New-Onset Palmar Psoriasis Following mRNA COVID-19 Vaccination: A Case Report Haya Alzamami, Omer AlSulaiman, Gaeda Alkaltham, Hadeel Alotaibi International Medical Case Reports Journal, 2025 Palmoplantar psoriasis is a rare, chronic, localized variant of psoriasis. It represents 3-4% of all psoriasis cases and is characterized by hyperkeratotic or desquamative erythematous plaques with fissures and erosions exhibiting acral distribution. Occasionally, superimposed sterile pustulations may also occur. New-onset psoriasis and various clinical subtypes of psoriasis exacerbation have been reported following the administration of messenger ribonucleic acid (mRNA)-based coronavirus disease 2019 (COVID-19) vaccines, with plaque-type psoriasis being the most common. However, palmar psoriasis has not yet been reported in association with COVID-19 vaccination. Herein, we present a case of new-onset palmar psoriasis along with nail changes following mRNA COVID-19 vaccination, which was further exacerbated following successive vaccine doses.
Herpes Zoster of the Sacral Region without Motor Dysfunction in a 55-year-Old Female: A Case Report and Literature Review Omer Alsulaiman, Ahmed Alsaati, Feras Alsulaiman, Danah Aljaafari, Majed Alabdali International Medical Case Reports Journal, 2025 Herpes zoster infection, commonly known as shingles, typically involves the thoracic and lumbar dermatomes. Sacral involvement is rare, occurring in only 4-8% of cases. This report presents a 55-year-old female with herpes zoster affecting the S1 dermatome, manifesting as burning pain and clustered vesicles on her right leg, without motor dysfunction or urinary symptoms. A clinical diagnosis was made based on a characteristic unilateral S1 dermatomal vesicular eruption. Despite the rarity of sacral herpes zoster without motor involvement, the patient responded well to antiviral treatment and neuropathic pain management, with no complications noted in the long-term follow-up. This case highlights the necessity for clinicians to consider sacral herpes zoster in patients with radicular pain to prevent misdiagnosis.
Balanitis Circumscripta Plasmacellularis in a Young Circumcised Male: A Case Report Omer Alsulaiman, Ahmed Alsaati, Nasser Alsaif, Khalid Al-Husain, Fatma Al-Jindan, Bader Algamdi International Medical Case Reports Journal, 2025 A 22-year-old incarcerated Saudi man presented with a 2-day history of painful vesicles on the shaft of his circumcised penis that were unrelated to drug use, trauma, exposure to ill persons, or sexual intercourse. Initial laboratory tests were normal, and the patient was initially diagnosed with herpes simplex infection. Due to nonadherence to the initial antiviral treatment, the lesions progressed, leading to a revised diagnosis of chancroid. Subsequent testing ruled out sexually transmitted infections and revealed methicillin-resistant Staphylococcus aureus in the culture of the purulent exudate. Histopathology confirmed balanitis circumscripta plasmacellularis (BCP, zoon's balanitis). The lesion responded well to topical betamethasone, with marked clinical improvement over 2 months. This case highlights that although rare, BCP should be considered in circumcised patients with non-venereal genital lesions. Additionally, it underscores that early histopathological confirmation is essential to avoid misdiagnosis and inappropriate treatment.
