    Dear Editor, The present letter refers to the Pulitzer Prize-winning book on the history of Cancer, “The Emperor of All Maladies: A Biography of Cancer,”[1] written by Siddhartha Mukherjee. Understanding the historical aspects of cancer is crucial in developing the most sophisticated and effective treatment of the disease. In this aspect, reading Siddhartha Mukherjee’s book will give physicians and researchers in the field an added advantage in working on the subject with new vigor. The book is presented in six parts. It begins with the story of a patient named Carla, who is fighting leukemia. The description of sickness and the physiopathological changes in the body are given in layperson’s terms, as seen through the eyes of a patient, doctor, researcher, surgeon, social activist, or even an administrator or policy maker. It is an eye-opener for the scientists in oncology to do more on the preventive methods for cancer. A single reading of the book will reveal the different stages of the disease and its terrific forms – like leukemia, breast cancer, lung cancer, lymphoma, Hodgkin’s lymphoma, esophageal cancer, gastrointestinal stomach tumor, and so on. In Part One, the mechanism of the development of leukemia and its treatment are depicted. It starts with the history of leukemia pathology, from ‘a suppuration of blood’ to ‘white blood’ by early physicians and researchers. Then he describes the further efforts of later researchers, who not just wanted to comprehend cancer by ‘invisible forces’ like miasmas, neuroses, bad humor, and hysterias but also wanted a better understanding, leading to the development of the cellular theory. Sydney Farber’s contributions to leukemia treatment are elaborated. The chapter hints at the importance of environmental factors while describing historical accounts of stomach cancer. He comments that Galen was probably right about the ‘humor which causes tumors.’ Though surgery was applied, the cancer ‘crab’ was still elusive. In Part Two, the story of Mary Lasker speaks volumes of the importance of fundraising for treatment and research in cancer. The importance of basic research is elucidated, and understanding the basic cellular mechanisms, which demand intensive research, is explained. Part Three echoes the cry of a cancer patient who can no longer take the suffering. This chapter gives a stimulating account of the developments in cancer research, from the beginning of randomized controlled surgical trials to the advent of palliative care and the need for preventive medicine for cancer. Part Four starts with the search for causes of cancer with the story of the ‘chimney sweeper boys’ affected with scrotal cancer. Thus began a new chapter in cancer prevention – Cancer Epidemiology. Further, the author takes the readers to Preventive Screening for Cancers by describing the Pap smear test and its application in female pathological conditions. In Part Five, the author further describes how the fight against cancer turned inward to basic biology and toward fundamental mechanisms. Readers will also come across three theories: genes cause cancers, exogenous chemicals cause cancers, and viruses cause cancers, which have been put forth in the field of cancer epidemiology. Accounts of the six essential alterations in cell physiology that dictate malignant growth compel the readers to reason about cancer. In Part Six, the shift from targeted therapy to the molecular world in cancer studies is described, from cellular physiology to sequencing cancer genome and from descriptive biology to functional biology of cancer. Atossa’s story is more like a time travel into the history of cancer. In the time of Hippocrates (500 BC), the disease was known as KARKINOS or crab. Galen expounded the cause as ‘black bile’ (168 AD). The author ends his work with Germaine’s story. a cancer patient suffering from the gastrointestinal stromal tumor (GIST), which shows the plight of cancer patients who, due to their inherent genes, go through twists and turns in their lives and start living a new normal of ‘living with cancer’ and leave the world ‘just a little earlier.’ Summary As the writer puts it, the book is not a history but a biography of the monster called Cancer. It comes in various forms, affecting multiple parts or organs of the body, indicating fundamental errors in the homoeostatic mechanisms, as understood by physicians of ancient times, such as Galen. The limitations of surgery alone applied in cancer also suggest the same. The name of the disease, Karkinos or Crab, shows the relationship of Greek medicine with Indian Knowledge Systems like Ayurveda. Crab in Sanskrit is called ‘Karkitaka’. Though the history of cancer in Indian systems of medicine is not mentioned in the book, this review can help fill the gap by pointing toward the need to understand cancer in terms of Ayurveda for its prevention and find simple and cost-effective medicines for its cure and palliation. Understanding the Tridosha theory of Ayurveda in cancer pathology and treatment can help oncologists and Ayurvedists work together. The advancements in medicine and the development of cancer medicine or oncology as a highly developed branch of medicine are fascinating. However, we still have miles to go to find a complete cure. This exceptional academic work on the history of cancer should find its worthy place in the library of any person directly or indirectly dealing with cancer management.
