Prevalence and associated factors with rhabdomyolysis Ana Paula de Souza Cunha, Bianca Oliveira Souza, Cícero Santos Souza, Danilo Miranda Bomfim, Marcos Vinicius Santos de Jesus, Gisele da Silveira Lemos, Rafael Pereira, Lucas Brasileiro Lemos Medicina Brazil, 2024 Introduction: Rhabdomyolysis (RB) is a frequent syndrome defined as skeletal muscle injurywhereby intramuscular molecules, such as myoglobin and creatine phosphokinase, are released into the bloodstream. This study aimed to identify the prevalence of RB and associated factors to this outcome in in-hospital patients. Methods: This is an observational study - cross-sectional, retrospective, with a descriptive and analytical approach. The sample was composed of all in-hospital patients from a Brazilian public hospital that presented serum creatine- phosphokinase measures recorded in medical records in the period from April 2020 to september 2021. Descriptive analysis was carried out with frequencies (absolute and relative) and means±standard deviation. The Chi-square test was used to infer the association to RB, and the variables presenting p-value ≤ 0.2 were included in a multivariate logistic regression technique applied to investigate the interaction among the selected independent variables and the RB. Results: Among 388 patients included in the present study, the prevalence of RB was 3.89% and the mortality rate was 39.43%. The main cause of hospitalization was circulatory system-related diseases. The frequency of RB was higher in males and in younger adults. The variables sex, age group, presence of comorbidities, trauma as the hospitalization cause, and history of recent exhaustive physical effort were significantly associated with RB. Conclusion: Our data share light on the profile of hospitalized patients prone to develop RB, a relevant clinical condition that could be unrecognized in many mild cases, but also lead to acute signals of renal failure. Additionally, our data brings a better understanding regarding mortality associated with RB, requiring greater attention from health professionals and managers.

This was a descriptive study with a quantitative and qualitative approach, carried out with 40 health professionals, who make up the multidisciplinary team of the ICU of a regional reference hospital of the state. Data collection was carried out between the months of May and December 2020. It took place through a semi-structured interview, guided by a thematic script and a sociodemographic questionnaire. Data were processed using IRAMUTEQ software, through Descending Hierarchical Classification, and were analyzed using the collective subject discourse technique. It was demonstrated that the lack of knowledge about the concept of patient safety, the lack of care protocols, interpersonal problems, the occurrence of adverse events, underreporting, and inadequate estimations are some of the factors that interfere with patient safety in the ICU. There is a need to resolve existing flaws, especially regarding personnel size, issues related to the interaction of the multidisciplinary team, and the implementation of institutional protocols, which guide care. By strengthening the safety culture, professionals feel empowered to report not only AEs, but also reportable circumstances.

Potential drug interactions in prescribed and non-prescribed medicinal products for hemodialytic patients Nara Jacqueline Souza dos Santos, Lucas Brasileiro Lemos, Paulo Henrique Ribeiro Fernandes Almeida, Adriana Alves Nery, Gisele Da Silveira Lemos Mundo Da Saude, 2018 O tratamento da Doença Renal Crônica (DRC) demanda grande quantidade de medicamentos, que pode aumentar a possibilidade de Interações Medicamentosas Potenciais (IMP), as quais podem ser encontradas com maior frequência quando se envolve a prática de automedicação. O estudo teve o objetivo avaliar as interações medicamentosas potenciais no tratamento farmacoterapêutico de pacientes portadores de Doença Renal Crônica hemodialíticos. Estudo transversal, com aplicação de formulário a 170 pacientes renais crônicos em uma clínica privada de hemodiálise, no período de março a maio de 2015; a análise das IMP foi feita através da base de dados Micromedex versão 2.0. Foram detectadas 604 IMP ao todo, em 74,7% da população do estudo, sendo 501 IMP entre os medicamentos prescritos, 85 entre os prescritos e não prescritos, e 18 entre os não prescritos (usados por automedicação). Houve predominância de IMP no sexo masculino (63,1%), em idade menor do que 60 anos (72,5%), e com tempo de hemodiálise menor do que 5 anos (55,5%). A análise de regressão logística demostrou que pacientes que usam cinco medicamentos ou mais têm 243 vezes mais chance de ter uma IMP (OR = 243,206, IC=95%) do que aqueles que usam menos de 5 medicamentos. O número de IMP detectadas neste estudo esteve diretamente relacionado ao número de medicamentos utilizados pelos portadores de DRC, mostrando um potencial fator causal entre a polifarmácia e IM, podendo a automedicação, concomitantemente, ter influenciado neste resultado. O conhecimento deste perfil de IMP, direciona maior atenção dos profissionais que acompanham o tratamento farmacoterapêutico destes pacientes.
Potential drug interactions in medical prescriptions containing restricted use antimicrobial, for patients in a hospital within the Bahia Danilo V. Piedade, Luciana A. F. Silva, Gisele S. Lemos, Gildomar L. Valasques Júnior, Lucas B. Lemos Medicina Brazil, 2015 Modelo do estudo: Estudo quantitativo, de natureza descritiva, com delineamento transversal, realizado no ano de 2013. Objetivo: Realizar uma avaliação das interações medicamentosas potenciais (IMP), em prescrições médicas contendo antimicrobianos de uso restrito, de pacientes internados em um hospital no interior do estado da Bahia. Metodologia: A coleta de dados foi realizada no período de janeiro a maio, empregando a análise documental, com acompanhamento prospectivo de prescrições contidas nos prontuários, para possibilitar a detecção das IMP. A ocorrência e a classificação das IMP foram verificadas através da base de dados MICROMEDEX® DrugReax System. Resultados: 50 pacientes participaram do estudo, sendo analisadas 797 prescrições médicas. Dentre o grupo dos antimicrobianos de uso restrito, a associação piperacilina + tazobactam foi a mais prescrita, estando presente em 33,0% das prescrições. Das prescrições utilizadas para análise das IMP, 357 (46,5%) apresentaram pelo menos uma IMP, sendo que destas, 7 (5,8%) envolviam antimicrobianos de uso restrito. Foram encontrados como fármacos envolvidos nas IMP: amitripitilina + linezolida (46,0%), piperacilina + warfarina (26,0%), entre outros. Do total de interações encontradas, 467 (31,9%) foram classificadas como sendo de gravidade moderada e documentação boa, 808 (53,5%) como interações de mecanismo de ação farmacocinético e 743 (50,8%) como interações de início de reação demorado. Conclusão: Foi realizada uma avaliação das IMP, em prescrições médicas contendo antimicrobianos de uso restrito, e a partir delas uma tabela de auxílio foi construída, criando subsídio para o uso racional de alguns antimicrobianos potencialmente envolvidos em interações medicamentosas.
Method validation to HMF determination in honey by HPLC-UV and its influence on the product quality Gisele da Silveira Lemos, José Soares dos Santos, Maria Lúcia Pires dos Santos Quimica Nova, 2010 In honey 5-hydroxymethyl-2-furaldehyde (HMF) is one of the most typical products of degradation: it is usually absent in fresh honey, but its concentration tends to rise as a result of heating processes or long-term storage. The validation protocol was performed in terms of detection and quantification limits, precision (by repeatability and intermediate precision), linearity and accuracy (by recovery tests). The method has been tested on 15 honey samples of different ages and geographical origin. HMF correlated highly with the age of the samples has been considered a very important parameter to put these honeys on the market or not and/or to estimate their shelf life.
