    Importance Total thyroidectomy is the established surgical standard for hereditary and sporadic medullary thyroid cancer (sMTC). However, for unilateral sporadic tumors, its benefit over lobectomy remains uncertain. Objective To compare oncologic outcomes between total thyroidectomy and lobectomy in patients with sMTC. Data Sources and Study Selection MEDLINE, Embase, Scopus, and Cochrane Central Register of Controlled Trials were searched from inception to December 2025 to identify comparative studies on patients with sMTC who underwent total thyroidectomy or lobectomy. Data Extraction and Synthesis Reviewers working independently and in duplicate screened titles, abstracts, and full-text articles for eligibility using standardized instructions. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses ( PRISMA ) and Meta-Analysis of Observational Studies in Epidemiology ( MOOSE ) guidelines were used for reporting. Main Outcomes and Measures Primary outcomes were mortality, overall survival, structural recurrence, biochemical cure, and distant metastasis development. The secondary outcome was postoperative complications. Effect measures were calculated as odds ratios (ORs) or relative risks (RRs) with 95% CIs using a random-effects model. The risk of bias was assessed using the Newcastle-Ottawa Scale. Results Nine retrospective studies met inclusion criteria and comprised 1371 patients (295 of 397 patients [74.3%] with documented sex were female, and median age ranged from 45.0 to 58.2 years). A total of 531 patients (38.7%) underwent lobectomy and 840 (61.3%) underwent total thyroidectomy. Of 341 tumors, 280 (82.1%) lacked extrathyroidal extension, 284 of 401 tumors (70.8%) measured smaller than 2 cm (70.8%), and 197 of 343 tumors (57.4%) were node negative; central neck dissection was performed in 445 of 492 patients (90.4%). Multifocal disease was uncommon and reported in 30 of 330 patients (9.1%). Mortality did not differ at 5 years (RR, 0.30; 95% CI, 0.07-1.35) or beyond (RR, 1.00; 95% CI, 0.40-2.47). Overall survival at 5 years was similar (RR, 1.02; 95% CI, 0.94-1.11). Total thyroidectomy was not associated with lower structural recurrence rates at 5 years (OR, 0.45; 95% CI, 0.14-1.49), but it was associated beyond 5 years (OR, 7.26; 95% CI, 1.07-49.21). No differences were observed for biochemical cure at 5 years (OR, 0.86; 95% CI, 0.47-1.56) or beyond 5 years (OR, 0.87; 95% CI, 0.26-2.89). Distant metastasis development did not differ at 5 years (OR, 1.64; 95% CI, 0.09-31.52). Sensitivity analyses revealed no differences across any outcomes. Postoperative complications were more common in total thyroidectomy. Risk of bias was high in 4 studies, moderate in 4, and low in 1. Conclusions and Relevance Findings in this systematic review and meta-analysis, based on data from retrospective studies, suggest that thyroid lobectomy may be associated with oncologic outcomes comparable to total thyroidectomy in selected patients with sMTC.
  • Transvaginal radiofrequency ablation for uterine myomas: A preliminary ecuadorian study

    Background: Transvaginal radiofrequency ablation (TVRFA) is a minimally invasive, incisionless, outpatient procedure. This study evaluates the feasibility, outcomes, and complications of TVRFA in women with symptomatic uterine myomas (UMs). Methods: This preliminary single-center, retrospective study included 79 women treated with TVRFA between December 2021 and April 2024 in Quito, Ecuador. Women with 1-4 UMs of any size or type were eligible. Preoperative and follow-up assessments included transvaginal ultrasound (TVUS) for both uterine and myoma volume. Outcomes (uterine and myoma volume + complications) were assessed at 45 days and 6 months. Complications were classified by Clavien-Dindo. Statistical analyses used paired tests for baseline vs. follow-up comparisons, with missing data excluded from each analysis. Results: The median baseline uterine volume was 160.0 mL. At 6 months, this decreased to 91.0 mL (-43.1 %, p < 0.001). The median baseline myoma volume was 22.5 mL, which decreased to 7.7 mL at 45 days (-55.1 %) and 3.7 mL at 6 months (-81.5 %, p < 0.001). Follow-up was completed by 68/79 patients (86.1 %) at 45 days and 59/79 (74.7 %) at 6 months. Minor complications occurred in 12.7 %, most commonly skin burns at the electrode site. Two patients (2.5 %) experienced intestinal perforation requiring surgery. Conclusion: TVRFA significantly reduced both uterine and myoma volumes, supporting its role as a minimally invasive alternative for fibroid management. However, the observed complication rate, particularly intestinal perforation, requires critical attention to operator training and procedural refinements. Prospective studies with standardized symptom measures are warranted.
  • Pre-Radioiodine Thyrotropin Thresholds During Withdrawal Preparation in Differentiated Thyroid Cancer after Total Thyroidectomy: A Systematic Review and Meta-Analysis

    Background: Preparation for radioiodine (RAI) therapy in differentiated thyroid cancer (DTC) often requires thyroid hormone withdrawal (THW) to achieve thyrotropin (TSH) stimulation. Current guidelines recommend TSH ≥30 mIU/L, a target that prolongs hypothyroidism and worsens quality of life, yet rests on limited evidence. Objective: To evaluate the association between pre-RAI TSH levels and oncologic outcomes in adults with DTC prepared by THW. Methods: We conducted a systematic review and meta-analysis of studies comparing outcomes across pre-RAI TSH thresholds (&lt;30 vs. ≥30, &lt;60 vs. ≥60, and &lt;90 vs. ≥90 mIU/L) in adults with DTC prepared by THW. Primary outcomes included disease-specific mortality, recurrence, and response to therapy. Searches in MEDLINE, Embase, Cochrane, and Scopus (inception to March 2025) identified eligible studies. Risk of bias was assessed using the CLARITY tool and certainty of evidence using GRADE. Pooled relative risks (RRs) were calculated using random-effects models. This systematic review was registered in PROSPERO (CRD42020158354). Results: This meta-analysis included eight retrospective cohort studies comprising a total of 4651 DTC patients, predominantly women (68.5%) with a mean age of 46 years. All patients underwent THW before RAI. Across all TSH thresholds examined (&lt;30, &lt;60, and &lt;90 mIU/L), higher pre-RAI TSH levels were not associated with better treatment response, lower recurrence, or reduced mortality. Specifically, patients with higher pre-RAI TSH levels (≥30 mIU/L) did not have better oncologic outcomes compared with those with lower levels (&lt;30 mIU/L). At 2- and 3-year follow-up, no significant differences were observed in excellent (pooled RR = 0.87; confidence interval [CI] 0.68–1.11) or indeterminate (RR = 1.28; CI 0.72–2.27) response rates. Similarly, recurrence rates did not differ (RR = 1.45; CI 0.83–2.55), and no study demonstrated a difference in disease-specific mortality across follow-up periods extending up to 10 years. The certainty of evidence for all outcomes was rated very low due to risk of bias, heterogeneity, and imprecision. Conclusions: The current evidence base is insufficient to support or refute the routine use of a specific TSH threshold before RAI administration. These findings question the recommendation to achieve TSH ≥30 mIU/L before RAI and highlight the need for trials to define the minimal effective level of TSH stimulation.

  • Minimally invasive converted to open versus upfront open surgeries for rectal cancer: a retrospective cohort study

    Background Minimally invasive surgery (MIS) has significantly better short-term outcomes than open surgery. However, it is unclear whether MIS converted (MISC) to open for rectal cancer carries any benefits compared to open as no prior study has compared these two surgical approaches for rectal cancer. This study compares short-term postoperative outcomes between MISC and upfront open surgery. Methods This is a retrospective cohort study on elective surgeries for rectal cancer from 2016 and 2022 from the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database. Patients were divided into the MISC group and the Open group and balanced using propensity-score matching. 30-day complication, unplanned readmission, and mortality rates and length of stays were compared with Student’s t-test and regression models. Results The Open group included 2551 (86.3%) patients, and the MISC group included 406 (13.7%) patients (80.5% laparoscopy, 19.5% robotic). Propensity-score matching yielded 400 patients in each group. On univariate analysis, the groups had comparable overall complications, unplanned readmission, and 30-day mortality rates, but the MISC group had a lower median length of stay (6 days vs. 7 days, p = 0.015). On regression analysis, the MISC was not associated with an increased risk for overall complications (OR = 1.04, 95%CI 0.843–1.280; p = 0.716), unplanned readmission (OR = 1.230, 95%CI 0.939–1.620; p = 0.132), and 30-day mortality (OR = 1.750 95%CI 0.648–4750; p = 0.269). However, MISC was significantly associated with a shorter length of stay (OR = − 0.869, 95%CI (−)1.609 − (−)0.129; p = 0.021). Conclusion Patients undergoing planned open proctectomy and those undergoing MISC to open proctectomy exhibit similar 30-day postoperative outcomes. As completed MIS offers advantages over open surgeries, MIS should be attempted as the default surgical approach for rectal cancer patients, even if conversion to open surgery is needed.
  • Liver metastases from oropharyngeal squamous cell carcinoma: a rare case report

    Oropharyngeal squamous cell carcinoma (OPSCC) commonly metastasizes to the lungs, bones, and distant lymph nodes. Liver metastases from OPSCC are rare and typically occur in advanced stages. We present a 59-year-old male diagnosed with OPSCC, initially treated with surgery followed by adjuvant radiotherapy (RDT). Due to political and governmental limitations, the patient received only six sessions of RDT, below the standard recommended regimen. Three years post-treatment, he presented with abdominal symptoms, and imaging revealed a hepatic lesion. A liver biopsy confirmed metastatic squamous cell carcinoma. Histopathological comparison with the primary tumor supported the diagnosis of hepatic metastasis from OPSCC. This case underscores the aggressive nature of OPSCC and the critical importance of completing multimodal therapy. Interrupted adjuvant treatment may have contributed to early distant spread. While liver metastases from OPSCC are rare, they should be considered in patients presenting with compatible symptoms and a history of incomplete treatment.

    IntroductionDifferentiated thyroid cancer (DTC) is the most common endocrine malignancy, with a high 5-year survival rate of approximately 98%. Despite advances in diagnosis and treatment, up to 20% of patients experience recurrence, adversely affecting their quality of life. Predictive models have been developed to assess recurrence risk and guide clinical decision-making, but these models often face limitations such as retrospective design, lack of diversity in study populations and absence of external validation. The primary aim is to externally validate existing predictive models for DTC recurrence using prospective data from a diverse Latin American cohort. The secondary aim is to explore opportunities for model recalibration to improve their performance in our population.Methods and analysisThe CaTaLiNA study is a multicentre prospective observational study conducted across 10 hospitals in five Latin American countries, including Ecuador, Peru, Uruguay and Mexico. Patients aged 18 years or older receiving treatment for DTC, such as the first thyroid surgery, active surveillance or radiofrequency ablation will be included. Recruitment will occur from November 2023 to June 2025, with follow-up extending until June 2028. Data collection will include baseline clinical, surgical and histological characteristics, treatment details and follow-up outcomes. Statistical analysis will follow the Transparent Reporting of a multivariable prediction model for Individual Prognosis or Diagnosis guidelines, using imputation strategies for missing data and evaluating calibration and discrimination of the prediction models. Calibration measures include the ratio of expected and observed events, calibration slope and calibration plot, while discrimination will be assessed using the C-index and area under the receiver operating characteristic curve.Ethics and disseminationThis study protocol was approved by Comité de Ética de Investigación en Seres Humanos de la Universidad San Francisco de Quito USFQ ‘CEISH-USFQ’ APO-010–2023-CEIHS-USFQ Oficio No. 161-2023-CA-23030M-CEISH-USFQ. Results will be disseminated via peer-reviewed publications.
