  • Comparative efficacy and safety of ferrous sulfate versus ferric carboxymaltose in different forms of anemia: A systematic review highlighting data discrepancies

    Introduction . This article discusses modern therapeutic problems and previously published data on iron deficiency correction using ferrous sulfate (FS) and ferric carboxymaltose (FCM). The data obtained are analyzed and compared based on the age of the study, the clinical entity of anemia, sample size, types of research and dosages of the drugs used. Aim . To conduct a systematic comparative analysis of the efficacy and safety of FS versus FCM for the correction of iron deficiency in various nosological forms of anemia. Materials and methods . A systematic literature search was performed in the PubMed database for the period from January 1, 2008, to February 1, 2026. Inclusion criteria were direct comparative studies or meta-analyses comparing FCM with FS in adult patients with anemia. Primary outcomes were efficacy (increase in hemoglobin level) and safety (incidence of adverse events). Results . Fourteen publications were included in the analysis. A key data discrepancy was identified: all four meta-analyses indicated a statistically significant superiority of FS over FCM in both efficacy (e.g., greater Hb increase by 4.9–8.5 g/L; p < 0.05) and safety profile. However, in three direct comparative randomized controlled trials meeting the criteria, the results for efficacy and tolerability were comparable. The vast majority of studies (19 mentions) were focused not on classic iron deficiency anemia (IDA), but on anemia of chronic disease (primarily in pregnant women), while true IDA was represented in only two works. Conclusion. The existing evidence demonstrates a significant discrepancy between the results of meta-analyses and direct comparisons of FS and FCM. The available data do not allow for a definitive conclusion about the superiority of one drug over the other, especially for the population with classic IDA, and highlight the need for targeted comparative studies in this area.

  • Correction of anemia using an iron supplement with a hepcidin-independent absorption mechanism in patients with chronic heart failure with preserved left ventricular ejection fraction

    Aim. To characterize anaemia in chronic heart failure (CHF) with preserved left ventricular ejection fraction (CHFpEF) and evaluate the effectiveness of an iron supplement with a hepcidin- independent absorption mechanism. Material and methods. An uncontrolled study included 30 patients with CHFpEF NYHA class I-III and anaemia. Patients received "standard therapy" for CHF in combination with sucrosomial iron (60 mg/day orally) for three months. Treatment response was assessed via clinical blood test, iron metabolism parameters, inflammatory markers, and functional tests at baseline and three months post-treatment. Results. Anaemia in CHFpEF corresponds to anaemia of chronic disease with elevated hepcidin content, 219 (149-553) ng/mL occurs with impaired absorption and metabolism of iron and the development of iron deficiency. Hemoglobin levelincreased from 115 (98-117) g/L to 120 (103-133) g/L, p = 0.01, red blood cell count increased from 3.6 (3.5-4.1) × 1012/L to 4 (3.7-5.3) × 1012/L, p = 0.05, and serum ferritin increased from 106 (40-181) μg/L to 117 (83-166) μg/L, p = 0.04. N-terminal natriuretic propeptide (NT-proBNP) level decreased from 374 (330-443) ng/mL to 236 (128-349) ng/mL, p = 0.004. After treatment, exercise tolerance exercise tolerance improved: 6-minute walk test distance increased from 343.1 ± 100 m to 397±73 m (p = 0.01). Quality of life (QoL) level increased and was determined by the clinical status assessment scale (CAS), p = 0.01 and by the Minnesota Health Failure Questionnaire (MHFLQ), p = 0.002. Conclusion. Adding oral hepcidin- independent iron to standard HFpEF therapy in patients with CHFpEF and anaemia significantly increased hemoglobin and serum ferritin levels, exercise tolerance, and QoL scores while reducing NT-proBNP levels and NYHA functional class after three months.

    Accurate assessment of the status of regional lymph nodes in cervical cancer is difficult due to the limitations of imaging methods of clinical staging. Indices of the response may reflect the extent of the tumor. Objective. To determine the most significant systemic inflammatory indices for the preoperative assessment of the regional lymph nodes status in patients with cervical cancer, to evaluate their predictive significance. Material and methods. The study included eighteen retrospective cohort studies involving 5.000 patients with cervical cancer, which assessed the relationship between systemic inflammatory indices and the lymph node status confirmed by the results of surgical staging. The data was searched in PubMed eLibrary, Google Scholar, Web of Science and Research Gate until March 2024. The meta-analysis was performed using random effects models to calculate the odds ratio (OR) with 95% confidence intervals. Results. The meta-analysis showed that systemic inflammatory indices can be used to improve the preoperative diagnosis of the risk of regional metastasis in patients with cervical cancer. The most significant indices are the ratio of neutrophils to lymphocytes and platelets to lymphocytes (NLR and PLR). High values of NLR (>2.78) and PLR (>149.3) are associated with the presence of regional metastases (NLR: OR=1.81 [1.50—2.19], I2=6%; PLR: OR=1.80 [1.46—2.21], I2=0%, p<0.001). At thresholds above these values, minimal heterogeneity and a more pronounced effect were observed. Conclusion. NLR and PLR are promising preoperative markers for assessing the risk of regional metastasis in cervical cancer. They can be used as an adjunct to standard clinical staging of cervical cancer.
  • Development of a method for differential diagnosis of iron deficiency anemia and anemia of chronic disease based on demographic data and routine laboratory tests using machine learning technologies

    Background. The study of machine learning methods, a branch of artificial intelligence science, is relevant for the development of optimal screening strategies, identification of risk groups, and application of less expensive and more accessible laboratory tests to assess the body iron status. Aim. To select an appropriate artificial intelligence algorithm for predicting serum ferritin (SF) levels and to evaluate its applicability for differential diagnosis of iron deficiency anemia and anemia of chronic diseases. Materials and methods. A dataset of 9771 patients with micro‑normocytic anemia was used to create the model. On the basis of demographic data (gender and age), clinical blood count, C‑reactive protein level and known SF level, a regression model was developed to calculate the expected SF concentration in a particular patient and, using the same parameters, a classification model to determine the SF level group to which the patient belongs: I – < 15 μg / L; II – 15–100 μg / L; III – 100–300 μg / L; Iv – ≥ 300 μg / L. Results. As a result, the regression model has moderate predictive ability (R2 = 0.70; median absolute error was 10.7 μg / L), the correlation coefficient between known and predicted SF level was r = 0.854 (p < 0.05). The obtained classification model has high diagnostic accuracy for different clinical groups according to the SF level (AuC ROC was 0.91; 0.79; 0.84; 0.90 and 0.96; 0.76; 0.71; 0.82 for patients with reduced hemoglobin levels in women (< 120 g / L) and men (< 130 g / L) in groups I, II, III, Iv, respectively). Conclusion. Prediction of SF level using the developed models can be used as an accurate and clinically relevant tool for differential diagnosis of iron deficiency anemia (predicted SF is decreased (< 100 μg / L), C‑reactive protein is normal) and anemia of chronic diseases (predicted SF is normal or increased (>100 μg / L), C‑reactive protein is increased) in real medical practice.
  • Features of the systemic influence of cervical cancer on peripheral blood and hemostasis according to the tumor histological subtype

    Aim. To identify the features of the systemic effect on hematopoiesis and the blood coagulation system of different histological subtypes of cervical cancer (CC).Materials and methods. A single-center retrospective cohort study of 428 patients (61 patients with high grade squamous intraepithelial lesion (carcinoma in situ) and 367 patients with CC) examined from 1997 to 2022 in the Polyclinic of P. Hertzen Moscow Oncology Research Institute – branch of the National Medical Research Radiology Center, Ministry of Health of Russia. The age of patients was from 18 to 90 years (median age – 42 [34; 51] years). Patients were divided into 4 groups: group 1 – patients with high grade squamous intraepithelial lesion and 3 groups depending on tumor’s histological subtype (squamous cell, glandular squamous cell, adenocarcinoma). Demographic and clinical data were analyzed including laboratory tests (general blood count, biochemical blood test and iron metabolism and coagulograms (total 32 variables)) compared to clinical and surgical stages of CC.Results. High grade squamous intraepithelial lesion does not have a systemic effect on the parameters of general, biochemical blood tests and hemostasis (p >0.05). On the other hand, statistically significant changes were found in the parameters of routine laboratory blood tests, which correlate with the results of microinvasive analysis of CC, thus revealing the systemic effect of a malignant tumor: at p ≤0.001 for thrombin time, total protein, alkaline phosphatase levels; at p ≤0.05 for alanine aminotransferase, aspartate aminotransferase, C-reactive protein, serum iron and urea levels. Among patients with CC, statistically significant (p<0.001) correlations were obtained between hematological, hemostasiological and biochemical blood parameters. Of statistical significance, the presence of the glandular component in the tumor increases the systemic effect of CC on blood parameters, hemostasis, markers of inflammation and iron metabolism: leukocytosis and maximum erythrocyte sedimentation rate values compared to CC, high fibrinogen and the level of soluble fibrin-monomer complexes which increased activation of coagulation hemostasis along the common coagulation pathway (thrombin time) (p<0.001). It leads to an increase in the risk of venous thromboembolic complications in this category of patients.Conclusion. The identified changes in blood parameters made it possible to reveal the unique biological characteristics of each histological subtype of CC, including biochemical ones, which will help in developing an improved diagnosis of CC depending on the histological subtype and stage of the disease, along with developing preventive measures and treatment methods adapted to each specific case.
  • Characteristics of anemia in heart failure: iron metabolism, erythropoietic activity and markers of inflammation

    Aim. To determine the characteristics of erythrocyte parameters, iron metabolism, erythropoiesis, inflammation markers in patients with heart failure (CHF) and anemia.Material and methods. The study included 68 patients with HF to describe the characteristics of anemia (general group). Patients of the general group were divided into 3 subgroups depending on the serum ferritin (SF) and transferrin saturation (TS).Results. Anemia in HF is not always associated with iron deficiency (ID), characterized by inadequate production of erythropoietin, increased levels of C-reactive protein and hepcidin. From the general group of patients, the following were distinguished: absolute ID (43%), functional ID (19%), and non-ID anemia (38%).Conclusion. In the general group of patients with HF, anemia is normochromic, normocytic, characterized by high levels of C-reactive protein, hepcidin, and inadequate production of erythropoietin. Patients with absolute and functional ID do not differ in the main iron metabolism parameters and in general account for 62% (p=0,01). In these two subgroups, iron supplements should be prescribed at further stages.

    This review considers the structure and regulation of endothelial activity, its role in angiogenesis and homeostasis maintenance. A special attention is paid to the influence of the vascular wall on the blood coagulation system and to the anti-thrombogenic properties of the endothelium. Different variants of endothelial dysfunction are presented, which can be severe and accompanied by high pro-thrombogenic activity, thrombosis, ischemia and fibrosis of organs and tissues or, conversely, have a slower course, in which the vascular intima involved in the process reduces its anti-thrombogenic function. The influence of these changes on homeostasis and the course of various diseases, including in obstetrics and gynecology, as well as the possibilities of modern diagnostics of endothelial dysfunction are discussed.
