  • Pattern of maxillofacial fractures in severe multiple trauma patients: A 7-year prospective study

    The incidence of facial trauma is high. This study has the primary objective of documenting and cataloging maxillofacial fractures in polytrauma patients. From a total of 1229 multiple trauma cases treated at the Emergency Room of the Santo Antonio Hospital - Oporto Hospital Center, Portugal, between August 2001 and December 2007, 251 patients had facial wounds and 209 had maxillofacial fractures. Aged ranged form 13 to 86 years. The applied selective method was based on the presence of facial wound with Abbreviated Injury Scale ≥1. Men had a higher incidence of maxillofacial fractures among multiple trauma patients (86.6%) and road traffic accidents were the primary cause of injuries (69.38%). Nasoorbitoethmoid complex was the most affected region (67.46%) followed by the maxilla (57.42%). The pattern and presentation of maxillofacial fractures had been studied in many parts of the world with varying results. Severe multiple trauma patients had different patterns of maxillofacial injuries. The number of maxillofacial trauma is on the rise worldwide as well as the incidence of associated sequelae. Maxillofacial fractures on multiple trauma patients were more frequent among males and in road traffic crashes. Knowing such data is elementary. The society should have a key role in the awareness of individuals and in prevention of road traffic accidents.
  • Rectrospective analysis of 186 cases of maxillofacial trauma from road traffic accidents

    Este estudo foi realizado no Hospital de Santo António - Centro Hospitalar do Porto e teve como objetivo caraterizar o tipo e a gravidade das lesões numa cohorte de politraumatizados graves, vítimas de acidentes de viação, com trauma maxilofacial. Este estudo caraterizou um total de 251 pacientes admitidos à sala de emergência, com traumatismos maxilofaciais entre agosto de 2001 e dezembro de 2007, dos quais 186 (74,1%) sofreram acidentes de viação. Usou o registo prospetivo que usa a metodologia TRISS para análise da gravidade. O critério de inclusão foi a presença de lesão anatómica na face. Um total de 159 (85,48%) doentes pertenciam ao sexo masculino, as idades variaram entre 13-86 anos e a faixa etária dominante foi a dos 15-19 anos (17,73%). A fratura da órbita (91; 48,92%), seguida da maxila (88; 47,31%) e do zigomático (66; 35,48%) foram as lesões nos tecidos duros mais frequentes. Os traumatismos crânio-encefálicos estavam presentes em quase todos os indivíduos (172; 92,47%). A redução aberta da fratura mandibular foi o tratamento mais frequente (16; 8,6%). A ocorrência de lesões maxilofaciais em pacientes politraumatizados foi mais frequente em indivíduos do sexo masculino e os veículos de 2 rodas foram a causa principal de acidentes de viação. This study was conducted in Hospital de Santo António - Porto Hospital Centre, and had the objective to characterize the type and severity of injuries in a cohort of severe multiple trauma, victims of car accidents, maxillofacial trauma. This study featured a total of 251 patients admitted to the Emergency Room with maxillofacial trauma between August 2001 and December 2007, of which 186 (74.1%) suffered road traffic accidents. Used the prospective registry using the TRISS methodology for analysis of gravity. The inclusion criterion was the presence of anatomical lesions on the face. A total of 159 (85.48%) patients were male, ages ranged from 13-86 years and the age range was the dominant of 15-19 years (17.73%). The orbital fracture (91; 48.92%), followed by the maxilla (88; 47.31%) and zygomatic (66; 35.48%) lesions were more frequent in the hard tissues. The traumatic brain injury trauma were present in almost all (172; 92.47%) individuals. Open reduction of mandibular fracture was the most common treatment (16; 8.6%). The occurrence of maxillofacial injuries in polytrauma patients was more frequent in males, and two-wheelers were the cause leading of road traffic accidents.

    Este trabalho caracterizou as lesões dentárias e maxilofaciais nos tecidos moles e duros num estudo cohorte de doentes politraumatizados graves. Utilizou o registo prospectivo de doentes politraumatizados que usa a metodologia TRISS para a análise da severidade. O critério de inclusão foi a presença de lesão anatómica na face, utilizando o Abbreviated Injury Scale decorrente de acidente de viação e trabalho. O estudo foi complementado com a consulta do processo clínico e a consulta de follow-up aos 6 meses. Entre 2002 e 2005 admitiram-se na Sala de Emergência do Hospital Geral de Santo António, 868 doentes politraumatizados graves. 151 tiveram lesão na face por acidente de viação (92%) e acidente de trabalho (8%). 85% pertencia ao sexo masculino e à faixa etária 15–19 anos de idade (18%). 37% das lesões localizaramse nos tecidos moles e a região mais atingida foi a órbita direita (14%). A região dos tecidos duros mais atingida foi a nasal (28%). A observação dos processos clínicos dos doentes complementou a informação do registo prospectivo mas só a entrevista aos 6 meses permitiu compreender a importância destas lesões no seu todo. Os registos das lesões desde o processo clínico à base de dados prospectiva, passando pela observação dos doentes no follow-up revelam que o trauma maxilofacial e principalmente dentário é pouco valorizado na avaliação inicial do doente. This study characterized the dental and maxillofacial injuries in hard and soft tissues in a cohort of severe trauma patients. Used the prospective registry of trauma patients that uses TRISS methodology for severity analysis. Inclusion criterion was the presence of anatomical lesion in the face, using the Abbreviated Injury Scale due to road traffic accident and work-related accidents. The study was complemented with the patient's registries and 6 months follow-up. Between 2002 and 2005 were admitted to the Emergency Room of Santo Antonio General Hospital, 868 severe trauma patients. 151 had lesions on the face due to traffic accident (92%) and accidents at work (8%). A total of 85% was males, and the age group 15–19 years of age dominated (18%). 37% of lesions were located in the soft tissues and the region most affected was the right orbit (14%). The region most affected in the hard tissue was the nasal (28%). The records of injuries from the clinical prospective data through observation of patients in follow-up show that the main dental and maxillofacial trauma is of little importance in the initial evaluation of the patient.
