The Gesture Homunculus: A Body-Cortical Topography of User-Defined Gestures Jean Vanderdonckt, Radu-Daniel Vatavu Conference on Human Factors in Computing Systems Proceedings, 2026 Gesture research for interactive computer systems has primarily relied on taxonomies using external dimensions of gesture articulation. While effective for documenting gestures and supporting computational recognition, this focus provides an incomplete account by overlooking internal dimensions related to how the body parts involved in gesture articulation are represented in the brain. To address this aspect, we introduce the Gesture Homunculus, a body-cortical topographical representation grounded in the medical literature, reinterpreted through the lens of user-defined gestures. To construct it, we analyzed 2,294 elicited gestures and 3,637 corresponding body parts, which we relate to somatosensory and motor cortical mappings. Based on this representation, we identify future work opportunities for gesture elicitation studies, such as exploration of underrepresented body parts, combination of multiple body parts within a single gesture, and designing gesture interactions that span both the somatosensory and motor cortices.
The Impact of Graviception on Human-Computer Interaction Jean Vanderdonckt, Philippe Lefevre, Radu-Daniel Vatavu, Nuwan T Attygalle, Laura-Bianca Bilius, Gaëlle Calvary, Sophie Dupuy-Chessa, Merle Fairhurst, Tommy Nilsson, Leif Oppermann, Paolo Roselli, Giovanni Saggio, Valérie Swaen Conference on Human Factors in Computing Systems Proceedings, 2026 Graviception, the human sensory perception of gravity and body orientation, plays a fundamental, yet often overlooked, role in how individuals interact with interactive systems. Human–computer interaction has traditionally emphasized visual, auditory, and tactile modalities, while comparatively neglecting the influence of gravity perception on embodiment, attention, motor control, and user experience. This paper draws on the field of biomechanics to examine how graviception can impact interaction, such as the impact of vision and gravity on body movements, especially in immersive, hypogravity environments, mobile contexts of use, and accessibility. By positioning graviception as a first-class concern in embodied interaction, we suggest actionable implications for interaction design that consider graviceptive factors for designers and researchers developing future graviceptive-aware interactive systems. Each implication is described by a short statement, a detailed statement, a rationale, and a series of examples illustrating the impact of graviception on design decisions.
User experience with adaptive user interfaces: Comparing performance and preferences Daniel Gaspar-Figueiredo, Jean Vanderdonckt, Silvia Abrahão, Emilio Insfran Journal of Systems and Software, 2026 Adaptive user interfaces dynamically change their content, presentation, and behavior to optimize the user experience, which has been primarily evaluated using classic usability measures but to a lesser extent by using neurological measures. While the perceived preference of specific user interface elements, such as graphical adaptive menus, has already been studied, no consensus exists regarding their performance and how to substitute a static menu with an adaptive one. To gain insights into how graphical adaptive menus could influence the user experience and to identify any correlation between users’ performance and their preferences, we conducted an experiment in which forty participants used twenty graphical adaptive menus while their brain activity was captured by employing electroencephalography to derive four measures ( i.e. , cognitive load, engagement, attraction, and memorization). User performance was measured using task completion time, specifically the time to select menu items. Statistical analysis suggested which graphical adaptive menus were significantly better or worse than the static menu, our baseline. These results are used as the basis to suggest implications for software developers and researchers to design more effective adaptive user interfaces.
Invisible Risks in Virtual Reality: Raising Awareness of Profiling and Overengagement through Lecture and Storyboard Interventions Suzanne Kieffer, Damien Renard, Romilla Syed, Michel Noutcha, Eloïse Yang, Jean Vanderdonckt Proceedings 2026 IEEE Conference on Virtual Reality and 3D User Interfaces VR 2026, 2026 This paper examines how awareness interventions relate to emotional responses to invisible risks in immersive systems, with an empirical focus on virtual reality. Across two complementary studies, we examine emotional responses elicited by a lecture and by narrative design-fiction storyboards focusing on biometric profiling and overengagement—two risks that undermine user agency and autonomy. The interventions elicit distinct emotional patterns, pointing to different ways in which invisible risks become salient across cognitive, affective, and moral dimensions. Qualitative responses indicate recognition of manipulation, loss of agency, and ethical concern. The results suggest that concise narrative scenarios can render otherwise imperceptible risks concrete and relatable.
Implementation of large language models in electronic health records Maxime Griot, Jean Vanderdonckt, Demet Yuksel Plos Digital Health, 2025 Electronic Health Records (EHRs) have improved access to patient information but substantially increased clinicians’ documentation workload. Large Language Models (LLMs) offer a potential means to reduce this burden, yet real-world deployments in live hospital systems remain limited. We implemented a secure, GDPR-compliant, on-premises LLM assistant integrated into the Epic EHR at a European university hospital. The system uses Qwen3-235B with Retrieval Augmented Generation to deliver context-aware answers drawing on structured patient data, internal and regional clinical documents, and medical literature. A one-month pilot with 28 physicians across nine specialties demonstrated high engagement, with 64% of participants using the assistant daily and generating 482 multi-turn conversations. The most common tasks were summarization, information retrieval, and note drafting, which together accounted for over 70% of interactions. Following the pilot, the system was deployed hospital-wide and adopted by 1,028 users who generated 14,910 conversations over five months, with more than half of clinicians using it at least weekly. Usage remained concentrated on information access and documentation support, indicating stable incorporation into everyday clinical workflows. Feedback volume decreased compared with the pilot, suggesting that routine use diminishes voluntary reporting and underscoring the need for complementary automated monitoring strategies. These findings demonstrate that large-scale integration of LLMs into clinical environments is technically feasible and can achieve sustained use when embedded directly within EHR workflows and governed by strong privacy safeguards. The observed patterns of engagement show that such systems can deliver consistent value in information retrieval and documentation, providing a replicable model for responsible clinical AI deployment.
Large Language Models lack essential metacognition for reliable medical reasoning Maxime Griot, Coralie Hemptinne, Jean Vanderdonckt, Demet Yuksel Nature Communications, 2025 Large Language Models have demonstrated expert-level accuracy on medical board examinations, suggesting potential for clinical decision support systems. However, their metacognitive abilities, crucial for medical decision-making, remain largely unexplored. To address this gap, we developed MetaMedQA, a benchmark incorporating confidence scores and metacognitive tasks into multiple-choice medical questions. We evaluated twelve models on dimensions including confidence-based accuracy, missing answer recall, and unknown recall. Despite high accuracy on multiple-choice questions, our study revealed significant metacognitive deficiencies across all tested models. Models consistently failed to recognize their knowledge limitations and provided confident answers even when correct options were absent. In this work, we show that current models exhibit a critical disconnect between perceived and actual capabilities in medical reasoning, posing significant risks in clinical settings. Our findings emphasize the need for more robust evaluation frameworks that incorporate metacognitive abilities, essential for developing reliable Large Language Model enhanced clinical decision support systems. Large Language Models demonstrate expert-level accuracy in medical exams, supporting their potential inclusion in healthcare settings. Here, authors reveal that their metacognitive abilities are underexplored, showing significant gaps in recognizing knowledge limitations, difficulties in modulating their confidence, and challenges in identifying when a problem cannot be answered due to insufficient information.
Context is Key for Reproducibility of Empirical Studies in Human-Computer Interaction Jean Vanderdonckt, Radu-Daniel Vatavu Proceedings of the 3rd ACM Conference on Reproducibility and Replicability ACM Rep 2025, 2025 Although most reproducibility frameworks are generic enough to apply across a wide range of scientific domains, they may fail to capture the specific dimensions relevant to particular fields, such as Human-Computer Interaction, where interactive prototypes must be evaluated in the context of use involving users, devices, and environments. To address this aspect, we introduce Ampliatum, a contextual reproducibility framework that extends general replication types to cover interactive systems with (1) replication along various contextual dimensions, (2) extension and generalization of previous studies using different populations, and (3) repurposing studies with goals different from those of the original research. To demonstrate the applicability and utility of our framework, we report case studies in gesture-based input. In our first example, we classify existing gesture elicitation studies using our revised terminology. In the second case, we replicate a previous user study under different contextual conditions. Finally, we propose recommendations for the implementation of replicability in Human-Computer Interaction.
Context-Aware Adaptive Visualizations for Critical Decision Making Angela Lopez-Cardona, Mireia Masias Bruns, Nuwan T. Leiva Frontiers in Artificial Intelligence and Applications, 2025 Effective decision-making often relies on timely insights from complex visual data. While Information Visualization (InfoVis) dashboards can support this process, they rarely adapt to users’ cognitive state, and less so in real time. We present SYMBIOTIK, an intelligent, context-aware adaptive visualization system that leverages neurophysiological signals to estimate mental workload (MWL) and dynamically adapt visual dashboards using reinforcement learning (RL). Through a user study with 120 participants and three visualization types, we demonstrate that our approach improves task performance and engagement. SYMBIOTIK offers a scalable, real-time adaptation architecture, and a validated methodology for neuroadaptive user interfaces.
What is Extreme in Human-Computer Interaction Research? Radu-Daniel Vatavu, Jean Vanderdonckt Mobilehci 2025 Adjunct Proceedings of the 2025 Conference on Mobile Human Computer Interaction, 2025 Researchers in human-computer interaction have often approached interactive system design and engineering involving atypical users, novel technological advancements, and unconventional environments that deviate from the average, moderate, or standardized. This paper examines the extent to which researchers characterize their work as extreme, through a targeted literature review and an analysis of the linguistic connotations of the term. We emphasize extreme users, who display exaggerated behaviors, acquire extraordinary abilities, and engage in high-stakes, high-performing collaborations. We also highlight extreme platforms that facilitate input and output modalities surpassing conventional limits, foster intense synergy between users and machines, and employ boundary-pushing design approaches. Lastly, we discuss extreme environments, whether physical, digital, virtual, or mixed, which pose very serious, severe and potentially risky conditions. We conclude with the benefits for the HCI field of engaging in more scientific and technical explorations driven by “extreme” interaction elements.
