In metastatic disease, evidence regarding the efficacy of chemotherapy (CT) combined with bevacizumab or anti-EGFR agents is limited to small studies. This study aimed to assess, in real-world practice, the effectiveness of first-line CT combined with targeted therapies (TT)-either antiangiogenic (AA) or anti-EGFR-compared with CT alone in patients with metastatic SBA (mSBA) and proficient mismatch repair/microsatellite stable (pMMR/MSS) status. PATIENTS AND METHODS: This retrospective multicentre study included all patients receiving at least one cycle of CT with or without TT for mSBA. The primary endpoint was progression-free survival (PFS); secondary endpoints included overall survival (OS), objective response rate (ORR), and safety. Analyses used inverse probability of treatment weighting (IPTW) in univariable Cox models to account for potential confounding factors that were unbalanced between groups. RESULTS: A total of 255 patients were included: 153 received CT alone, 45 CT+AA, and 16 CT+anti-EGFR as first-line therapy. Median PFS was 8.0 months with CT alone versus 11.9 months with CT+AA (IPTW HR=0.38; 95% CI: 0.29-0.49; p < 0.0001). Median OS was 15.9 versus 23.1 months (IPTW HR=0.38; 95% CI: 0.28-0.51; p < 0.0001). ORR did not differ significantly (33.8% vs 43.2%; p = 0.26). Among 78 patients with RAS wild-type tumours, outcomes did not significantly differ between CT alone and CT+anti-EGFR groups. CONCLUSION: Adding antiangiogenic therapy to CT significantly improved PFS and OS in first-line treatment of mSBA, warranting confirmation through prospective randomized trials.
Perineural invasion is a prognostic factor in cholangiocarcinoma, regardless of anatomical location: A systematic review and meta-analysis Sefora Conti, Natalia S. Tian Jhep Reports, 2026 BACKGROUND & AIMS: Cholangiocarcinoma (CCA), a heterogeneous malignancy that includes intrahepatic (iCCA), perihilar (pCCA), and distal (dCCA) subtypes, often exhibits perineural invasion (PNI). Despite PNI being associated with postoperative recurrence, evidence on its prognostic impact remains inconsistent. To clarify this, we conducted a systematic review and meta-analysis. METHODS: A comprehensive literature search was conducted across the Medline-PubMed, Embase, and Web of Science databases from their inception through February 27, 2025. We included studies involving patients diagnosed with CCA who underwent surgery and for whom PNI data were available. The primary outcomes were overall survival (OS) and disease-free survival (DFS), extracted from both univariable and multivariable analyses. The study is registered in PROSPERO (CRD42024575458). RESULTS: = 70.5%)). Prognostic consistency was observed across anatomical subgroups. Univariable OS HRs were 2.14 for iCCA, 1.69 for pCCA, and 1.66 for dCCA. Corresponding multivariable HRs were 1.74, 1.60, and 1.66, respectively. CONCLUSIONS: PNI is a prognostic factor for both DFS and OS in CCA, regardless of anatomical location. Further research is needed to elucidate its biological mechanisms and potential therapeutic implications. IMPACT AND IMPLICATIONS: This comprehensive review and meta-analysis confirms that perineural invasion (PNI) predicts poorer overall and disease-free survival in cholangiocarcinoma, independent of tumor location. These results highlight the potential of PNI as a reliable histopathological marker to stratify postoperative risk and inform patient management. While most evidence is retrospective and smaller studies may overstate the effect, the consistent findings emphasize the need for large, multicenter prospective studies with standardized pathological assessment. Such efforts could facilitate the integration of PNI into prognostic models and support more individualized treatment strategies for patients with cholangiocarcinoma.

Lately, <50 years of age has been rising, presenting unique challenges and clinical features that distinguish them from late-onset cases. Here, we review the clinical, biological, and molecular characteristics of early-onset biliary tract cancer and pancreatic cancer and compare them with their late-onset counterparts. We also examine treatment patterns, efficacy, and the potential role of targeted therapies, as well as the potential psychological and social effects on younger patients, with particular emphasis on fertility. We therefore offer insights into the unique challenges and therapeutic strategies in managing early-onset disease.

The association with Lynch syndrome (LS) is documented but remains under-researched. However, there are implications for prevention, genetic counseling, and therapeutic approaches. The objective of this study is to conduct a descriptive cohort of patients with high-grade glial tumors in the context of LS. Methods We included adult patients with glioblastoma (GBM) or grade 4 astrocytoma (WHO 2021 classification) associated with LS, diagnosed at Lille University Hospital or Valenciennes Hospital between 2014 and 2022. We retrospectively collected clinical, radiological, histopathological, molecular, and therapeutic data. Results We included 6 GBM cases with a median age of 58.7 years (IQR 32.4-63.3). Five cases had MSH2 mutations, and one had PMS2 mutation. In one case, MMR protein expression was preserved, and the RER phenotype showed low microsatellite instability. Loss of ATRX expression, overexpression of p53, and giant cells were observed in 50%, 83%, and 66% of cases, respectively. TP53 mutations were found in all cases, and PTEN mutations in 4 cases. Immunotherapy was given to two cases. The 24-month overall survival rate was 50%. Conclusions GBMs associated with LS exhibit specific histopathological and molecular biology characteristics that may guide syndrome-related research. These tumors could represent a particular subclassification, potentially leading to specific therapies such as immunotherapy. We propose with a review of case in the literature an algorithm for investigating LS upon discovering a GBM. These exploratory results need to be confirmed by a larger cohort.

A netrin1 antibody (NP137)3 has been shown to inhibit tumour EMT in preclinical1 and clinical4 settings. In animal models of pancreatic cancer, netrin1 and its receptor neogenin have been shown to promote tumour progression5, EMT5 and metastasis6. Here we report the results of a phase 1b study that assesses the combination of NP137 with modified FOLFIRINOX (mFOLFIRINOX) in first line patients with locally advanced pancreatic cancer (ClinicalTrials.gov: NCT05546853 ). Forty-three patients were enrolled and received mFOLFIRINOX plus NP137 every other week for up to 12 cycles. NP137 was well tolerated. Median progression-free survival (PFS) was 10.85 months (95% confidence interval, 10.03–15.61) and median overall survival was 16.43 months (95% confidence interval, 12.75–non-reached), with 21 patients remaining alive at the time of data cut-off. Post-therapy conversion surgery occurred in 23% of patients. Laser capture microdissection was performed on pre-therapeutic biopsies and surgical specimens. Microbulk RNA sequencing confirmed that the main pathway that was down-regulated with the combination of mFOLFIRINOX plus NP137 was EMT. Moreover, survival outcomes were extended for patients with tumour cells that expressed high levels of the netrin1 receptor neogenin—median PFS 15.65 months in neogenin-high versus 10.22 months in neogenin low. Our results support the idea that netrin1 blockade alleviates resistance to chemotherapy by inhibiting EMT, particularly in neogenin-high pancreatic cancer. The Lap-NET1 phase 1b trial reports that combined netrin1 antibody (NP137) plus modified FOLFIRINOX therapy on patients with locally advanced pancreatic cancer was well tolerated and improved progression-free survival and overall survival via inhibition of epithelial–mesenchymal transition.

However, since the early 2000s, epidemiological studies have reported a rise in the incidence and mortality of early-onset colorectal cancer (diagnosed before the age of 50) worldwide, in Europe, and in France. This increase has raised worldwide concern about the public health impact of early-onset colorectal cancer. Some countries have already decided to implement organized screening from the age of 45, or are actively considering it. In France, the increase in incidence and mortality could accelerate in the coming years, underscoring the need to take immediate action and strengthen prevention and screening for early-onset colorectal cancer. The question of lowering the recommended starting age for screening also requires careful consideration and preliminary studies.
