Preoperative assessment of sarcopenia and osteosarcopenia prevalence in bariatric surgery candidates over 60 J. Molero, J. Vidal, J.R. Mendoza, S. Cañizares, M. Caballero, S. Logwin, A. Pané, A. de Hollanda, L. Flores Clinical Nutrition Espen, 2026 BACKGROUND: The prevalence of obesity in adults over 60 is rising, presenting challenges for healthcare systems. Bariatric surgery (BS) is increasingly considered for older adults, but complex health risk such as sarcopenic obesity (SO) and osteosarcopenic obesity (OSO) need evaluation to optimize clinical outcomes and guide preoperative risk stratification. OBJECTIVES: The aims was to describe the prevalence and severity of SO, OSO, and to examine the associated health consequences in BS candidates over 60 years. SETTING: This prospective cross-sectional study was conducted at a single tertiary care university hospital. METHODS: Cross-sectional study of 109 BS candidates over 60 years. The assessments included anthropometry, body composition (BC), muscle strength and function, bone mineral density (BMD), physical activity (PA) levels and metabolic pathologies associated with obesity. SO and OSO were defined using ESPEN/EASO (2022) consensus criteria. RESULTS: The mean age of the participants was 64.1 (2.9), and, 36 % were male. The prevalence of SO was 42 %, with no significant differences between sexes. This prevalence was primarily due to greater impairment in the 5-times sit-to-stand test (39.8 %). The prevalence of OSO was 26.8 %, largely attributed to a higher prevalence of osteopenia in females. Among obesity-related pathologies, only obstructive sleep apnea was more prevalent in males. Those with SO had a lower BMD and PA levels. CONCLUSIONS: The high prevalence of SO and OSO in BS candidates over 60 highlights the importance of comprehensive preoperative assessment. Sex and age-specific differences in BC and sarcopenia severity underscore the need for tailored interventions.

Umpierrez, María M Malagón Scientific Reports, 2025 Weight stigma remains a major public health issue that negatively impacts individuals living with obesity. This study explores its prevalence in the Spanish adult population and examines whether viewing obesity as a disease influences societal attitudes. A representative sample of 1,000 adults participated in a Computer-Assisted Telephone Interview assessing knowledge, stereotypes, prejudice, and discrimination related to obesity, along with sociodemographic data and personal beliefs about its causes. While 40.8% attributed obesity to a lack of self-control, 59.2% considered it a disease. Those in the latter group were more likely to recognize its multifactorial causes and support public funding for treatment. However, weight bias remained prevalent across both perspectives, with no significant differences in discriminatory attitudes. Around 30% of participants admitted to holding negative stereotypes or engaging in weight-based discrimination. These findings suggest that simply framing obesity as a disease does not meaningfully reduce weight stigma. Broader efforts are needed-beyond education alone-to challenge societal narratives and address structural contributors to bias, ultimately fostering a more supportive and inclusive environment for individuals affected by obesity.

Purpose This 1-year cross-sectional study in potential BS candidates had several objectives: (a) assess the prevalence of risky drinking, alcohol use disorder (AUD), and other substance use/disorder; (b) compare the prevalence of these behaviors to that of the general Spanish population; (c) determine the proportion of patients with positive results in toxicology tests; and (d) study the predictive factors of risky drinking. Setting: tertiary university hospital. Materials and Methods Alcohol and other substance use were evaluated with the AUDIT-C and ASSIST questionnaires. Urine tests analyzed several markers (ethyl-glucoronide [EtG] ≥ 500 ng/ml, amphetamine, benzodiazepine, cannabinoid, cocaine, and opioid). The Mini-International-Neuropsychiatric-Interview (5.0.0) was employed to assess psychiatric diagnoses. Results Among 308 candidates for BS, 196 were accepted to participate (69% women; mean age 46.7 ± 10.9 years; mean body mass index 45.6 ± 5.9). AUDIT-C and ASSIST identified 7% and 5% of risky drinkers, respectively. Men were more frequently risky drinkers compared to women (18% vs. 2%) and compared to the general population (18% vs. 8%). Six percent of individuals had AUD, being men the most affected, and 2% met criteria for other substance disorder. Fifteen percent of the sample presented risky tobacco use. Cannabis was self-reported only by males (3%). EtG ≥ 500 ng/ml was present in 15% of the sample, being a risk factor for risky drinking together with the male sex. Conclusion Identification of candidates at risk for risky drinking can help to prevent any alcohol misuse after BS. The combination of subjective and objective measures improves the validity of the assessment of substance use. Graphical Abstract
