Long term outcomes of the Indian childhood cancer survivorship (C2S) cohort: a multicentre study (2016–2024) Rachna Seth, Gauri Kapoor, Nirmalya Roy Moulik, Prakruthi Kaushik, Nishant Verma, Amita Mahajan, Priyakumari T, Nita Radhakrishnan, Revathi Raj, Amitabh Singh, Sujata Sharma, Venkatraman Radhakrishnan, Sirishia Rani, Parthasarathi Bhattacharyya, Yamini Krishnan, Piali Mandal, Emine A. However, data on long-term outcomes and late effects are limited. The Indian Childhood Cancer Survivorship (C2S) Study was envisaged to build a nationwide survivor cohort and systematically capture treatment exposures and long-term outcomes. Methods: The C2S study is a prospective multicentric observational cohort initiated in 2016 under the Indian Pediatric Hematology Oncology Group (InPOG-LE-16-01). Children diagnosed with cancer before 18 years of age and in remission post-treatment were enrolled. Demographic, clinical, and treatment exposure data were collected at respective centres and monitored centrally. Follow-up was conducted every three months, assessing survival, relapse, and abandonment rates. Findings: As of December 2024, the study includes data from 20 centres across India with 5419 survivors being enrolled, with survival data available for 5140. Acute leukemia was the most common diagnosis (40.9%). Common therapeutic exposures included chemotherapy (94.7%), surgery (30.3%), and radiotherapy (26.3%). The 5-year overall survival (OS) and event-free survival (EFS) rates for the entire cohort were 94.5% (95% CI: 93.7-95.3)% and 89.9% (95% CI: 88.8-91.0)%, respectively. For the 2266 survivors with ≥2 years post-treatment follow-up, 5 y-OS and EFS were 98.2 (95% CI: 97.5-98.7)% and 95.7 (95% CI: 94.7-96.6)%, respectively. Interpretation: The C2S study represents the first structured attempt to build a nationwide childhood cancer survivors' cohort in India. This cohort will serve as a denominator for future research on late effects, support the development of survivorship guidelines, and inform policy planning in India and comparable LMIC settings. Funding: There was no dedicated funding for this study.

Acute Pancreatitis as an Initial Manifestation of Pediatric AML: A Case Report with Review of Literature Tanya Singh, Shilpa Khanna Arora, Alok Hemal Indian Journal of Medical and Paediatric Oncology, 2025 Acute myeloid leukemia (AML) accounts for around 15 to 20% of childhood leukemia. Acute leukemia has variable presenting features and extramedullary manifestations typically occur in 10 to 20% of patients with AML, even before bone marrow involvement. Therefore, leukemia may get unnoticed if symptoms from other organs predominate. Acute pancreatitis, as an initial manifestation of acute leukemia, is seen commonly during and after the course of chemotherapy. Here, we report the case of a 16-year-old female patient who presented with pain abdomen, multiple swellings in the neck, axilla, and breast for 2 months and had jaundice at presentation was diagnosed to be having acute pancreatitis with AML. Pancreatitis was resolved after starting cytoreductive chemotherapy. Hence, the causation of pancreatitis in the index case was likely due to pancreatic infiltration by the leukemic cells, unlike the common etiologies such as hypercalcemia and chemotherapy drugs seen in the setting of leukemia.
Unusual case of 'scorbutic proptosis' Shilpa Khanna Arora, Manisha Jana, Rachna Seth BMJ Case Reports, 2024 Proptosis is a frequent presenting symptom/sign of many paediatric malignancies. Acute-onset proptosis is an ophthalmic emergency that can endanger vision if not treated promptly. Appropriate treatment must be instituted only after investigating for the underlying aetiology. Here, we report a developmentally delayed boy in middle childhood who presented with recent onset bilateral proptosis. Clinical examination followed by radiological evaluation suggested scurvy to be the underlying cause and vitamin C supplementation led to prompt reversal of proptosis. The relevant literature has been reviewed and presented here to apprise the paediatric oncologists about this rare but easily treatable cause of proptosis.

Axillary lymph node in extra-ocular retinoblastoma - Benign or malignant? Shubha Gadde Ravindra, Sambit Sagar, Shilpa Khanna Arora, Rachna Seth, Neiwete Lomi, Rakesh Kumar Indian Journal of Nuclear Medicine, 2023 A 1-year-old male child presented with whitish discoloration of pupil of the left eye and swelling over the left axilla. A contrast-enhanced magnetic resonance imaging of the brain and orbits performed revealed left eye extra-ocular retinoblastoma. 18F-fluorodeoxyglucose positron emission tomography/computed tomography scan was done in this child as a part of baseline staging of retinoblastoma in an ongoing research project. The scan revealed left eye extra-ocular retinoblastoma along with calcified left axillary level I lymph node.
