  • Impact of prehypertension on left ventricular structure, function and geometry

    BACKGROUND Awareness of prevalence, determinants, and prognosis of asymptomatic untreated prehypertension is still lacking especially in India and subcontinent. The present study was to assess the effects of prehypertension on structure, function and geometrical pattern of left ventricle on the basis of left ventricular mass (LVM), left ventricular mass indexed to height (LVMI/Ht), and relative wall thickness (RWT) recorded by echocardiography based on the American society of echocardiography (ASE) convention. METHODS The study population included prehypertensives (n 61; 31 M, 30 F) and normotensives (n 38; 19 M, 19 F) between age 25 and 65 years, and were assessed by echocardiography. RESULTS It was observed that the stroke volume (SV), cardiac output (CO), cardiac index (CI), body mass index (BMI), body surface area (BSA), were found to be little elevated but was not significant in hypertensive females compared to normotensives. Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), pulse pressure (PP), mean arterial pressure (MAP), end systolic stress (ESS), and end isovolumetric systolic stress (EISS) were significantly elevated (p<0.001) in female prehypertensives compared to normotensives. Left ventricular mass (LVM) was significantly (p< 0.05) elevated, indicating alterations in cardiac morphology and functions even during prehypertensive stage. However, in prehypertensive males, SBP, DBP, HR, PP, MAP, ESS, and EISS were significantly (<0.001) raised; ejection fraction (EF%) and fractional fibre shortening (FS%) were noted to be within normal range in both sexes. Prehypertensive males showed changes in left ventricular geometry in the form of concentric remodeling (CR-3.44%), eccentric hypertrophy (EH-3.44%) and concentric hypertrophy (CH-13.79%). Prehypertensive females showed (CR-6.45%), (EH-3.22%) and (CH-6.4%). CONCLUSION Such findings carry prognostic implication and require further population survey involving a larger group. Early diagnosis of prehypertension will help to take necessary preventive measures to reduce mainly the future cardiovascular complications. The care of prehypertensive subjects should include, to reduce the afterload in order to improve the left ventricular contractile state as early as possible. So it is advisable to do routine echocardiography after the age of 40 years.
  • Asymptomatic obese hypertensives and need of routine echocardiography for left ventricular mass assessment and treatment

    BACKGROUND Echocardiographic determination of Left Ventricle Mass (LVM) - an important marker of cardiovascular disease, has been given a lot of importance in clinical diagnosis and in planning of treatment. Clinically asymptomatic compensated hypertensives show some pathological findings which are indicative of left ventricular dysfunction. METHODS The study population of 106 males, after a detailed clinical examination, were evaluated by echocardiography and were classified as per the body mass index classification of WHO Western Pacific Region in 2000 for Asian population. Fasting blood samples were taken to estimate blood sugar and lipid profile. RESULTS It was observed that subjects in normal range of body mass index <45 years (23.68%) and >45 years (16.1%), subjects of overweight <45 years (15.7%) and >45 years (10.29%) and obese I and II<45 years (60.52%) and >45 years (73.52%). The comparison between left ventricular mass which was indexed to height(2.7) in subjects who were <45 years and >45years was observed to be statistically significant (p<0.03). On comparing LVM/ht(2.7) of normal BMI group with that of those with higher BMIs, it was noted to be significantly different (p<0.009), which was suggestive of adverse effects of increasing BMI on LVM. It was also observed that persons with increased BMIs showed changes in left ventricular geometry - 30.13% had concentric hypertrophy, 17.80% had concentric remodeling, 8.21% had eccentric hypertrophy and that 38.35% had normal left ventricle geometry. CONCLUSION The present study therefore, indicated that it was better to do an echocardiographic screening of asymptomatic subjects who had even a marginal increase in blood pressure and BMI, to diagnose potential cardiac dysfunction.
  • Estimation of serum ferritin - A better screening test for blood donors

    Background: Haemoglobin and haematocrit estimation are the commonest methods used worldwide in Blood Banks to screen donors. In order to detect Non-anaemic iron deficient state in repeat donors, these tests are inadequate. Several studies have reported a high incidence of iron deficiency in repeat donors. Aims: The present study was undertaken to assess routine haematological parameters and body iron stores of blood donors in order to identify those who were potentially prone to develop iron deficiency anaemia. Method: Predonation haemoglobin, haemogram and serum ferritin were done in 116 male donors. These were divided into two groups on the basis of number of donations. Results: First time donors (81.07±97.12) had higher mean serum ferritin level than those in repeat donors (46.01±49.09). 10.52% of first time donors and 27.5% of repeat blood donors were found to be iron deficient as indicated by serum ferritin level &lt;12 ng/ml. In addition a higher RBC count, reticulocyte % and lower MCV were noted in repeat donors. Conclusion: We concluded that haemoglobin estimation was not adequate to detect iron deficient non anaemic state in repeat blood donors. Serum ferritin proved to be a better investigation to detect the same and should be done in repeat donors. Iron supplementation for an adequate period post donation is recommended.
