  • Differences and Curriculum Compliance Among Infectious Diseases and Clinical Microbiology Residency Training Clinics in Türkiye: A Nationwide Survey

Objective: Infectious Diseases and Clinical Microbiology (IDCM) residency training in Türkiye is structured according to the national core curriculum of the Medical Specialization Board. However, notable variations exist in its implementation across training institutions. This study evaluated the extent of curriculum implementation, educational processes, and inter-institutional differences among IDCM training clinics in Türkiye. Methods: This cross-sectional survey included all institutions authorized to provide IDCM residency training in Türkiye. Data were collected through face-to-face or online administration of a 52-item questionnaire to IDCM residents and early-career specialists. The questionnaire addressed demographic characteristics, academic staffing, curriculum-based educational practices, clinical infrastructure, and working conditions. Data analysis employed descriptive statistics and comparative tests. Results: One hundred participants, representing all IDCM training clinics in Türkiye (n=87), were included. The median participant age was 30 (25–40) years, and 71% were female. Clinics had a median of 6 (1–13) faculty members and 11 (1–55) residents. In 40% of clinics (35/87), the faculty-to-resident ratio was below 1:3. No statistically significant differences were observed among hospital types regarding faculty-to-resident ratio (p=0.134) or number of residents (p=0.127). However, city hospitals had significantly more faculty members (p=0.037). Only 22% of clinics (19/87) provided a separate to rest during night shifts, and 53% (46/87) reported fewer number of computers than residents. Approximately half of the clinics lacked an in-house microbiology laboratory (44/87; 50.5%). Presentations were the most common educational activity (98.9%), while courses were least frequent (13.8%). Faculty participation as presenters during seminars or presentations was significantly higher in university and training and research hospitals (p=0.006). Conclusion: The findings indicate a need to improve residents’ workload, educational quality, and working conditions. Marked differences exist among IDCM training clinics in Türkiye regarding the implementation of the core curriculum and clinical practices. This study provides comprehensive nationwide data to monitor curriculum compliance and inform the development of residency training.
  • Clinical and microbiological characteristics of Nocardia infections: a 10-Year multicentre study from Türkiye

European Journal of Clinical Microbiology and Infectious Diseases, 2026
Multicentre studies from countries such as Spain, Australia, Japan, China, and Taiwan have revealed significant geographic variation in Nocardia species patterns, resistance profiles, and patient outcomes. In Türkiye, however, most available data originate from single-centre reports with limited species-level identification and antimicrobial resistance profiling, highlighting a significant gap in the current understanding of nocardiosis at the national level. Our aim was to evaluate the clinical and microbiological characteristics of Nocardia infections in Türkiye. In this multicentre, retrospective observational cohort study, adult patients (≥ 18 years) diagnosed with microbiologically confirmed nocardiosis between January 1, 2014, and December 31, 2024 from 18 tertiary care hospitals in Türkiye were examined. 109 microbiologically confirmed nocardiosis cases were identified, with a mean age of 55.9 years (median 59, range 18–80) and a predominance of male patients (66.1%). The most common presenting symptom was productive cough (40.3%), and sputum was the most frequent specimen type yielding Nocardia isolates (41.2%). Species-level identification was achieved in 51 cases, with Nocardia farcinica (41.2%), N. cyriacigeorgica (29.4%) being the most prevalent species. In multivariate logistic regression, increasing age was independently associated with mortality (OR = 1.053, 95% CI 1.012–1.096, p = 0.011) and the CCI was independently associated with mortality (OR = 1.244, 95% CI 1.043–1.483, p = 0.015). This study demonstrates that clinical outcomes in nocardiosis are primarily dictated by patient-intrinsic factors, namely advanced age and cumulative comorbidity burden. This finding requires confirmation in prospective studies.

  • A Case of Leukocytoclastic Vasculitis Related to Acute Parvovirus B19 Infection

Infectious Diseases and Clinical Microbiology, 2025
Parvovirus B19 (PVB19) infection can cause various clinical presentations. While mostly erythema infectiosum, transient aplastic crisis, and fetal hydrops are seen, it rarely causes myocarditis, hepatitis, or vasculitis. We report a patient admitted with fever, fatigue, arthralgia, and rash. Although there was no aetiological clue in the first-line studies, the tests were repeated because of clinical suspicion. Therefore, >4-fold increase in PVB19 IgM titer, IgG seroconversion, and pathological findings led to the diagnosis of leukocytoclastic vasculitis (LCV) related to PVB19. We aimed to emphasize the importance of testing repetition in high clinical suspicion.
