Physical Activity and Survival After an Acute Coronary Syndrome: Data From the ERICO Study Carlos Alberto Treff, Gisela Tunes da Silva, Alessandra Carvalho Goulart, Alexandre Costa Pereira, Paulo Andrade Lotufo, Isabela Martins Bensenor, Itamar de Souza Santos Journal of Physical Activity and Health, 2025 Background: Due to enhancements in health care, the number of individuals who survive an acute coronary syndrome (ACS) event is rising. In this population, the impact of physical activity on long-term survival is still a matter of debate. We aimed to analyze the association between the level of physical activity during the first 4 years of follow-up and survival. Methods: We analyzed data from 692 patients admitted due to an ACS event in a community hospital in Brazil. Physical activity was assessed using the long version of the International Physical Activity Questionnaire at 30 days, 180 days, and yearly after the first event. Mortality data were obtained using phone contacts, medical records, and official death documents. We built time-dependent Cox proportional hazard models to analyze whether physical activity levels during follow-up were associated with survival. Results: Our main sample had 280 (40.5%) women and a mean age of 62.6 (12.5) years. In all assessments, 50.8% to 74.9% of study participants had physical activity levels below recommendations for healthy individuals. Physical inactivity increased the risk of death (hazard ratio [HR], 1.82; 95% confidence interval, 1.13–2.92). There was no statistically significant survival difference between insufficiently active and active participants. Conclusions: Many individuals who survive an ACS event have poor physical activity levels. Physical inactivity after an ACS event significantly increases the risk of death. Our findings provide additional support to reinforce, in clinical practice, the benefits of physical activity in individuals who survive an ACS event.
The utility of an artificial intelligence model based on decision tree and evolution algorithm to evaluate steatotic liver disease in a primary care setting A.C. Goulart, A.P. Alencar, G. Rienzo Brazilian Journal of Medical and Biological Research, 2025 Many ways of classifying steatotic liver disease (SLD) with metabolic conditions have been proposed. Thus, SLD-related variables were verified using a decision tree. We tested if the suggested components of the actual classification (metabolic dysfunction-associated steatotic liver disease, MASLD) are also present in young and middle-aged adults. In a cross-sectional study involving 6,839 adults (median age: 46 years, 69.5% men) in a primary care setting, a decision tree was created to determine potential clinical and laboratory variables related to SLD. The odds ratio (OR) with a respective 95% confidence interval (95%CI) was calculated for both sexes. SLD frequency was 26.6% (23% in men). More variables and with higher ORs for the association with SLD were identified in women: category 1 (body mass index (BMI) ≥29 kg/m2, age <51 years, high-sensitivity C-reactive protein (hs-CRP) ≥0.195 mg/dL): OR=10.9, 95%CI: 4.40-26.6; category 2 (BMI <9 kg/m2, metabolic syndrome (MS), age ≥50 years, neck circumference (NC) ≥36 cm): OR=8.1, 95%CI: 2.2-29.9; and category 3 (BMI ≥29 kg/m2, age <51 y-old, dyslipidemia, high-density lipoprotein cholesterol (HDL-c) <42 mg/dL): OR=4.7, 95%CI: 2.20-10.7. For men: category 1 (waist circumference (WC) ≥101 cm, alanine aminotransferase (ALT) <28 mg/dL, glycated hemoglobin (HbA1c) ≥5.7%): OR=4.7, 95%CI: 2.8-7.9; and category 2 (WC ≥101 cm, ALT ≥28 mg/dL): OR=3.2, 95%CI: 2.5-4.0). The decision tree identified more variables related to SLD, particularly in women, such as age of more than 50 years, elevated hs-CRP, and NC≥36 cm than variables related to MASLD.
Bayesian mixed model for survival data with semicompeting risks based on the Clayton copula Elizabeth González Patiño, Gisela Tunes, Nelson Ithiro Tanaka Brazilian Journal of Probability and Statistics, 2024 Motivated by a chronic kidney disease dataset, we propose a Bayesian model for clustered semicompeting risks data based on Archimedean copulas, allowing for treatment switching. We consider the modeling of both independent and clustered observations. For clustered data, random effects are included to consider the dependence among observations in the same group. For the Clayton copula, we provide theoretical results for the posterior distribution when improper priors are used. A simulation study was conducted to evaluate the performance of the proposed model. Finally, the results of the analysis of chronic kidney disease data are discussed.
Long-term survival, quality of life, and quality-adjusted survival in critically ill patients with cancer Karina Normilio-Silva, Adelaide Cristina de Figueiredo, Antonio Carlos Pedroso-de-Lima, Gisela Tunes-da-Silva, Adriana Nunes da Silva, Andresa Delgado Dias Levites, Ana Tereza de-Simone, Patrícia Lopes Safra, Roberta Zancani, Paula Camilla Tonini, Ulysses Vasconcelos de Andrade e Silva, Thiago Buosi Silva, Juliana Martins Giorgi, José Eluf-Neto, Anderson Costa, Ludhmila Abrahão Hajjar, Alexandre Biasi Cavalcanti Critical Care Medicine, 2016 Objectives: To assess the long-term survival, health-related quality of life, and quality-adjusted life years of cancer patients admitted to ICUs. Design: Prospective cohort. Setting: Two cancer specialized ICUs in Brazil. Patients: A total of 792 participants. Interventions: None. Measurements and Main Results: The health-related quality of life before ICU admission; at 15 days; and at 3, 6, 12, and 18 months was assessed with the EQ-5D-3L. In addition, the vital status was assessed at 24 months. The mean age of the subjects was 61.6 ± 14.3 years, 42.5% were female subjects and half were admitted after elective surgery. The mean Simplified Acute Physiology Score 3 was 47.4 ± 15.6. Survival at 12 and 18 months was 42.4% and 38.1%, respectively. The mean EQ-5D-3L utility measure before admission to the ICU was 0.47 ± 0.43, at 15 days it was 0.41 ± 0.44, at 90 days 0.56 ± 0.42, at 6 months 0.60 ± 0.41, at 12 months 0.67 ± 0.35, and at 18 months 0.67 ± 0.35. The probabilities for attaining 12 and 18 months of quality-adjusted survival were 30.1% and 19.1%, respectively. There were statistically significant differences in survival time and quality-adjusted life years according to all assessed baseline characteristics (ICU admission after elective surgery, emergency surgery, or medical admission; Simplified Acute Physiology Score 3; cancer extension; cancer status; previous surgery; previous chemotherapy; previous radiotherapy; performance status; and previous health-related quality of life). Only the previous health-related quality of life and performance status were associated with the health-related quality of life during the 18-month follow-up. Conclusions: Long-term survival, health-related quality of life, and quality-adjusted life year expectancy of cancer patients admitted to the ICU are limited. Nevertheless, these clinical outcomes exhibit wide variability among patients and are associated with simple characteristics present at the time of ICU admission, which may help healthcare professionals estimate patients’ prognoses.

Nevertheless, there is no standard procedure to predict anemia development after blood donation. The São Paulo Blood Center is responsible for maintaining a database with information on each donation. However, this database doesn't have a good quality and consequently it is difficult to establish systematic analysis using the donation database without previous data validations. In order to provide a better quality of donation data, this paper presents a provenance description based on a classification criteria defined by specialists. More concretely, this paper answers the follow main question: is there a connection between blood donation and decrease in hematocrit level in order to prevent undesirable outcomes to blood donors? In this paper we show that it is possible to provide detailed investigations in order to answer this main question using the data description without the need to impose changes in the current database system structures that is sponsored by São Paulo Blood Center.
