The Taurus sign: A novel clinical sign for rapid diagnosis of patent vitello-intestinal duct with double intusussception Rafey Abdul Rahman, Survesh Kumar Gupta, Muniba Alim Tropical Doctor, 2026 A patent vitello-intestinal duct uncommonly presents as neonatal bowel prolapse. We describe a 20-day-old infant with double intussusception showing the Taurus sign, confirmed surgically and treated by resection and anastomosis with good recovery. This distinctive sign allows rapid bedside diagnosis, signals vascular compromise, guides surgery, and promotes urgent intervention, helping prevent ischaemic morbidity in this rare neonatal emergency, especially in resource-limited settings, thus improving early outcomes through timely surgical referral and management.
A low-cost pen-type neuromuscular electrical stimulator for intraoperative muscle complex localization in anorectal malformation surgery Rafey A Rahman, Survesh K Gupta, Anshuma Kant, Muniba Alim, Vinay Kanaujia Tropical Doctor, 2026 Accurate placement of the neo-anus within the sphincter complex is critical for continence in anorectal malformation (ARM) surgery. Intraoperative localization may be difficult, and dedicated stimulators are not always available. We describe the modification and clinical use of a low-cost pen-type neuromuscular electrical stimulator for intraoperative muscle complex localization, with successful localization achieved in all patients, enabling precise neo-anal placement without device-related complications or significant increase in operative time.
Breaking the cycle: Evidence-based insights from 500 SNCU admissions in rural India Ramesh Chand, Muniba Alim, Rafey Abdul Rahman, Neha Saroj, Nishant Sharma Tropical Doctor, 2026 India's Special Newborn Care Units are critical for reducing preventable neonatal deaths. Systematic evaluation of admission patterns provides evidence for targeted healthcare improvements. Birth asphyxia dominates rural neonatal morbidity. Focused investments in intrapartum monitoring, resuscitation training, and rapid referral systems are essential for reducing preventable deaths.
Glycerine soaked bandage drain: A novel technique for non-surgical management of neonatal rectal prolapse Survesh K Gupta, Rafey A Rahman, Muniba Alim, Umesh K Gupta Tropical Doctor, 2025 Neonatal rectal prolapse (NRP) is a rare, distressing condition in neonates, often linked to malnutrition, diarrhoea, and constipation. The effectiveness of glycerine soaked bandage drain for non-surgical management of idiopathic NRP is demonstrated.
Quality Assessment of YouTube Videos on Laparoscopic Pyloromyotomy Using a Validated Tool: An Appeal to Trainees to Follow the Peer-Reviewed Videos for Learning Purposes Sachit Anand, Rafey Abdul Rahman, Shishir Jadhav, Prabudh Goel, Vishesh Jain, Devendra Kumar Yadav, Anjan Kumar Dhua Journal of Laparoendoscopic and Advanced Surgical Techniques, 2022 Background: Surgical trainees often resort to operative videos on the internet during preparation for surgery; YouTube is the most commonly explored video source by them. However, the quality of these videos is often questioned. This study was performed to assess the quality of available YouTube™ videos on laparoscopic pyloromyotomy (LP). Materials and Methods: The term “laparoscopic pyloromyotomy” was searched on YouTube on June 2, 2021, and 20 most-viewed videos on LP were included. A reference video on LP from WebSurg was also selected. The laparoscopic surgery video educational guidelines (LAP-VEGaS) tool was utilized for quality assessment of these videos. Descriptive variables, including the surgeon's details; year of video upload; duration of the video; view, like, and dislike counts; number of ports used; and instruments used for pyloromyotomy, were recorded and their relationship with video quality was studied. Results: The majority of videos were from the United States and India, and a surgeon could be identified in 90% of them. The median (range) video duration and view, like, and dislike counts were 2.89 (0.68–8.80) minutes, 2308 (1102–23,682), 5 (0–59), and 0 (0–11), respectively. The LAP-VEGaS score of the reference video was 17. In contrast, the scores of YouTube videos ranged from 1 to 14. The quality of these videos was poor in 5/9 domains. None of the descriptive variables showed a significant association with high video quality. Conclusion: Compared with the reference video, the overall quality of YouTube videos on LP was poor. Therefore, until a screening tool is available for selection of high-quality YouTube videos, trainees must resort to peer-reviewed video platforms.
Diversity of spectrum and management of animal-inflicted injuries in the pediatric age group: A prospective study from a pediatric surgery department catering primarily to the rural population UmeshKumar Gupta, RafeyAbdul Rahman, Shashank Agrawal, Prabudh Goel, Muniba Alim Journal of Indian Association of Pediatric Surgeons, 2020 INTRODUCTION: Animal-inflicted injuries continue to be a major health problem worldwide. In developing countries, the outcome of such injuries, especially in children may be poor. AIM: The study aimed to evaluate the diversity of spectrum and management of animal-inflicted injuries in the pediatric age group. MATERIALS AND METHODS: This was a prospective study on animal-inflicted injuries in children between 1 to 15 years of age over a period of 12 months. Data on various parameters such as age and sex, animal species involved, provoked/unprovoked, mechanism of injury, time of injury, prehospital care, injury-arrival interval, pattern and type of injury, trauma score, body region injured, treatment given and complications were collected and analyzed. RESULTS: < 0.01) in delayed presenters. The length of hospital stay for the admitted children ranged from 3 to 28 days. CONCLUSION: Animal-inflicted injuries are rare in children and have a wide spectrum of presentation. Severe injuries require extensive resuscitation and expert surgical care. Mild injuries can be managed conservatively with the use of proper dressings, antibiotics, and analgesics.
Estimation of renal functional reserve in children with different grades of vesicoureteric reflux Veereshwar Bhatnagar, RafeyAbdul Rahman, Sandeep Agarwala, Rakesh Kumar Journal of Indian Association of Pediatric Surgeons, 2018 <br><b>Background:</b> Vesicoureteric reflux (VUR) is one of the most common anomalies encountered in pediatric urology. The concept of renal functional reserve (RFR) as the ability of the kidney to increase glomerular filtration rate (GFR) following a protein load was introduced in the 1980s.<br><b>Aim:</b> This study aims to evaluate RFR using 99Tc diethylenetriamine pentaacetic acid (DTPA) as the filtration agent for GFR estimation in children with VUR.<br><b>Materials and Methods:</b> RFR was estimated in 53 children, of which 31 patients had unilateral VUR (Group I) and 22 patients had bilateral VUR (Group II), by subtracting baseline GFR from stimulated GFR following an intravenous protein load. GFR was determined by double compartment-2 sample method using 99Tc DTPA radioisotope as the filtration agent. Both the groups were further subgrouped into low-grade (IA, IIA) and high-grade VUR (IB, IIB).<br><b>Results:</b> The RFR was significantly lower in unilateral high-grade VUR (Group IB) as compared to unilateral low-grade VUR (Group IA) (<i>P</i> = 0.024). RFR was significantly lower in bilateral high-grade VUR patients (IIB) as compared to unilateral low-grade VUR group (IA) (<i>P</i> = 0.0226). Furthermore, the stimulated GFR shows very strong correlation to baseline GFR in both major groups (<i>r</i> = 0.9659 and <i>P</i> = 0.001 in Group I and <i>r</i> = 0.9856 and <i>P</i> = 0.001 in Group II) concluding that the baseline GFR and the stimulated GFR increase or decrease in tandem in both the groups.<br><b>Conclusion:</b> The RFR is impaired in children with both unilateral high-grade VUR and bilateral high-grade VUR while it is relatively preserved in unilateral low-grade VUR and bilateral low-grade VUR.<br>
