- Female genital arousal in women using combined hormonal contraceptives

PURPOSE: To correlate the sexual arousal and vaginal lubrication scores with free androgen index (FAI) and clitoral blood flow in women using different types of hormonal contraceptives (HCs) containing ethinylestradiol (EE), oestradiol valerate (E2V), 17β-oestradiol (E2), or estetrol (E4), combined with different progestogens having antiandrogenic properties. METHODS: One hundred and thirty-two women (age range 18-41) participated in the study. The Female Sexual Function Index (FSFI) and the Female Sexual Distress Scale (FSDS) questionnaires were used to assess sexual function and sexual distress, respectively. Sex homnone-binding globuline (SHBG) and total testosterone (TT) were measured, and FAI was calculated. Translabial colour Doppler ultrasound was adopted to measure the pulsatility index (PI) of clitoral arteries. RESULTS: Arousal and lubrication scores were lower in women on HCs with EE than in those using combined oral contraceptives (COCs) containing E2V, 17β E2, or E4. The lowest FAI percentages were achieved by women using combined hormonal contraceptives (CHCs) containing EE, while the highest ones were found in women using COCs with natural and native oestrogens. Sexual arousal and lubrication scores had a positive correlation with FAI. Finally, COCs with native and natural oestrogens produced lower PI values than CHCs containing EE. Moreover, clitoral artery PI values were negatively correlated with FAI, with a stronger negative correlation obsterved for COCs containing natural and native oestrogens conpared with those contanining EE. CONCLUSIONS: Unlike what was observed in women who were using COCs containing E2V, 17 E2, or E4, a minority of those on CHCs with EE might experience decreased sexual arousal and vaginal lubrication. - Contraception for adolescent and young women. OBJECTIVE: This consensus aimed to establish a consensus on hormonal contraception for adolescents and young women by synthesising current evidence and expert opinions, ultimately forming an evidence-based framework to guide clinical decision-making for methods approved in Italy. METHODS: An expert panel convened by the Italian Society of Contraception conducted a systematic review of the literature regarding contraceptive efficacy, cycle control, hyperandrogenism, sexual function, mood, bone health, and vaginal health. Consensus statements were debated and refined iteratively during the Society's 15th National Congress. Approximately 150 clinicians cast anonymous electronic votes, requiring a consensus of at least 75% to finalise each statement. RESULTS: All statements exceeded the 75% consensus threshold. Recommendations emphasise the prioritisation of long-acting reversible contraceptives as primary choices for adolescents due to their enhanced efficacy and adherence rates. The panel highlighted the importance of anticipatory counselling regarding bleeding patterns and other side effects to reduce discontinuation rates. Combined hormonal methods have been proven effective for managing acne, with recommendations offered for selecting formulations that enhance sexual health, stabilise mood, and consider bone metabolism. CONCLUSIONS: Selecting a hormonal contraceptive for young women requires a personalised evaluation that considers pregnancy prevention, tolerability, and additional non-contraceptive advantages. This consensus provides Italian clinicians with a structured, evidence-based framework to facilitate personalised counselling and encourage the sustained, safe use of contraceptives. - Quality of life, sexual function and metabolic parameters of women during 10 years of using the etonogestrel implant

Objective: To study the effects on the quality of life (QoL), sexual function and metabolic aspects in women using a subcutaneous implant containing Etonogestrel (ENG-implant) over 10 years of observation period. Study design: This study was performed from January 2011 to September 2023. A database was set up to collect data from women. Counseling on the ENG-implant to be inserted at the termination of pregnancy, and on insert a new device every 3 years was adopted. The Short Form-36 (SF-36), the Female Sexual Function Index (FSFI) and the Female Sexual Distress Scale (FSDS) questionnaires were used to investigate the QoL, sexual function and sexual distress, respectively, of women at baseline and at 6, 12, 24, 36, 72 and 108-month follow-ups. Blood samples were collected from women to measure metabolic parameters. Results: 32 of 52 (61.5 %) enrolled women completed the study. In summary, after removing the first (at 36th month of use) and the second implant (at 72nd month of use), 44 (84.6 %) and 32 (61.5 %) women asked for the insertion of a new device, respectively. Somatic (p < 0.001) and mental (p = 0.04) categories of SF-36 improved from the 12-month follow-up to the 108-month follow-up (p < 0.001). FSFI scores also increased from 12-month follow-up to the 108-month follow-up (p < 0.001). The improvement of the total scores depended on the gradual increase in each FSFI item (p < 0.001).FSDS score became statistically significant starting from the 12-month follow-up to the 108-month follow-up (p < 0.001). Although lipid metabolism, fasting glucose, liver function and hemostatic parameters were slightly reduced, they remained within the range of the normal values. Conclusions: The long observation period of 10 years allowed us to state that the ENG-implant could have neutral effects on metabolic parameters, promote a better quality of life, and improve sexuality in the users. The main limitation of our study was the small number of women enrolled, mainly due to the cost of the implant. The specific Italian Group of Study of the Menopause formulated a consensus opinion on the use of estrogen therapy (ET) or combined estro-progestin hormone therapy (HT) after breast and gynecological cancers. This consensus is based on the risk of recurrence of the specific cancer during ET/HT, the presence of steroid receptors in cancer cells, the use of adjuvant hormone therapies and data on the use of ET/HT after cancer. The following positions were reached. ET/HT can be used after vulvar cancers and melanoma, but with great caution after the rare adenocarcinomas. ET/HT can be used after cervical cancer, but ET should be used with caution after adenocarcinomas. ET/HT can be used after International Federation of Obstetrics and Gynecology (FIGO) stage I-II estrogen-dependent endometrial cancers, except in Black women, and can probably be used after estrogen-independent endometrial cancers. ET/HT cannot be administered or should be used with great caution after most uterine sarcomas. ET/HT can probably be used after ovarian neoplasms except for granulosa cell tumors, and with great caution after low-grade serous ovarian carcinoma and serous borderline ovarian tumors. ET/HT can be used with great caution in women after estrogen receptor (ER)/progesterone receptor (PR)-positive breast cancer and is probably allowed after ER/PR-negative breast cancer. - Correlation of sexual desire with sexual hormone binding globulin and free androgen index in women using combined contraceptives

PURPOSE: To correlate the sexual desire levels with sexual hormone binding globulin and free androgen index in women taking different types of hormonal contraceptives (HCs) containing ethinylestradiol (EE), oestradiol valerate (E2V), 17β-oestradiol (E2), or estetrol (E4), combined or in phasic formulation with different progestogens having antiandrogenic properties. METHODS: Three hundred and sixty-seven women (age range 18-46) participated in the study. SHBG and total testosterone (TT) were measured, and the Free Androgen Index (FAI) was calculated. The Female Sexual Function Index (FSFI) and the Female Sexual Distress Scale (FSDS) questionnaires were used to assess sexual function and distress, respectively. RESULTS: ≤ 0.0001). CONCLUSIONS: A minority of women using HCs with EE might experience a decreased sexual desire. This was not observed in women on HCs containing E2V, 17 E2, or E4. OBJECTIVE: To evaluate the efficacy of daily vaginal gel containing hyaluronic acid (HA) and Biosaccharide Gum-1 (BG-1) on vulvovaginal atrophy (VVA) and on sexual function and quality of life (QoL). MATERIALS AND METHODS: One hundred-four postmenopausal women with VVA were enrolled in the nonrandomized comparison cohort study. Of them, 50 women on HA/BG-1 participated in the study group and 54 women on lubricants/moisturizers on-demand as a control group. The primary endpoint was the efficacy of the vaginal gel on VVA evaluated by the Vaginal Health Index (VHI) score. Secondary endpoints included sexual behavior by the self-administered female sexual function index (FSFI) questionnaire, and quality of life (QoL), by the Short Form-36 questionnaire (SF-36). RESULTS: All symptoms of AVV improved after 12 weeks of treatment in women on HA/BG-1. The VMI, although improved at the 12-week follow-up compared to baseline, it connoted a low estrogenic stimulation value. Sexual function improved significantly in women on HA/BG-1. Moreover, women reported a significant improvement in the somatic aspects of QoL. No benefits were obtained by the women in the control group. CONCLUSIONS: Treatment with HA/BG-1 could have used in postmenopausal women who complain of vaginal dryness.
