosed with undescended testis, over a period of 1 year. Data were collected after the study was approved by the IEC-MGCH on December 19, 2022. Materials and Methods: Twenty patients diagnosed with cryptorchidism, undescended testis, or atrophic testis were included. Findings of hematoxylin and eosin-stained slides of each case were noted, and Prussian Blue (special staining) was performed in four cases. Statistical analysis used: As this is a descriptive observational study, simple arithmetic calculations were performed. No unique statistical analysis was performed. Results: Among twenty cases of vanishing testes syndrome, 20 cases showed fibrovascular tissue only, and 1 case showed the presence of seminiferous tubules with Sertoli cells. Conclusions: Vanishing testes syndrome is a rare condition that is difficult to diagnose on clinical examination, and a definitive diagnosis may be missed if the pathologist is unaware of the condition. Also, there is a risk of germ cell tumor if there are residual germ cells in the testicular nubbin.
  • Deciphering the enigma of glomus tumors: A case series

    Glomus tumors are rare contributing to less than two percent of all soft tissue tumors. Given the rarity of this tumor, there is a possibility of low suspicion and a missed clinical diagnosis. This case series suggests diagnostic and morphological clues for early diagnosis of this tumor.This retrospective study includes nine interesting cases of glomus tumors from January 2015 to January 2024. Data was tabulated and analysed by descriptive statistics using Microsoft Excel 2016. In this series the glomus tumors showed a mean age of (37.67± 17.5 years). Male to female ratio was 1.25:1. Most common sites were digital parts of extremities (5/9, 55.56%). Four of these digital locations were subungual (4/5, 80%). The left side (5/9, 55.56%) was the most common laterality. Duration of symptoms till diagnosis was established showed a median of three months [range was one month to nine years]. All nine cases presented with pain. The mean size of glomus lesions was 2.1±1.5 cm). Multiplicity was observed in (2/9, 22.22%) cases. Eight cases were benign (8/9, 88.89%) while one was malignant (1/9, 11.11%). Two out of these eight benign cases were glomangiomatosis. Excision biopsy was the surgical procedure performed in all of our benign cases while the malignant case was treated with amputation of the finger.Diagnosis of glomus tumors requires a high index of clinical suspicion, especially in extra-digital locations. Awareness is essential for early diagnosis and treatment of this lesion, as it is an extremely painful condition and pain is out of proportion to the size of the lesion. Magnetic Resonance Imaging (MRI) is the first investigation of choice for imaging. An early biopsy can confirm the diagnosis of a glomus tumor in time. Complete surgical excision is curative with a low risk of recurrence.

  • A clinico-epidemiological study of breast eczema sparing nipple and areola from a tertiary care hospital in western Rajasthan

    Nipple eczema can be a local manifestation of atopic dermatitis/eczema when it arises with classic age-related patterns of eczema on other body sites, or it can occur in isolation. In our study, breast eczema sparing nipple and areola, was observed in females of western Rajasthan. This study reports the demography, clinico-morphological patterns and factors causing it. After due informed consent, thirty consecutive female patients of breast eczema were undergone a detailed history, clinical examination and patch testing. Hamilton Anxiety Rating Scale was used for anxiety severity assessment. Most common site of breast eczema was the upper-outer quadrant (15/30) followed by upper-inner quadrant (9/30). The cell-phone was the most common object which was kept in close contact with the breast followed by purse (26/30) and money (11/30). Total 20 positive patch tests were seen in 17 patients. Fifteen patch tests were relevant to the current dermatitis. Mild anxiety cases were significantly more in acute type whereas moderate-severe type was more with chronic type (p value=0.021). Larger study is required to establish the exact cause of this type of breast eczema. Breast eczema seems to have diverse etiology, supplemented by various factors such as anxiety, perspiration in hot arid weather, traditional loosely-fit inner clothing of women in this region of western Rajasthan and friction produced by the objects (cell phones) that are kept in contact with breasts, work together to produce dermatitis.
