  • Investigating the prevalence of Toxoplasma gondii infection in Parkinson’s disease patients and its association with disease severity: a case-control study

    BACKGROUND: Toxoplasma gondii infects nearly one-third of the global population and has been implicated in neurologic dysfunction; however, its contribution to Parkinson's disease (PD) remains unresolved. AIM: Here we assess whether T. gondii seropositivity is associated with PD occurrence or severity in a well-defined group of patients and matched controls. METHODS: A case-control design included n = 60 PD patients and n = 60 matched controls, with anti-T. gondii IgG/IgM quantified by ELISA and PD severity graded using the Hoehn and Yahr scale. Logistic regression evaluated associations (P < 0.05). RESULTS: Among 120 participants (60 PD, 60 controls; mean age 69.7 ± 9.0 years; 66.7% male), education differed significantly (illiteracy: 58.3% vs. 30.0%, p < 0.001). Risk factors for T. gondii infection showed no significant differences: cat contact (61.7% vs. 56.7%; OR = 1.22, 95% CI: 0.57-2.62), unwashed vegetables (20.0% vs. 21.7%; OR = 0.90, 95% CI: 0.36-2.25), and undercooked meat (15.0% vs. 16.7%; OR = 0.88, 95% CI: 0.30-2.57). Overall IgG seroprevalence was 90.8% (PD 88.3%, controls 93.3%; adjusted OR = 0.56, 95% CI: 0.16-1.94, p = 0.36). IgG positivity did not vary significantly by PD severity (85.7-100% across Hoehn and Yahr stages), and only one PD patient was IgM-positive. CONCLUSIONS: The data establish that chronic T. gondii exposure does not differentiate PD patients from controls and does not convincingly modulate disease severity. This work provides a new framework for interpreting parasite-neurodegeneration links in high-exposure settings and opens the door to future studies using molecular assays and longitudinal designs to resolve causal pathways.
  • Nosocomial oral myiasis caused by Chrysomya bezziana in Iran: A new case and review of human myiasis in the country

    Chrysomya bezziana can potentially cause myiasis, a condition in which fly larvae infest living tissue in humans and animals. Contributing factors to this rare ailment include poor oral hygiene, alcohol abuse, and infected wounds. Hospital-acquired myiasis, a rare type of myiasis, occurs in patients following hospital admission. Herein, we report a case of C. bezziana oral myiasis in an 89-year-old female patient hospitalized in northeastern Iran. In this article, we also present an updated review of reported human myiasis cases in Iran up to 2025. According to our findings, C. bezziana has been responsible for 13 documented cases of myiasis in Iran. Most nosocomial myiasis cases in the country are related to pharyngeal and nasal infestations and are primarily observed in ICU patients.
  • Primary pelvic hydatidosis associated with ovarian attachment: A case report

    Background Hydatid cyst (HC) represents a remarkable zoonotic parasitic disease, mostly affecting the liver and lungs. In this report, we present a patient from northern Iran who exhibited HC involvement in the pelvic region, a rare occurrence that is often asymptomatic. Case Presentation A 41-year-old female presented with hypogastric pain, and imaging survey showed the potential presence of cystadenoma, endometriosis, and various typical ovarian lesions. She underwent laparotomy, during which pelvic HC was identified. Serological testing confirmed this finding. Also, albendazole was prescribed, and she was discharged in relatively good condition. Conclusion In regions where this infection is prevalent, HC must be considered in the differential diagnosis of ambiguous cystic tumors. This case highlights the importance of including HC in the differential diagnosis of pelvic cysts, especially in endemic areas. Furthermore, we recommend the using of molecular techniques during surgical interventions as potentially beneficial.

  • Coexistence of Tuberculosis and Lophomoniasis in a Patient With Alzheimer's Disease

    The coexistence of lophomoniasis and tuberculosis (TB), both airborne diseases, is relatively uncommon. Co‐infections like these can complicate treatment strategies due to overlapping symptoms and potential drug interactions. We report a rare case of comorbidity involving two pulmonary diseases, lophomoniasis and TB, in an 82‐year‐old woman with Alzheimer's disease (AD) from northern Iran. Her primary symptoms included weakness, lethargy, dyspnea, sputum production, night sweats, and significant weight loss. Both TB and lophomoniasis can compromise the immune system, potentially worsening the progression or severity of AD by increasing susceptibility to infections or enhancing neuroinflammation. Following the prescription of appropriate drug regimens for both diseases, the patient was discharged from the hospital in stable condition. Overall, it is crucial to consider lophomoniasis in the differential diagnosis of patients with pulmonary tuberculosis, especially in endemic areas where both infections are prevalent, to ensure timely diagnosis and effective management.
  • Persistent trichomoniasis in a man in his early 90s with a history of prostatic hyperplasia: A case report

    The protozoan Trichomonas vaginalis is a parasite associated with numerous sexually transmitted infections worldwide. In males, although Trichomonas vaginalis infection (also known as trichomoniasis) often remains asymptomatic, it can lead to conditions such as prostatitis or urethritis. Herein, we report the case of a man in his early 90s who was admitted to a hospital in northern Iran with mild dysuria, whose precise onset remained unclear. He had a history of chronic prostate discomfort for several years and acknowledged having multiple sexual partners over the past two decades. He complained of dysuria; therefore, urinalysis was performed, which detected trichomoniasis. He received treatment with metronidazole, resulting in the resolution of urinary symptoms and elimination of the parasite. This case highlights the importance of considering the possibility of trichomoniasis in men, especially older men, as they may face an increased risk of inflammation and prostate cancer.
  • Pulmonary toxoplasmosis in a multiple myeloma patient: A Case report and literature review

    infection is typically mild in immunocompetent hosts, it can cause severe disease in immunocompromised populations. A literature review was conducted using PubMed, Scopus, and Google Scholar (January 2000-May 2025) with keywords: 'pulmonary toxoplasmosis,' 'multiple myeloma,' 'non-HIV,' 'immunocompromised,' and 'BAL PCR.' Only English-language case reports and reviews were included, with emphasis on studies from endemic regions and the past five years. We report a 49-year-old Iranian man with newly diagnosed IgG kappa MM who presented with fever, chills, vomiting, and dyspnea. Chest CT revealed a 4.2 × 3.8 cm consolidation in the posterior segment of the right lower lobe. T. gondii infection was confirmed via bronchoalveolar lavage (BAL) through tachyzoite detection on Giemsa stain and molecular identification using qPCR and nested PCR targeting the Gra6 gene. He showed transient clinical improvement and was discharged on hospital day 7. Unfortunately, he died at home within 48 hours due to MM progression, before anti-toxoplasmosis therapy was initiated. This case underscores the diagnostic difficulty of pulmonary toxoplasmosis in non-HIV immunocompromised patients and the importance of molecular BAL testing. Prophylactic measures such as co-trimoxazole should be considered in high-risk MM patients. Guidelines must address screening and management in this vulnerable population.
  • Multi-venous thrombosis associated with a perforated hydatid cyst: A rare case report and comprehensive literature review

This case report details the diagnosis and management of a patient with a hepatic hydatid cyst complicated by extensive multi-venous thrombosis. Case presentation: A 61-year-old male stockbreeder presented with abdominal pain localized in the hypogastric region, exacerbated by food intake. Imaging revealed a hepatic HCD and thrombosis of the portal, superior mesenteric, and splenic veins. The patient was successfully treated with albendazole for the cyst and anticoagulant therapy for the thrombosis, with no recurrence of symptoms during a three-year follow-up period. Conclusion: While multi-venous thrombosis associated with HCD is rare, documented cases highlight the need for increased awareness among physicians to effectively recognize and manage this complication.
  • Spatial Modeling and Risk Mapping of Human Fascioliasis in Iran: A Geographical Information System-Based Survey from 2013-2022

    Background: Human fascioliasis, a growing health concern, has increased significantly over the last few decades and serves as a notable example of emerging and re-emerging disease foci in numerous countries. Objectives: This study aimed to evaluate the incidence, spatial distribution, and hotspot regions of Fasciola infection in Iran using geographic information system (GIS) analyses from 2013 to 2022. Methods: Data on Fasciola cases and populations at risk across various provinces were obtained from the Ministry of Health and Medical Education, Tehran, Iran, and other relevant organizations for the years 2013 to 2022. A map illustrating the geographical distribution of fascioliasis was generated. Spatial analyses were conducted using ArcGIS 10.5 software to identify hotspot regions of fascioliasis in Iran. The correlation between temperature, relative humidity, the normalized difference vegetation index (NDVI), and the incidence of fascioliasis (variables influencing the disease) was assessed using geographically weighted regression (GWR) in ArcGIS 10.5. Data analysis was performed using linear regression and SPSS version 21 software. Results: Hotspot provinces for fascioliasis were identified in Gilan, Kermanshah, and Khorramabad. Geographically weighted regression analysis revealed a high correlation between humidity, temperature, vegetation density, and the incidence of Fasciola infection in the provinces of Gilan, Kermanshah, Khorramabad, Kurdistan, Semnan, and South Khorasan (P-value = 0.025). Conclusions: This study demonstrated a significant relationship between relative humidity, mean annual temperature, NDVI, and the incidence of Fasciola infection in Iran. Geographic information system analysis proved to be an effective tool for identifying risk factors and assessing endemic areas of fascioliasis within specific regions.
