  • Spatial and spatio-temporal distribution of infant mortality in the state of Sergipe, Brazil: An ecological study, 2019–2023

    OBJECTIVES: To analyze the spatial and spatiotemporal distribution of infant mortality in the state of Sergipe from 2019 to 2023. STUDY DESIGN: Ecological study. METHODS: This ecological study used publicly available data on infant deaths in Sergipe between 2019 and 2023, obtained from the Mortality Information System. Variables related to the child, mother, and healthcare factors were considered. Bayesian mortality rates were calculated and mapped across municipalities. Moran's Index was applied to assess spatial correlation. RESULTS: Between 2019 and 2023, Sergipe recorded 2547 infant deaths. The leading causes were perinatal conditions (Chapter XVI) and abnormal findings in examinations (Chapter XVIII), predominantly in the northern region. Higher mortality was observed among Black female infants with very low or low birth weight, as well as clusters of deaths among adolescent mothers in the Itabaiana region, and adult or late-age mothers in Propriá, which also showed higher mortality among preterm infants. The municipalities of Propriá and Nossa Senhora do Socorro reported more deaths associated with cesarean deliveries. Aracaju, São Cristóvão, and Nossa Senhora do Socorro formed high-mortality clusters, with a risk 641.77 times greater; conversely, Nossa Senhora da Glória, Lagarto, and Estância exhibited a 66 % lower risk. CONCLUSION: Infant mortality in Sergipe is unevenly distributed, concentrated in high-risk clusters, and reflects regional disparities as well as specific maternal and neonatal factors.
  • COVID-19 in the Neonatal Period in a Reference Maternity for High-Risk Pregnancy: A Hospital-Based Case-Control Study

    COVID-19 in newborns presents a multifaceted clinical spectrum, with the potential for severe outcomes. This study aimed to evaluate the clinical evolution and hospital outcomes of neonates with a molecular diagnosis of COVID-19. A case-control study was conducted in a public referral maternity hospital for high-risk pregnancies. Two controls were selected for each case, matched by sex and gestational age. Variables related to birth data, symptoms, and clinical progression were collected from medical records and analyzed statistically, with crude and adjusted relative risks calculated using Poisson regression with robust standard errors. A total of 25 neonates with confirmed SARS-CoV-2 infection were identified among 875 newborns. Compared with controls, infected neonates had a longer hospital stay (median of 19 days vs. 8 days; p < 0.001) and higher readmission rates (16% vs. 0%; p = 0.03). After adjusting for potential confounders, COVID-19 infection was associated with a 2.41-fold higher risk of neonatal death (95% CI: 1.24–4.67; p = 0.009). No evidence of vertical transmission was found. These findings suggest that neonates with COVID-19 may experience longer hospitalizations and an adjusted higher risk of mortality, emphasizing the need for vigilant surveillance and supportive care. However, given the observational design of the study, these results indicate associations rather than causal relationships. Understanding the clinical behavior of COVID-19 in this population—characterized by inherently low immunity—and recognizing its interaction with other neonatal conditions are essential for improving hospital management and outcomes.
  • Incidence and mortality by pulmonary tuberculosis in Brazil: Trends and projections, 2002-2034

    • The North region presented the worst scenario in the country. • High adjusted incidence and mortality rates were observed among men. • In general, the incidence and mortality decreased in Brazil by 2034. • Among females, there was a reduction in the risk of dying from tuberculosis. • It was projected an increase in death risk in the North, South, and Center-west. To analyze the temporal trend of incidence and mortality from pulmonary tuberculosis in Brazil from 2002 to 2019, and to project these trends until 2034. Ecological study with tuberculosis cases extracted from the Disease Notification and Mortality System in Brazil from 2002 to 2019. The age-period-cohort model was used for projection until 2034 using R. Subsequently, the percentage variation was estimated using Joinpoint. Brazil recorded 1,093,070 new cases and 76,205 deaths from 2002-2019, and projections util 2035-2034 estimated 1,192,092 new cases and 67,532 deaths. The North region had the highest standardized incidence and mortality rates in the country for both sexes. An increase in deaths among men and reduction among women was projected, along with an increase in incidence in both sexes. About 36% of the increase in incidence and 34.1% of the mortality among men was explained by a rise in disease risk. Among women, 11.7% of the increase in incidence was due to population growth, while 44.8% of the reduction in deaths was due to lower risk. The North presented the worst scenario in the country. The projections are not favorable to the globally established targets. An increase in incidence was projected for both men and women, with an increase in deaths only among men. More efforts are needed to change this potential scenario.
  • Trend of epidemiological indicators and factors associated with tuberculosis treatment dropout and death among street people in Brazil: an ecological and cross-sectional study, 2014-2022

    Resumo Objetivo Analisar a tendência temporal dos indicadores epidemiológicos e os fatores associados ao abandono de tratamento e óbito pela tuberculose em pessoas em situação de rua. Métodos Estudo, transversal sobre tuberculose em pessoas em situação de rua no Brasil de 2014-2022, usando dados do Sistema de Informação de Agravos de Notificação. Calcularam-se os indicadores de cura e abandono, incidência e mortalidade. Estimou-se a tendência temporal dos indicadores pela regressão de Prais-Winsten, expressa via variação percentual. Utilizaram-se variáveis presentes na ficha de notificação para estimar os fatores associados ao abandono e ao óbito pela tuberculose usando regressão logística, que resulta na razão de chances (odds ratio, OR) e intervalo de confiança de 95% (IC95%). Resultados Foram notificados 21.904 casos de tuberculose. A cura foi -5,8% (IC95% -7,9; -3,8) no Brasil, a incidência, -6,2% (IC95% -8,9; -3,5) e a mortalidade, -5,3% (IC95% -10,0; -0,4) na região Sudeste. Os preditores do abandono foram: tratamento diretamente observado ausente (OR 3,92; IC95% 3,65; 4,20), 20-39 anos (OR 1,81; IC95% 1,41; 2,33) e uso de drogas ilícitas (OR 1,57; IC95% 1,46; 1,69). Os fatores associados ao óbito foram: tratamento observado ausente (OR 3,18; IC95% 2,86; 3,54), >60 anos (OR 3,14; IC95% 2,14; 4,61) e uso de drogas ilícitas (OR 1,18; IC95% 1,05; 1,34). Conclusão O cenário da tuberculose nessa população é preocupante e seu contexto desafiador requer mais esforços para melhor rastreamento e tratamento, considerando-se os múltiplos fatores de risco identificados.

    Objective: The objectives of this study were: (1) to analyze the temporal trend of tuberculosis treatment outcomes in the state of Sergipe; (2) to identify the existence of seasonality of tuberculosis; (3) to verify the influence of the rapid molecular test (MTB-RIF) in the time series of tuberculosis and its treatment outcomes in the state of Sergipe; and (4) to verify treatment outcomes. Methods: Ecological study on tuberculosis and three treatment outcomes (cure, interruption of treatment, and death) extracted from Datasus. Incidence and mortality rates were calculated for the crude occurrences of cases and deaths and the proportions of cure and interruption of treatment (%). The time series was analyzed using Prais-Winsten regression from Jan to Dec/2021. Results: The total incidence rate was 36.35 cases per 100,000 inhabitants, with an increase of 0.44% per month (95%CI 0.35; 0.54). The cure rate was 64.0% with a steady trend (p>0.05). The percentage of treatment interruption was 13.3%, with a reduction of −0.73%/month (95%CI −1.11; −0.34). The total mortality rate was 1.92 deaths/100,000 inhabitants with a stationary trend. After the implementation of the MTB-RIF, there was an increase in the incidence rate of 0.65% per month. Seasonality was not identified in any of the analyses performed (p>0.05). Conclusion: There was an increase in incidence rates, reduction in treatment interruption and mortality in the state of Sergipe. Seasonality was not identified. The rapid molecular test showed a growth effect on the incidence rate.
  • Hepatitis mortality in Brazil and regions, 2001– 2020: temporal trend and spatial analysis

    Objective: To analyze the spatial distribution and the temporal trend of the hepatitis mortality rate in Brazil from 2001 to 2020. Methods: Ecological, temporal, and spatial study on mortality from hepatitis in Brazil with data from the Mortality Information System (Sistema de Informações sobre Mortalidade – SIM/DATASUS). Information was stratified by year of diagnosis, region of the country, municipalities (of residence). Standardized mortality rates (SMR) were calculated. The temporal trend was estimated by Prais-Winsten regression and the spatial distribution by the Global Moran Index (GMI). Results: The highest SMR means in Brazil were for Chronic viral hepatitis with 0.88 deaths per 100,000 inhabitants (SD=0.16), followed by Other viral hepatitis with 0.22/100,000 (SD=0.11). In Brazil, the temporal trend of mortality from Hepatitis A was −8.11% per year (95%CI −9.38; −6.82), while for Hepatitis B it was −4.13% (95%CI −6.03; −2.20), of Other viral hepatitis of −7.84% (95%CI −14.11; −1.11) and of Unspecified Hepatitis −5.67% per year (95%CI −6.22; −5.10). Mortality due to chronic viral hepatitis increased by 5.74% (95%CI 3.47; 8.06) in the North and 4.95% in the Northeast (95%CI 0.27; 9.85). The Moran Index (I) for Hepatitis A was 0.470 (p<0.001), for Hepatitis B 0.846 (p<0.001), Chronic viral hepatitis=0.666 (p<0.001), other viral hepatitis=0.713 (p<0.001), and Unspecified Hepatitis=0.712 (p<0.001). Conclusion: The temporal trend of hepatitis A, B, other viral, and unspecified hepatitis was decreasing in Brazil, while mortality from chronic hepatitis was increasing in the North and Northeast.

  • Epidemiology of COVID-19 in the State of Sergipe/Brazil and Its Relationship with Social Indicators

    A pandemic is capable of generating a great impact, not only from the point of view of health, but also socioeconomically. In March 2020, the World Health Organization (WHO) declared that a new pandemic situation had arisen, due to the SARS-CoV-2 virus, whose probable origin was zoonotic. The largest number of cases of this disease is concentrated in the United States of America (USA), India, and Brazil. The mortality rate is estimated at 3.4%, but regional differences may exist, and places with a high demographic density have become true epicentres and may be related to higher rates of transmission. In addition to the above, lower human development indexes (HDI) can be related to worse outcomes, especially in the North and Northeast regions of Brazil since they are the least developed places. The Northeast region is the second-most-affected place in the number of COVID-19 cases in Brazil. An analytical observational study of an ecological type was carried out from April to October 2020 to assess the epidemiological situation of COVID-19 in the state of Sergipe and specifically to analyse the incidence of cases and deaths resulting from COVID-19 in the different health regions of the state of Sergipe, in relation to the values of the HDI and demographic density. During the study period, 84,325 cases of COVID-19 were identified, in which 2205 resulted in death. In most of the regions studied, there was a positive association between the number of cases and deaths and the greater the demographic density, but there was no increase in the risk of becoming ill, nor of dying the lower the HDI. Large and crowded cities are places of greatest vulnerability to illness, due to their greater capacity of transmitting the virus; however, further studies are needed to identify other factors that are decisive in the outcomes of this new disease.
  • Selenium concentrations in elderly people with Alzheimer’s disease: a cross-sectional study with control group

    Objective: To investigate possible differences in plasma and erythrocyte concentrations of selenium among elderly with and without a diagnosis of Alzheimer’s disease (AD). Methods: Cross-sectional study, performed with an elderly group with Alzheimer’s disease, diagnosed by a geriatric doctor, and compared to an elderly group without the disease, equaling gender, education, and age. Atomic absorption spectrophotometry determined plasma and erythrocyte concentrations of total selenium (Set). Results: The mean age was 74.41±7.1 years in the Alzheimer’s disease group and 71.46±5.1 years among the control group. The Alzheimer’s disease group presented lower plasma concentrations (mean of 45.29±14.51 µg/dL vs. 55.14±14.01 µg/dL; p=0.004), and erythrocyte Set (median of 56.36 µg/L vs. 76.96 µg/L; p<0.001). The logistic regression model indicated an association between erythrocyte Set concentrations and diagnosis of Alzheimer’s disease (p=0.028). Conclusion: Elderly with Alzheimer’s disease present lower selenium concentrations in the evaluated organic compartments.
