- Real-time detection of extravasation events of diagnostic and therapeutic radiopharmaceuticals in oncology

Background Radiopharmaceutical extravasation—when injected activity remains in soft tissue rather than entering the bloodstream—can compromise quantification in positron emission tomography/computed tomography (PET/CT) and, in therapeutic settings, deliver high local radiation doses. These effects may lead to inaccurate Standardized Uptake Value (SUV) measurements, potential misdiagnosis, repeat examinations, and to avoid patient risk. We assessed a simple, real-time approach for detecting extravasation during administration to enable immediate management and quantitative correction. Results Real-time monitoring with portable gamma detectors was applied to 885 diagnostic PET examinations ( 18 F-FDG, 68 Ga-DOTATOC/TATE, 68 Ga-PSMA-11) and 15 administrations of 177 Lu-DOTATATE therapy. In the diagnostic cohort, 53 extravasation events were identified and confirmed on PET images. Analysis of dose-rate–time curves yielded practical thresholds for extravasation detection: Δp in nor = 0.30 and ΔR t = 388 µSv/h. A statistically significant association was observed between SUV correction factors and specific curve parameters, enabling prospective adjustment of SUV before image interpretation. In the therapeutic cohort, one extravasation was observed, with an estimated absorbed self-dose of 11.6 Gy to the affected region. The monitoring enabled timely intervention that limited unnecessary exposure and workflow disruption. Feasibility analyses indicated that, for diagnostic procedures, a single detector can be sufficient for reliable extravasation identification and for deriving SUV correction factors. Conclusions A real-time, detector-based strategy can identify radiopharmaceutical extravasation during injection, support immediate mitigation, and preserve diagnostic accuracy by allowing early SUV correction. In therapeutic applications, it can reduce risk by prompting management of rare but consequential events. The operational simplicity, low cost, and effectiveness demonstrated across both diagnostic and therapeutic scenarios support integration of this approach into routine clinical practice, with the added practicality that a single detector appears adequate for diagnostic workflows. - A Pipeline for MRI-Guided dose painting by number radiation Therapy: GLIORAD − a phase I-II clinical trial in recurrent glioblastoma

Purpose: Spatially fractionated radiation therapy (SFRT) is an irradiation technique developed to improve large cancer response. Although preliminary studies report highly positive results, data are still limited. The aim of this retrospective monocentric study was to investigate SFRT safety and activity. Methods and Materials: We analyzed all patients who underwent SFRT as a palliative treatment for large solid extracranial cancer (>4.5 cm) at our institution. The primary endpoint was objective response rate assessment at 3 months. Additionally, patients' antalgic response, target volume reduction, and performance status modification were measured. Toxicity data were recorded. Results: , 32%-69%; range, 6%-80%). At 6 months, all patients were free from local disease progression. All patients reported an antalgic response with a rapid onset. All treatment-related toxicities occurred within 1 month after SFRT and quickly recovered. No acute toxicity ≥ grade 3 and late toxicity was reported. No patient experienced a worsening in performance status. PURPOSE: STereotactic Arrhythmia Radioablation (STAR) showed promising results in patients with refractory ventricular tachycardia. However, clinical data are scarce and heterogeneous. The STOPSTORM.eu consortium was established to investigate and harmonize STAR in Europe. The primary goal of this benchmark study was to investigate current treatment planning practice within the STOPSTORM project as a baseline for future harmonization. METHODS AND MATERIALS: Planning target volumes (PTVs) overlapping extracardiac organs-at-risk and/or cardiac substructures were generated for 3 STAR cases. Participating centers were asked to create single-fraction treatment plans with 25 Gy dose prescriptions based on in-house clinical practice. All treatment plans were reviewed by an expert panel and quantitative crowd knowledge-based analysis was performed with independent software using descriptive statistics for International Commission on Radiation Units and Measurements report 91 relevant parameters and crowd dose-volume histograms. Thereafter, treatment planning consensus statements were established using a dual-stage voting process. RESULTS: ranged from 25.5 to 34.6 Gy, demonstrating a large variety of dose inhomogeneity. Estimated treatment times without motion compensation or setup ranged from 2 to 80 minutes. For the consensus statements, a strong agreement was reached for beam technique planning, dose calculation, prescription methods, and trade-offs between target and extracardiac critical structures. No agreement was reached on cardiac substructure dose limitations and on desired dose inhomogeneity in the target. CONCLUSIONS: This STOPSTORM multicenter treatment planning benchmark study not only showed strong agreement on several aspects of STAR treatment planning, but also revealed disagreement on others. BackgroundChemoradiation therapy (CRT) is the treatment of choice for locally advanced non-small cell lung cancer (LA-NSCLC). Several clinical trials that combine programmed cell death 1 (PD1) axis inhibitors with radiotherapy are in development for patients with LA-NSCLC. However, the effect of CRT on tumor cells programmed cell death ligand-1 (PD-L1) expression is unknown.MethodsIn this multicentric retrospective study, we analyzed paired NSCLC specimens that had been obtained pre- and post-CRT. PD-L1 expression on tumor cells was studied by immunohistochemistry. The purpose of this study was to evaluate the feasibility, risk of complications, and clinical relevance of performing re-biopsy after CRT in patients with PD-L1 negative LA-NSCLC.ResultsOverall, 31 patients from 6 centers with PD-L1 negative LA-NSCLC were analyzed. The percentage of tumor cells with PD-L1 expression significantly increased between pre- and post-CRT specimens in 14 patients (45%). Nine patients had unchanged PD-L1 expression after CRT, in five patients the rebiopsy material was insufficient for PD-L1 analysis and in two patients no tumor cells at rebiopsy were found. The post-rebiopsy complication rate was very low (6%). All patients with positive PD-L1 re-biopsy received Durvalumab maintenance after CRT, except one patient who had a long hospitalization for tuberculosis reactivation. Median PFS of patients with unchanged or increased PD-L1 expression was 10 and 16.9 months, respectively.ConclusionCRT administration can induce PD-L1 expression in a considerable fraction of PD-L1 negative patients at baseline, allowing them receiving the maintenance Durvalumab in Europe.
